The purpose of this Standard Operating Guideline (SOG) is to define and outline the
roles and responsibilities of the UNCH AHA Training Center.
UNC Healthcare-CTC is part of a network established by the AHA to serve the
employees of UNC Healthcare and its affiliates, broaden the outreach for Basic and
Advanced courses and to further prepare citizens and organizations to strengthen
the Chain of Survival. The CTC is responsible for:
I. Program administration and course quality
a. Serve as quality assurance and educational leadership for the Training
Center;
b. Monitor, update and coach instructors;
c. Assists with at least one instructor course every two years as requested
by the Training Center Coordinator.
d. Develop, monitor and update instructors under the direction of the Training
Center Coordinator
II. Instructor resource management:
a. Training instructors
b. Managing alignment of instructors
c. Providing instructor and class reporting tools
d. Monitor, update and coach instructors
III. Effective communication with instructors, students and designated Training Sites
regarding guidelines, policies, procedures, and course updates; and
IV. Serving as the key resource for information, support and quality control for all
AHA instructors aligned with the UNC-CTC.
V. Providing resources for expanding community CPR initiatives.
VI. Sets fees for services.
VII. Provides a mechanism for conflict and dispute resolution.

The purpose of this Standard Operating Guideline (SOG) is to provide a strategy for oversight and
monitoring of AHA classes, by defining “teaching sites” and identifying personnel authorized to
conduct courses.
I. All ALS program courses must be taught by approved AHA Instructors who are affiliated with the
UNC-CTC.
a. Occasional use of subject matter experts who are not certified AHA instructors are
permitted, however limited to no more than ten percent of total instructors for a given
course.
b. Use of adjunct AHA instructors aligned with other AHA Training Centers is approved.
The Lead Instructor must provide non-aligned instructors with AHA Instructor
Reporting Forms which they can submit to their Training Center as evidence of the
teaching activity.
I. Basic Life Support courses may be taught at any site that has sufficient space and provides
convenient access to students. All Basic Life Support teaching sites must have the following:
a. Sufficient classroom space to accommodate the total number of students enrolled;
b. Audio and video equipment sufficient to run the AHA course DVD and with a visible
viewing area large enough to accommodate the class;
c. Sufficient space to perform skills testing. This area must be capable of providing
students with a reasonably undisturbed skill testing experience; and
d. Minimum equipment list as stated in the pertinent instructor manual.
II. All Advanced Life Support courses are coordinated by the UNC-CTC ALS Program Manager.
a. The ALS Program Manager may assign a Lead Instructor other than him/herself but
must be available to provide assistance and consultation to the Lead Instructor if
needed.
III. With the exception of approved Training Center Sites, all Advanced Life Support courses will be
conducted at the UNC-CTC Training Center.

The purpose of this Standard Operating Guideline is to provide official
documentation regarding the validity of American Heart Association (AHA) courses
and to outline what credit is made available to students participating in courses
sponsored by UNC.
I. Courses offered within the Advanced Life Support (ALS) program are American
Heart Association programs of various disciplines.
II. All ALS program courses are conducted in-accordance with the most current
AHA guidelines and meet ALL standards outlined in the most current edition of
the AHA Program Administration Manual.
III. Students successfully completing an AHA course receive a completion card valid
for 2 years through the end of the month in which the card was issued.
a. The American Heart Association requires that students must receive their
AHA class completion cards no more than 20 days after completing the
class.
i. It is the belief of the UNCH ALS Programs AHA Training Center
Manager that students are entitled to receive their course
completion cards as quickly as possible.
IV. To successfully complete any American Heart Association program:
a. Students must attend the entire class.
i. Students who arrive late, after the class begins must be turned
away or the instructor may elect to “make up” the missed materials
with the student.
b. Students must successfully complete any associated cognitive and
performance examinations.

