Magnitude of the problem in Europe
The systematic data collect on the annual number and type of operations are onlyavailable at a national level in 23 European countries .
In 2004, the number of major surgical procedures has estimated to be at therate of 4% of the world population per year.
Applied to Europe with an overall population of over 500 millions, an estimation of19 million major procédures annually.
Considering a proportion of 30% of patients with cardiac comorbidities, 5.7 millionprocedures annually are performed in European patients with increased risk ofcardiovascular complications.
Considering an incidence of overall complications between 7-11% and that 42% ofthem are related to cardiovascular complications, 167 000 cardiac complicationsannually may be considered including 19 000 life-threatening

Gestion des antiplaquettaires préopératoires

• Apport myocardique en oxygène (CaO2 et Débit sanguin coronaire)
• Adaptation à une demande métabolique par vasodilatation coronaire limitée par
athérome coronaire (+++)
• Inadéquation entre transport et besoins en O2
• Contrôle contraintes hémodynamiques (tachycardie, hypotension artérielle,
hypertension) et métaboliques (anémie, hypoxémie, inflammation, hormones de
stress)
• Variable la plus facilement modulable CaO2 (hémoglobine et SaO2)

Rappels physiologiques
Syndrome coronarien aigu périopératoire
• Plus fréquente des complications cardiovasculaire +++
• « Dommage » ou « nécrose » myocardique
• Evaluation postopératoire de troponine cardiaque (T ou I) au delà du
99ème percentile de la distribution des valeurs normales du laboratoire
• IDM est un SCA qui associe à l’élévation de troponine un des critères
suivants:
� Modification ECG (sus ou sous décalage ST)
� Symptomatologie angineuse
� Vulnérabilité myocardique (hypokinésie à l’ETT)
� Thrombus intra-coronaire (coronarographie)
• Rare symptomatologie angineuse et/ou électrocardiographique
• Peut survenir en dehors pathologie coronaire préalable
• Existence coronaropathie chronique (facteur de risque)
• Survie moyen et long terme (intégrité du capital myocardique)
