DATE AND LOCATIONThe 22nd Annual Mayo Clinic Symposium on Sports Medicine will be held November 9-10, 2012. Course headquarters will be located in Heritage Hall, in the subway level of the Kahler Grand Hotel in Rochester, Minnesota. Meeting facilities are easily accessible by skyway and pedestrian subway, which connect to Mayo Clinic, shops, restaurants, and other hotels.
CANCELLATION POLICYIfyoucancelyourparticipationinthiscourse,your registration fee, less a $75 administrative fee, will be refunded when written notification is received by Mayo School of CPD before October 26, 2012 ([email protected] or fax#: 507-284-0532). No refunds will be made on or afterOctober26,2012.Canceledregistrationsare non-transferable.