The purpose of this Standard Operating Guideline (SOG) is to develop a uniform
and effective Quality Assurance (QA) plan for all ALS Programs at UNC.
I. QA continues to be the key to effective ECC training programs. To ensure proper
QA/Monitoring, the UNC-CTC and its instructors will:
a. Use only current AHA written exams, skill evaluations and content in all
courses that require testing for issuance of a course completion card;
b. Ensure each student has the most current AHA textbook before, during
and after courses, as well as the accompanying CD, when applicable;
c. Develop and incorporate a written strategy for monitoring and renewing
affiliated instructors;
d. Maintain security of course cards and written examinations through
locking cabinets and daily on-site supervision;
a. Examination materials must be stored in a locking area.
b. Examination materials cannot be posted to:
i. Website
ii. Open, unsecure network drives
e. Maintain adequate resources to complete all course requirements. This
includes, but is not limited to, equipment, staff and budget;
f. Develop and incorporate a written internal dispute resolution policy;
g. Ensure all course records are properly completed and filed;
h. Develop and incorporate a strategy for monitoring courses taught at
remote sites; and
i. Develop and incorporate a mechanism to evaluate classes, instructors
and program administration

The purpose of this Standard Operating Guideline (SOG) is to define and outline the ALS
Programs/AHA Training Center methodology for conflict/dispute resolution..
I. It is conceivable that instructors or Training Site Coordinators may be faced with conflicts
involving (but not limited to) the following:
a. ALS Programs/AHA/TC-Site class attendance policies
b. ALS Programs/TC-Site dress code
c. Student behavior
d. Testing, scoring and remediation.
II. In the event such conflict arise at the class level:
a. The Lead Instructor/ Class Coordinator should be the first line in conflict resolution.
i. The LI/CC should make every attempt to resolve the issue using, as reference,
the AHA Program Administration Manual, UNCH ALS Programs SOG document
and/or the AHA Instructor manual for the discipline taught.
b. If the conflict/dispute cannot be reconciled by the LI/CC, the issue should be referred to
the Training Site Manager for approved Training Sites.
c. If a TC-Site is not involved or cannot reconcile the conflict/dispute, the issue should be
referred to the UNCH ALS Program Training Center Manager.
d. If a ALS Program Training Center Program Manager cannot reconcile the conflict/dispute,
the issue should be referred to the UNCH Director of Nurse Practice and Professional
Development.
e. At any time during the process of conflict/dispute resolution, the student or instructor
initiating the complaint may take the issue directly to the American Heart Association.
i. The person initiating the complaint should be made aware, up front, of this
option.
III. The resolution of any conflict should be handled with due regard for professionalism and
courtesy. Instructors, TC-Site Coordinators, the AHA Training Center Manager and others
involved should, at all times, be cognizant that they represent the UNCHCS.
IV. All conflicts/disputes should be resolved as quickly and efficiently as possible.
V. All conflicts/disputes and the process of resolution should be reported to the ALS Programs AHA
Training Center Manager within 14 business days of the conflict/dispute.
a. The ALS Programs AHA Training Center Manager may be contacted by the American
Heart Association about any conflict/dispute and should have all necessary information to
defend the resolution made by instructors, Site Coordinators, etc.

The purpose of this Standard Operating Guideline (SOG) is to define and outline the
roles and responsibilities of Training Center Faculty (TCF) at UNC.
I. TCF is defined as an instructor, affiliated with the UNC Training Center who has
met all AHA requirements.
II. TCF are responsible for quality assurance and are the educational leadership for
the Training Center instructors.
III. TCF perform duties for the Training Center beyond those of standard instructors.
These duties include:
a. Serve as quality assurance and educational leadership for the Training
Center;
b. Monitor, update and coach instructors;
c. Assists with at least one instructor course every two years as
requested by the Training Center Coordinator; and
d. Develop, monitor and update instructors under the direction of the
Training Center Coordinator.
IV. Monitors a minimum of four courses annually taught by instructors and training
sites. These cannot be courses in which the TCF are affiliated instructors.
a. Completes appropriate Training Center – Course Monitoring Forms
and submits them to the Training Center Coordinator.
V. Monitors instructors for renewal.
a. Completes appropriate Instructor Monitoring Form and submits them to
the Training Center Coordinator.
VI. Administers provider written exams to instructors as part of their renewal
process.
a. Submits the scored provider exam to the Training Center Coordinator

The purpose of this Standard Operating Guideline (SOG) is to create a standard fee
structure for AHA Training Center services provided to UNC affiliated and non-
affiliated persons.
I. Fees for service are set by the manager of the UNCH Advanced Life Support
Program.
II. Fee are charged for the following services:
a. Printing and issuance of AHA class completion cards.
b. Reprinting cards due to loss.
c. Class enrollment
d. Training Center alignment
e. Annual Training Center membership fees
f. Instructor records transfer fee
III. A complete listing of ALS Programs AHA Training Center fees are available upon
request.

