CHW Pipes unless racked on shelves with tilting-preventive edges, to be stacked in bundles where each two adjacent cylinders or pipes are tied together so as to prevent spreading or tilting. Cylindrical materials shall be piled according to their type and size on a solid and level surface. Stacked pipes shall not be higher than 1.5 meter and adequately secured so as to be stable. Either a pyramid or wedged stack shall be used. Before cutting the tie wires, cylindrical stacks shall be maintained stable and no person shall be on the unloading side of the carrier.L

Storage and Handling of Adhesive glue.- Fire- Spillage- Ill health effect- Unsafe storageStore man/User MHM All chemicals shall be stored in approved storage area and recommendation on the MSDS. COSHH training shall be provided to all concern. The storage of flammable and combustible liquids should not be in proximity to heating or an ignition source. Packages shall be kept closed when not in use. Any action to open a package of flammable liquid or decant from it should be carried out in a well-ventilated area and away from potential ignition sources. The transfer of a flammable liquid from storage to the point of use shall be carried out in a manner that minimizes the possibility of spillage or fire. Liquids shall not be stored or used where they may jeopardize escape from building in the event of fire. Persons who handle Flammable liquids must be aware of the hazards involved. Any spillage shall be cleaned up immediately and the materials used in the clean up shall be disposed off accordingly. Any materials that might interact dangerously if mixed shall be kept apart so that the possibility of interaction is minimized, e.g.: fuel.L

Never use scaffolding unless it is properly erected & inspected by competent person provided with a green tag. Never use scaffold tagged in red. Ensure a safe means of access/ egress is provided. All scaffolds must be fitted w/ guard/ mid rails, toe boards. If fall protection from scaffold is not practicable to provide, safety harness must be use by workers & secure overhead. Where edge protection is removed for access, or is not reasonably practicable, fall arrest or restraint equipment will be used by operatives working at or near the edge. Suitable eye protection to be worn where there is a foreseeable risk of eye injury. Suitable RPE to be worn where there is a foreseeable dust hazard. Good housekeeping shall ensure.L

Other person in the vicinityHHH Hot working permits must be in place before work commences. All staff to follow their part of permit. Operators must be trained in safe operation of equipment. Remove any combustible materials near the area. Protect any combustible materials that cannot be moved, from close contact with flame, heat, sparks or hot slag. Use suitable guards or covers such as metal sheeting or fire retardant blankets. Handle cylinders carefully. Keep them in an upright position and fasten them to prevent from falling or being knocked over. Flashback arrestors must be fitted on both cylinder regulators. Wear protective clothing, gauntlet gloves and eye protection.L

Cutting of CHW pipe using Disc Cutter and hand grinder.- Bursting of grinding wheels or discs.- Flying debris and sparks from grinding wheel and/or work piece.- Cuts or burns from work piece.- Risk of fire in surrounding area due to sparks from grinding wheel.- Electric shock.- Slip, trips.Operatives

Other person in the vicinityHHH Hot working permits must be in place before work commences. All staff to follow their part of permit. Combustible material around the area must be removed. Suitable fire extinguisher to be available. Cutting machines (angle grinders, disc cutter) must not be used without their wheel guards being fitted and correctly positioned. Maximum RPM rating of each abrasive wheel must be compatible with RPM rating of grinder motor. New abrasive wheels must be visually inspected and ring tested beforemounting. Goggles (safety glasses) and face shields must be worn. Each electrically operated grinder/ cutter must be effectively grounded. Ensure proper housekeeping.

Where possible loose flammable materials should be removed and other flammable materials protected within the working area. PPE must be worn. Second person must be in attendance. Extra ventilation should be considered in work area. Effective fume control equipment should be used if required. Welding equipment must be inspected for good condition before use. Protect work area with screens if possible; if not, exclude persons other than operatives who must have appropriate eye protection. Work area to be kept clear of debris etc. Safety goggles or visor must be worn when chipping slag from workpeices or grinding. Have a fire extinguisher available. No cutting or welding during the last hour of work.L

Always consider the position of other people and warn them before lifting the load. Ensure work area suitably cleared and warning signs posted as required.L

Use of Hand tools.- Eye injury- Injury to hand, feet, body.OperativesMMM Tools provided must be assessed to ensure that they are suitable for the purpose in which they are to be used. Tools are to be in good working condition prior to issue to employees. Eye protection is to be provided and used whenever work involves using hand tools where there is a risk of flying particles or pieces of the tool breaking off. Damaged tools should be immediately removed/ tagged out to prevent inadvertent use.L

Use of power tool. Drill machine- Clothing becomingentangled in moving parts- Eye injuries from dust or other fragments- Wrist and hand injuries due to tool jamming or binding- Electric shock- Slip, trip & fall.Installation crews and other person nearby.MMM Loose clothing to be kept clear of moving parts of power tools. Operatives shall be trained in the correct use of portable tools. Suitable eye protection to be worn where there is a foreseeable risk of eye injury. Suitable RPE to be worn where there is a foreseeable dust hazard. Tools must be used in accordance with manufacturers instructions. All power feeds maintained in good condition. Portable tools shall only be used in well-lit and ventilated areas. All wire should be hanged 7 ft above the ground. Ensure proper housekeeping.L

PRESSURE TESTING

Pressurization Process- Unauthorized personnel/ access.- Defective tools/ equipment- Exceeding maximum allowable pressure- Injury to workers- Damage to equipment- Unattended pressurized tool/ equipment- Slip, trip.Installation crews and other person nearby.HMM Pressure test planning should be coordinated to all relevant concern at site before the execution date. Barricade the area where pressure test to be carried out. Post warning notice. Ensure the equipment/ tool to be used for hydro testing (such as hose, pump, coupling, pressure gauge etc.) are free from any defects and suitable for the pressure rating of the system. Pressure measurement tools are calibrated and status is still valid. Ensure the pressure will not exceed to recommended pressure. Keep away from the pressurized part of the system being hydro-tested. Monitor the pressure gauge from a safe distance away from potential source of high pressure jetting. Ensure workers are wearing PPE. Never increase the hydro-test pressure more than the maximum allowable pressure. Do not leave the pressurized hose/ pump unattended when the hydro-test is going on. Isolate the equipment when not in use. Ensure proper housekeeping at work site.L

De- pressurizationProcess- Sudden release of pressure.- Contaminated waterOperatives

EnvironmentHHH Start de- pressurization by opening gradually the vent valve. Do not open the drain valve if the vent valve is still closed, this is to avoid vacuum condition inside the pipe. Contaminated water must dispose as per local regulation.

Store hazardous materials in suitable containers, ensuring only as much as is needed is in the workplace, and that lids are replaced when not in use. Store chemicals as per recommendation from the MSDS. Keep away from heat, sparks or open flame and other sources of ignition. Avoid contact with chemicals; use PPE as gloves, goggles, masks etc. Always wash hands after use, and do not eat, drink or smoke when handling hazardous substances Spilled chemicals should be cleared off immediately & contaminated cleaning materials must dispose off accordingly. All chemicals should be place in its original container and labeled properly. Chemicals should be use in a well ventilated area.L
