Accommodation Agreement and Undertaking - Pluto LNG Park Project of 4

ACCOMMODATION STANDARDS ON PLUTO LNG PARK PROJECT

The Pluto LNG Park Project (the Project) has a range of accommodation types to which Contractor’s personnel may be assigned. These accommodation types fall into three (3) main categories:

1. Motel Style Accommodation; 2. Permanent Room Style Accommodation – Ensuite; and 3. Permanent Room Style Accommodation – Shared Ablutions.

Whilst there may be some variations between different facilities that fall under the same broad headings, the services and facilities and the conditions related to each facility fit well under the above broad headings. Further particulars related to these three (3) accommodation types are provided on the form attached. To ensure that all Contractor’s personnel mobilising to the Pluto LNG Park Project are fully aware of the types of accommodation to which they may be assigned, it is a requirement in order to approve mobilisation to the Project, that all proposed Contractor’s personnel are provided with the attached document and that they sign their acceptance of the conditions related to the accommodation options. Failure to signify acceptance of the accommodation conditions will result in the Project denying mobilisation of that proposed Contractor’s personnel to the Project.


PROJECT PROCEDURE




AGREEMENT AND UNDERTAKING – PROJECT ACCOMMODATION The Pluto LNG Park Project (the Project) has three different categories of accommodation available. You may be assigned to any one of these types of accommodation during your employment on the Project. Assignment to one type of accommodation is no guarantee that you will not be assigned to a different types of accommodation at another time. The following information provides an overall description of the accommodation facilities: 1. Motel Style Accommodation:

a. Motel style accommodation means accommodation at a facility where you will be required to check in on arrival and check out on departure for each work cycle that you are employed on the Project.

b. The room assigned to you at check in will be from the current stock available. This means Motel

Guests will not occupy the same room successive rostered work periods on the Project.

c. Room standards have been designed to cater for this motel style of Village operation by providing a higher standard of in-room facilities. This includes, for example, provision of a TV/DVD/Foxtel/Stereo system (with IPod docking) to avoid the inconvenience and cost for Motel Guests having to provide their own in-room entertainment equipment. General room equipment includes: TV/DVD/Foxtel/Stereo system (with IPod docking) Fridge Ensuite Storage WiFi connection plus purchase

d. Manageable private belongings that Motel Guests do not wish to transport to and from the Village may be stored during R&R. Lockable storage containers and a secure storage facility and service will be provided for this purpose. Please note that any private belongs stored must not contain dangerous goods and you will be required to sign a declaration stating that you have not included dangerous goods.

e. Cleaning and standards of hygiene will receive appropriate emphasis under this style of operation

by Village management. This includes: Weekly Cleaning will be: • 2 x full cleans per week • All linen changed twice per week • Towels changes twice per week • Full room and ensuite clean

Pre - Arrival Cleaning will comprise: • Each room will be thoroughly cleaned prior to arrival of each returning Motel Guest • All linen changed including mattress protector • Towels changed • Refrigerator cleaned internally • Air conditioner filters cleaned

f. Motel Guests will be presented with rules that relate to this type of accommodation and describe

the standards required of all Motel Guests. All Motel Guests in residence are expected to act with due consideration for the comfort and enjoyment of other Motel Guests in the facility.

g. If you are assigned to motel style accommodation, your initial roster of 4 weeks on and 1 week

R&R will continue to be the default work roster that will apply to you. Changes to your roster can be requested and must be mutually agreed before being changed. However, changes will only be approved if there is no future impact to the availability of motel rooms for the Project.

2. Permanent Room Style Accommodation (Ensuite):

a. Permanent room style accommodation means assignment to a room with an ensuite bathroom and keeping that same room (except for unforeseen circumstances) for successive work cycles whilst you are employed on the Project.

b. Room standards are within set guidelines and each room is equipped with:

c. Room cleaning consists of 1 full clean and 1 half clean each week. d. Your work cycle and roster, including 1 week R&R at the completion of each work cycle, will be as

expressed in the industrial agreement that applies to you. Changes to your work cycle and roster can be requested and approved in consultation with your employer and consistent with the industrial agreement.

3. Permanent Room Style Accommodation (Shared Ablutions):

a. Permanent room style accommodation (shared ablutions) means assignment to a room and keeping that same room (except for unforeseen circumstances) for successive work cycles whilst you are employed on the Project.

b. That room will be located at a facility that has shared ablutions. The industrial agreement that applies to you specifies a daily allowance of $26 per day for this situation. The allowance applies for each day that you continue to reside at a facility and share the ablutions.

c. Room standards are within set guidelines and each room is equipped with:

King single bed Large 5 Draw Chest Large Wardrobe, with hanging space with top and bottom shelf Bed side desk, with sliding draw Bar Fridge Colour TV - free to air channels only Each block has a common room with tea and coffee facilities, individual lounge chairs, household sized refrigerator, micro wave oven, and large screen TV.

d. Room cleaning consists of 1 full clean and 1 half clean each week.

e. Your work cycle and roster, including 1 week R&R at the completion of each work cycle, will be as

expressed in the industrial agreement that applies to you. Changes to your work cycle and roster can be requested and approved in consultation with your employer and consistent with the industrial agreement.

I fully understand the Accommodation standards and options, as outlined above, that are applicable to the Pluto LNG Park Project and agree to accept the conditions of all options I may be accommodated in during my employment on the Project. I am fully aware that I will be assigned to any one of the above accommodation facilities during my employment on the Project. I further understand and accept that, at any time, I may be re-assigned to another type of accommodation and that there is no guarantee as to the type of accommodation to which I will be assigned.