The purpose of this Standard Operating Guideline (SOG) is to define the various
types of Instructors affiliated with UNC and to briefly establish the roles and
responsibilities of UNC Training Center Faculty. Refer to SOG_ALS 2011.7, Role of
Training Center Faculty for more information.
I. An Instructor is defined as any persons affiliated with the UNC AHA Training
Center to provide AHA programs to the staff of UNC Health Care Systems, its
affiliates, the Community and to healthcare providers not affiliated with UNC
HCS. The types of Instructors affiliated with UNC AHA Training Center is as
follows:
a. Affiliated Instructor is defined as any instructor who is affiliated directly
with UNC HCS or its affiliates.
b. Community Instructor is defined as any instructor who is not affiliated with
UNC HCS or its affiliates but is aligned with the UNC Training Center to
teach AHA programs to the community. Primarily, these instructors do not
charge a fee for their service beyond that which the UNC Training Center
charges the instructor.
II. Training Center Faculty (TCF) is defined as an instructor, affiliated with the UNC
Training Center who has met all AHA requirements Training Center Faculty
requirements. TCF staff performs duties for the Training Center beyond those of
standard instructor.
These duties include: monitoring of instructors, monitoring the quality of classes,
teaching instructor courses and testing/evaluating instructors for renewal of AHA
instructor credentials.

This Standard Operating Guideline (SOG) is developed to establish a standard for
Instructor participation in AHA/ALS Programs at UNC-CTC.
I. AHA Instructors must follow all requirements in accordance with the AHA
Program Administration Manual (PAM), plus those set forth by the UNC-CTC as
follows:
a. Within the two year certification period, Instructors will teach a
minimum of two courses for each discipline, in which the instructor
holds certifications.
Note: A “course” is defined as one single group of students.
II. BLS instructors must teach an entire BLS course to receive credit for the class.
III. ACLS and PALS instructors must teach a minimum of a half-day or equivalent to
receive credit for the course.
IV. Instructors must teach at least one class per discipline at the ALS Programs
Training Center facility. This class should be taught in the final six months of the
instructor’s renewal period and is for the purpose of monitoring the instructor for
renewal purposes.
a. For renewal purposes instructors must score a minimum of 90% on the
AHA written exam for the discipline(s) they teach.
V. Instructors must attend any mandatory Training Center updates or instructor
meetings.
a. Failure to attend mandatory meetings will result in the immediate
suspension of instructor privileges through the UNCH ALS Programs AHA
Training Center.
VI. Instructors must remain up-to-date and knowledgeable on AHA guidelines,
teaching materials and course specific cognitive and psychomotor domains.

The purpose of this Standard Operating Guideline (SOG) is to create a standard process for the
training and aligning of AHA instructor with the UNC-CTC.
I. Instructor Candidates apply to attend a UNC-CTC instructor training class or Instructor
Development Process.
a. The IC must submit a completed application to the Training Center;
b. The IC must submit copies of current AHA provider cards for each discipline in which he
or she is qualified to teach and wishes to transfer to UNC.
c. The IC submits copies of the AHA Instructor Essentials certificate of completion.
d. The IC submits a resume or CV with emphasis on his/her teaching background.
e. The IC either attends an Instructor Training Class or is enrolled in the Instructor
Development Program.
II. The IC observes (shadows) a ALS Programs AHA class of the discipline he/she is applying to
teach.
a. The purpose of this is so that the IC can see how a class is properly run and how the ALS
Programs AHA Training Center expects classes to be managed.
III. The IC team-teaches in another class. During this class the IC will act in the role of Lead
Instructor for some part of the class and is monitored by the Training Center Coordinator and/or
designee in each discipline for which the IC has applied to teach as follows:
a. BLS – Lead Instructor
b. ACLS – Case simulations, Putting it all Together and Megacode/Simulations
c. PALS – Core Case Simulations, Putting it all Together and Core Case Testing.
IV. Once the IC is monitored, the Training Center Coordinator will make a final decision to accept the
IC request for transfer.
V. Upon completion of the above requirements the IC and the ALS Programs AHA Training Center
Manager will meet to determine if both parties believe that alignment with the UNCH AHA TC is a
good fit for all.
a. If both parties agree that the IC alignment is in the best interest of both, the TC Manager
will accept the IC as an instructor with full privileges as outlined in this document.
VI. The TC Manager reserves the right to discontinue the alignment of any instructor at any time.
a. Instructor alignment is at the sole discretion of the TC Manager and may be dissolved at
any time in which the TCM feels that the Training Center mission is not being well served
by the alignment.

