The Department of Human Services is collecting the information and contact details you are giving, to assist in providing cultural support for the Aboriginal child/young person identified in the Cultural Support Plan. The details you provide will be distributed to the care team, family and child/young person aged ten and over.

The Department of Human Service, the Aboriginal Child Specialist Advice and Support Service and the Placement agency involved are committed to protecting the privacy of your information. If you have any queries about how your information is handled, to correct details or if you would like to request access to the information we hold about you please contact the Child Protection worker involved in developing the Cultural Support Plan that you are a part of.

The Department of Human Services will use the information we collect from you only for the purpose of providing cultural advice for the identified Aboriginal child/young person or as authorised by you or as permitted by law.
