Basics of Plan AdministrationAs an administrator of your group insurance plan with MedMutual Life, you are responsible for tasks that are essential for accurate and timely administration of the plan, including:

n Determining employees’ eligibility for coverage

n Enrolling new employees into the plan

n Distributing applications and certificates to covered employees

n Managing records, submitting changes, terminations and reinstatements

n Administration of benefits, including submitting forms and claims when necessary

n Submitting premium payments

Online Tools for Administration

EmployerLink, MedMutual Life’s secure online portal, provides a number of tools, resources and information to make managing your company’s health and specialty benefits quick and simple. Getting started with EmployerLink is easy:

1. Visit MedMutual.com and click the Employers tab.

2. Under Login to EmployerLink click Register Now.

If you experience any difficulty with registration, please contact the MedMutual Help Desk at 1-800-294-7583.

EmployerLink gives you the flexibility to:

n Add or remove employees

n Update employees’ personal information

n Pay your monthly bill via electronic funds transfer

n View and download current and past bills

n View certificate policies

n Update company information, including administrative roles, phone numbers and group contact information

n Download employee roster

n View and download claim forms and important documents

n View marketing materials available to your employees

Evidence of Insurability Requirements

Evidence of insurability is a statement of medical history and related information used to determine whether an applicant is eligible for coverage. If an employee is required to submit evidence of insurability to MedMutual Life, the coverage will become effective on the date MedMutual Life determines that the evidence is satisfactory and provides written notice of approval.

Evidence of insurability is required in the following instances:

n An employee applies for contributory coverage more than 31 days after they first become eligible.

n An employee applies for contributory dependent coverage more than 31 days after their first dependent becomes eligible.

n For insurance amounts in excess of the group’s non-medical maximum limit.

n For any other reason according to our current standard underwriting procedures and guidelines as outlined in your group policy and certificate.

Do not begin withholding employee contributions for the amount in excess of the non-medical maximum

until you receive approval from MedMutual Life.

Billing and Remitting Premium Payments

You will receive a detailed billing statement containing a roster of all insured employees. The premium amount due will be calculated for you based on the information submitted to MedMutual Life.

Determining Employee and Dependent Eligibility1

An employee is eligible for coverage if the employee is:

n A member of an eligible class of employees

n Working the minimum number of hours

n Actively at work

An eligible person may also include a retiree under the group’s formal retirement program, but only if the retiree coverage is approved by MedMutual Life.

The actively-at-work provision will not apply to life, accidental death and dismemberment and dependent life insurance for employees:

n Who are not disabled

n Who are on layoff, leave of absence or sabbatical leave

n Who were being covered by an extension of benefits provision under the prior carrier’s policy

n Who are no longer eligible for coverage under the prior carrier’s policy

An eligible dependent includes:

n An employee’s lawful spouse or domestic partner

n Any unmarried children (natural or adopted) who are within the age limits

Insurance will continue under this provision for a maximum of three months.

Determining Coverage Effective Dates1

The employee effective dates are as follows:

Non-Contributory Benefits (employer pays 100 percent of premium)

An employee who is actively at work will become insured for non-contributory benefits on the day following completion of the employee waiting period, if any.

Contributory Benefits (employee contributes to premium)

An employee may apply for contributory insurance coverage at any time. The coverage will become effective as follows, provided he or she is actively at work on that date:

n If the enrollment form is signed on or before the end of the waiting period, if any, the coverage will become effective on the day following completion of the waiting period.

n If the enrollment form is signed after the end of the waiting period, but within 31 days after that day, the coverage will become effective the date the employee signs the enrollment form. If the enrollment form is signed after the 31-day period, the employee is considered a late applicant and must furnish evidence of insurability satisfactory to MedMutual Life before coverage can begin. Coverage will become effective on the date MedMutual Life determines that the evidence is satisfactory and provides a written notice of approval.

Effective Date for Dependents

A dependent’s coverage becomes effective on the date the dependent satisfies all eligibility requirements if the election is made within 31 days of becoming eligible for coverage. If the election is not made within 31 days, coverage will become effective on the date MedMutual Life determines that evidence is satisfactory and provides a notice of approval.

Managing Membership

Completing the Required Forms

Our website, MedMutual.com/LifeForms, contains the forms necessary for enrolling your employees. In addition, all forms are available on EmployerLink.

Managing Employee Changes

When an employee changes coverage, a new application should be submitted reflecting any changes requested. In most cases, changes will become effective on the first billing due date after we receive the request, subject to any required underwriting approval.

When an employee gets married or has children, and a new dependent coverage is elected, the employee should complete an application to add the spouse or any new dependent children. This election must be made within 31 days of the family status change.

When an employee terminates coverage, whether the employee cancels coverage or terminates employment, the group is required to notify the Membership/Billing department. The notification should be sent within 30 days of termination or cancellation to [email protected] listing the effective date and reason for termination (e.g., left employment, waiving coverage). The group should then notify the employee of his or her conversion rights.

When a covered employee passes away, the group is required to notify the Membership/Billing department within 30 days of the event, indicating the date of death as the term date. If the employee had dependent coverage at the time of death, the group should notify the dependents of their dependent life insurance conversion rights.

When an employee’s insurance amount changes due to a change in salary, class or occupation, the change should be reported on the Premium Remittance form accordingly. The notification outlining the changes to employees’ compensation should be sent once a year to [email protected].

When an employee wants to change a beneficiary designation, the group is required to provide a MedMutual Life Beneficiary Designation Change form to the employee. The form must be signed and dated by the employee, and the employer is to keep all Beneficiary Designation forms on file. MedMutual Life will only require a copy in the unfortunate event of a death claim.

MedMutual Life also offers an online beneficiary tool that provides covered employees the flexibility to designate beneficiaries through a secure website, My Health Plan. (Please note this functionality is only available with healthcare coverage.) Confirmation of the employee’s designation is sent via e-mail to the employee.

Continuing Coverage after Employee Termination

An employee whose coverage terminates may have the right to convert his or her group term life insurance to an individual whole life insurance policy without evidence of insurability. If offered, life coverage for any dependents may also be converted at the same time. It is the employer’s responsibility to notify eligible persons of their conversion rights.

The right to purchase a conversion policy is available when the employee’s group term life insurance coverage ends for a reason other than the group’s coverage ending or changing. The employee has 31 days after the group term life coverage ends to apply for conversion coverage. The individual must complete an application for conversion and submit it to MedMutual Life via regular mail.

Claims

Submitting Claims

All claim forms are available on our website and can be downloaded at MedMutual.com/LifeForms or can be obtained by calling 1-866-925-2542 (select option 1). An overview of submitting life and disability claims is outlined below.

Submitting Life Claims

The group representative is responsible for completing the Death Claim form and submitting it to our office with a Certified Death Certificate, a copy of the original Enrollment form and the most recent Beneficiary Designation forms. The life insurance claim may be faxed or e-mailed to MedMutual Life.

If Accidental Death Benefits are being claimed, a copy of the police report, newspaper clippings, witness statement and any other available supporting documentation should also be included. Please note that in the event the death claim is faxed or sent electronically, the Certified Death Certificate is still required to be mailed to our claims department for processing at the address below.

Submitting Disability Claims

If an employee is absent from work due to disability and it is believed that the disability will extend beyond the policy’s elimination period, a disability claim should be filed. The claimant, the group representative and the claimant’s physician are each responsible for completing a portion of the claim form. The completed Disability Claim form should be faxed, e-mailed or mailed to our Claims Department for processing using the information below.