The purpose of this Standard Operating Guideline (SOG) is to create a standard process for assisting
an existing AHA instructor with transfer from another training center to UNC-CTC.
I. Instructor Candidates (IC) wishing to transfer to UNC-CTC will follow the process set forth by the
Training Center Coordinator.
II. The initial process for transfer to UNC AHA Training Center is as follows:
a. The IC must submit a completed application to the Training Center;
b. The IC must submit copies of current AHA Instructor cards for each discipline in which he
or she is qualified to teach and wishes to transfer to UNC;
c. The IC must attend a New Instructor Orientation session, which will be scheduled with
the Training Center Coordinator; and
d. The IC must attend a regularly scheduled AHA course held at the UNC training facility, as
an observer, for each discipline he or she applies to teach. This will enhance the
Instructors knowledge of logistics and methodology.
III. The IC will then be monitored by the Training Center Coordinator and/or designee in each
discipline for which the IC has applied to teach as follows:
a. BLS – Lead Instructor
b. ACLS – Case simulations, Putting it all Together and Megacode
c. PALS – Core Case Simulations, Putting it all Together and Core Case Testing.
IV. Once the IC is monitored, the Training Center Coordinator will make a final decision to accept the
IC request for transfer.
V. If the TC Coordinator accepts the IC’s request for transfer:
a. The IC will be provided a records transfer form;
b. The IC will send the records transfer form to his or her former training center; and
c. The IC’s training center will forward all pertinent records data to the UNC-CTC.
VI. Once the IC’s records are received, the IC will be accepted as an Instructor with the UNC-CTC.

This Standard Operating Guideline (SOG) is developed to establish a standard for Instructor renewal.
I. Within the two year certification period, Instructors will teach a minimum of four courses for each
discipline, in which the instructor holds certifications.
Note: A “course” is defined as one single group of students.
II. Within the six month time frame prior to the instructor’s must renewal date:
a. The instructor must teach at least one class at the ALS Programs Training Center facility.
b. The instructor is monitored in a Lead Instructor capacity by an ALS Programs AHA
faculty member.
c. The instructor takes the cognitive examination for the discipline and must score a
minimum of 90%.
d. The instructor must have attended any mandatory Training Center updates or instructor
meetings.
III. Upon completion of the requirements above, the ALS Programs AHA Training Center manager
will issue an updated instructor card.
IV. Instructor card renewal date:
a. The renewal date of instructor cards shall be the last day of the month, two years from
the date the instructor meets all renewal requirements and the new card is issued.
b. Instructor cards issued prior to the Recommend Renewal Date of their existing card will
have an expiration based on the issue date of the initial card.
V. Lapsed Instructor Cards:
a. Once an instructor’s card expires, that instructor cannot participate as a Lead Instructor in
any AHA class and the ALS Programs AHA Training Center cannot issue student
completion cards bearing the instructor’s name.
b. Lapsed instructors have a maximum of six months to meet all renewal requirements to
renew.
c. Instructors who are lapsed for greater than six months shall have their status changed to
“Inactive”.
d. If an instructor who is lapsed for greater than six months desires to continue as an AHA
instructor, he or she must retake an AHA instructor class or apply for entry into the ALS
Programs AHA Training Center Instructor Development Program.

