ADM-103 CISM 1 of 2 ADM-103 Critical Incident Stress Management Purpose To provide a guideline for initiating assistance from the San Mateo County CISM Team. Related Documents SMCO Fire Chief’s Guideline G-154 Critical Incident Debriefing Summary Activation of the CISM Team will provide trained team members for intervention after traumatic incidents in order to minimize stress-related injury to Fire Department personnel. Policy 1.0 San Mateo County has a CISM team that is made up of members in various first responder disciplines within the county (fire, police, ems, dispatchers, mental health, etc.) trained in stress-related counseling. The team can be activated anytime by contacting County Communications at (650) 363-4907. (*Add link to county policy) 1.1 Defusings (within 24 hours after incident) or debriefings (more than 24 hours after incident) may be conducted depending upon the circumstances. Assistance may be given in an individual one-on-one basis or in a group setting. 1.2 Critical incident stress defusings and debriefings are not critiques of Fire Department operations. The defusing/debriefing process provides a format in which personnel may discuss their feelings and reactions and gain a better understanding of normal human behavior under stress. By participating in the debriefing process, individuals can reduce stress resulting from exposure to critical incidents. ALL DEFUSINGS AND DEBRIEFINGS ARE STRICTLY CONFIDENTIAL. 1.3 CISM assistance should be considered when we respond to any incident that creates a strong emotional reaction for personnel. The following are examples of incidents that may benefit from CISM assistance. Serious injury or death of a Fire Department member or other emergency personnel. Mass casualty incidents. • SeriousinjuryordeathofacivilianduringFireDepartmentoperations(i.e.,autoaccident,fire,etc.)

• Deathofachild,orviolencetowardachild.• Lossoflifeofapatientfollowingextraordinaryandprolongedexpenditureof

physicalandemotionalenergyduringrescueefforts.• Anincidentinwhichcircumstancesweresounusualordistressingthatit

producedahighlevelofemotionalreaction. 1.4WhenitisdeterminedthatrespondingpersonnelmaybenefitfromCISMassistance,theprocessofschedulingadefusingordebriefingshouldbeinitiatedassoonaspossible.1.5AnymemberoftheFireDepartmentmayinitiatethedefusing/debriefingprocessbycontactingtheirimmediatesupervisor.1.6TheBattalionChiefmayrequestaCISMresponsethroughCountyCommunicationsandthenwillbecontactedbyaCISMTeamcall-outcoordinator.Afterreceivingdetailsabouttheincident,theCISMTeamcall-outcoordinatorwillscheduleadefusingordebriefingandprovidetheappropriatenumberofCISMpersonnelbasedonthelevelofassistancerequired.1.7Allagenciesinvolvedwiththeincidentshouldbeinvolvedinthedebriefing,includingdispatchers,ambulance,coroner’soffice,andlawenforcementpersonnel.Onlyrespondingpersonnelinvolvedintheincidentareinvitedtoattendthedefusing.1.8DepartmentmembersinterestedinbecomingapartoftheCISMTeammaycontacttheCISMTeamCoordinatorformoreinformation.
