Army Family Action Plan (AFAP) is a grass roots program in which Army community members can relay to
Leadership issues that affect well-being at the local or even DA level. Use this form if you have a concern or idea
that you would like to see considered at the next USAG Fort Hamilton AFAP conference.
Issues: What is the problem?
Scope: Why is this a problem? (tell us about it - why is this a problem & who does it affect)
Recommendation: What do you think could be done to solve the problem? (how should we fix it)

The Army Family Action Plan (AFAP) is input from the
people of the Army to Army Leadership. This process
allows Soldiers & Families to express their feelings in
regards to military life & how they are affected. This alerts
Commanders & Army Leaders to areas of concern which
require their attention & gives Command the opportunity
to quickly execute policies towards resolving any issues
Soldiers and Families may have to safeguard their well-being.
How Does AFAP Work?
Success & Facts
AFAP is year-round. Issues are continually monitored
& worked on to create positive changes. Many AFAP
issues have improved and made great impact on the
quality of life in the Army.

Volunteer There are many ways to get involved.
Submissions made by the community are collected
from the AFAP Designated Deposit Boxes which
are placed around the garrison or t he ACS AFAP
Program Manager.
At the AFAP Conference; Soldiers (Active Duty,
National Guard and Reserve) Retirees, Surviving
Spouses, DA civilians and their Families identify,
develop and prioritize issues.
Once the issues have been identified, the Garrison
Commander will review the submissions and work
towards a solution. Unresolved Installation issues
are forwarded to the Regional AFAP. If the issues
are not resolved at the Regional level they are thenforwarded to the DA AFAP.

beneficial please fill out the AFAP Submission formon the reverse side. Fill out the appropriate fields
and submit it in an AFAP Designated Drop Box or
to the ACS AFAP Program Manager TODAY!