VI. Instructors whose card lapses for any of the following reasons shall be granted an extension past
the six months deadline to renew:
a. Catastrophic illness or disability leading to the instructor’s inability to meet renewal
requirements.
i. This may include illness of direct family members requiring commitment of the
instructor to care for the family member.
b. Pregnancy during the last six months of the certification period. The pregnancy must be a
key factor in preventing the instructor from meeting renewal requirements.
c. Military deployment during the last six months of the certification period resulting in the
instructor’s inability to meet renewal requirements.
d. Any other condition or circumstance in which the ALS Programs AHA Training Center
Manager deems appropriate to grant an extension.
e. The ALS Programs AHA Training Center Manager shall issue a extension based on the
time the instructor’s circumstances prevented him/her from meeting renewal
requirements.
i. NOTE: This is not an extension of the renewal date. It is an extension of the time
an instructor is given past the six month deadline for renewal. The instructor is,
indeed, lapsed and cannot participate as a Lead Instructor in any class.

This Standard Operating Guideline (SOG) is developed to establish a standard for
cleaning and disinfecting UNC-CTC training equipment.
I. All manikins and biomedical equipment used in ALS programs, whether on/off-
site must be cleaned and/or disinfected after each use. It is the responsibility of
the Lead Instructor to:
a. Ensure that all equipment is cleaned using cleaning products
recommended and approved for the cleaning of biomedical equipment;
b. Ensure that manikins are disinfected using an UNC-HCS approved
surface disinfectant product known to be safe for cleaning teaching
equipment such as CPR manikins, human simulation manikins etc;
c. Ensure that manikin lungs are changed after each use and appropriate
size lungs are replaced; and
d. Ensure that all cushion face masks (i.e. Pocket Face Mask) are
cleaned following use and prior to next scheduled BLS, ALCS or PALS
course.

The purpose of this Standard Operating Guideline is to provide official
documentation regarding the expectations of the American Heart Association and
the UNCH ALS Program AHA Training Center as related to conducting BLS
Classes.
I. All AHA BLS classes must be taught in compliance with the requirements found
in the AHA Instructor Manual for the specific class and the AHA Program
Administration Manual.
II. Minimum equipment for conducting a BLS class:
a. Manikins designed for training of chest compression and ventilations.
i. The ratio of students to manikins cannot be greater than 3:1.
1. Whenever possible a 1:1 ratio is preferred.
ii. Infant and child classes must use appropriate manikins.
b. One-way-valve or other ventilation barrier device: 1 per student
c. Ventilation mask designed for use with the manikins used in the class.
d. DVD specific to the class taught and device to play the DVD.
i. The AHA requires that the DVD for a class be used in its entirety,
start-to-finish.
1. Use of the DVD must be in an instructor-led class
environment. In other words, it is not permissible to give a
student a DVD to watch off line without the ability to practice
skills in the presence of an instructor.
e. Automated External Defibrillator (AED). All BLS classes now teach AED
use. Instructors must use either a Trainer AED (preferred) when available.
If a trainer AED is not available, instructor can use a “real” AED to show
the functions.
i. DO NOT activate the AED.

f. Adult and pediatric Bag-valve-mask for use in BLS for Healthcare Provider
class.
III. Attendance Requirement:
a. Students must attend 100% of the class. Late arrivals and students who
leave prior to the class cannot be given credit for a class unless the
instructor has a method in place for the student to make up the missed
material in an instructor-to-student setting.
IV. Testing:
a. All students must complete any required cognitive and psychomotor
examinations required of the class.
i. Note: Some classes, such as Heartsaver, do not require a written
exam.
V. Remediation:
a. Any student who cannot adequately pass a required cognitive or
psychomotor examination should be remediated by an AHA Instructor.
b. The AHA instructor providing remediation should decide if the student is
prepared for a retake of the exam.
i. Any retake of the written exam must use a different version of the
exam.
ii. Any student who fails both the initial exam and retake must retake
the class at a later date.

The purpose of this Standard Operating Guideline (SOG) is to create a standard process for AHA
class pre-registration, end of class reporting and maintenance of AHA records.
I. All AHA classes taught by instructors off-site should be registered using the appropriate format,
no less than five days prior to the class.
a. Off-site is defined as any location other than the AHA Training Center Facility.
b. The purpose of pre-registration of class is to allow the TC Manager the opportunity to
verify that instructors possess current instructor certifications, potentially audit the class.
II. At the conclusion of the AHA class, the instructor reports the class to the Training Center using
the appropriate reporting form.
III. Unless otherwise indicated, all fields on the student roster must be completed.
a. Email addresses are required from all students unless they either do not have one or
refuse to provide it.
IV. It is important that all assisting instructors are listed on the reporting form.
V. All students must be given an AHA Student Evaluation Form at the conclusion of class.
a. Students may return the completed form to the instructor
b. Any student who prefers not to return the evaluation to the instructor must be given the
contact information of the AHA Training center and told that they may submit it directly to
the TC Manager.
c. Any student who does not wish to submit the student evaluation to the instructor or to the
TC Manager must be given the AHA contact information and told that they can submit the
evaluation directly to the AHA.
VI. Instructors must maintain the following class information for a period not less than three years
from the date of class;
a. Student roster
b. Student written exam results
i. These may be kept as a summary with each students score if the instructor does
not wish to keep the actual written exam answer sheets for each student.
c. Student skill exam check sheets.
i. These may be kept as a summary with each students pass/fail if the instructor
does not wish to keep the actual written exam answer sheets for each student.
VII. AHA Student Evaluation Form for each student.

The purpose of this Standard Operating Guideline (SOG) is to create a process for the processing,
printing and distribution of AHA class completion cards.
I. The ALS Programs AHA Training Center prints all cards.
a. We do not issue blank cards to instructors
II. At the conclusion of the AHA class, the instructor reports the class to the Training Center using
the appropriate reporting form.
a. Instructors must be careful to select the correct AHA class on the online roster
III. Instructors must include their AHA Instructor ID on the roster.
IV. Student names must be legible and spelled correctly.
V. Within five business days of receiving the class roster, the ALS Programs Training Center will
print the cards and contact the instructor that they are ready for pickup.
a. Cards may be picked once any fees are paid
b. Cards can be mailed by USPS
c. We will not send cards via UNCH interoffice mail.
VI. All AHA cards dispersed by the ALS Programs AHA Training center will contain the following
information:
a. Name of class
b. Student name
c. Issue date
d. Recommended renewal date
e. Training center name and ID# and address
f. Course location
g. Instructor name and AHA ID#
VII. The AHA requires that students have their class completion cards within 20 days of completing
class.
a. The ALS Programs AHA Training Center expectation is that students have their cards as
soon as possible after class.
VIII. Reprinting Cards
a. Any AHA card reprinted for the following reasons will incur a reprint fee:
i. Lost card
ii. Misspelled name on class roster
iii. Any card/roster related issue not the result of Training Center error.

This guideline will establish a standard for the use, care and accountability of all UNC-CTC
equipment and supplies to minimize fraud, waste and abuse.
I. All training equipment and supplies owned by the UNC-CTC are intended for the use of programs
offered by UNC-CTC and NPPD. These include:
a. Classes being taught to UNC HCS staff, affiliates and guests of UNC-CTC
programs; and
b. Classes taught as community outreach to non-profit entities outside UNC
HCS
II. No UNC-CTC instructor may use Training Center equipment or supplies for the purpose of
teaching classes for personal or monetary gain.
III. Limited loaner equipment is available to Instructors for community outreach programs and off-site
UNC sponsored courses.
IV. Instructors utilizing loaner equipment must complete the online BLS Supply/Equipment Loan
Request Form and submit prior to being issued loaner equipment.
a. The online request form should be submitted 5 days prior to the scheduled
course or early enough to allow adequate response from CTC staff; and
b. The online form will serve as the UNC-CTC official equipment request and
must be completed in detail to ensure items are made available for issue.
V. Equipment may be reserved at any time but may only be picked up when a UNC-CTC staff
member is onsite and equipment has been reserved In-Accordance-With SOG_2012.1
VI. All UNC-CTC durable equipment items must be cleaned and returned to UNC-CTC in a
serviceable condition.
a. Durable equipment must be cleaned and disinfected using standard cleaning
and infection control practices established by UNC-CTC and outlined in
SOG_ALS 2011.6;
b. Instructors will be responsible for reporting non-serviceable equipment items
and any items damaged or stolen while in their possession and
c. Instructors are required to sign the “issue” and “return” section of the BLS
Supply/Equipment Loan Request Form with a UNC-CTC staff member
whenever equipment is being borrowed.
