2018 Snohomish School District Local Scholarship Notebook (LSN) Program Completed notebooks are due in the Career Center no later than 3:00 PM on Thursday, February 9, 2018. Step 1: Review the document and application below to determine the information you will need to provide. Input on the fillable pdf (blue data boxes showing) once you have all of your information. Type a one-page, one-sided document discussing the following topic: What education program do you plan to study and why? PAGE 6—Teacher Evaluation Form. Request that a teacher complete the LSN Teacher Evaluation Form - staff can access this form online. Submit your teacher request well in advance to ensure sufficient time for preparation. Pick this up from the teacher well before the due date. PAGE 7—Employer or Community Member Recommendation. Request a one-page, one-sided letter of recommendation from an employer or community member (may not be employed by the school district, Sno -Isle or R/S college). Submit this request well in advance to ensure sufficient time for preparation. PAGE 8—Financial Need Statement. The typed statement may be no longer than a one-page, one-sided document. Express in your own words why/how the scholarship money could help you reach your education goals. This statement needs to be written by the student from the student’s point of view. No personal information such as taxes, income or FAFSA info please. You can include family hardships or personal challenges that might help express your need. This information is important for the committee to consider your specific needs when reviewing notebooks. PAGE 9—Transcript Page. Career Center Assistant will provide the transcript in the notebook for you. Step 2: Assemble Notebook. Starting with the first inside document sleeve, place completed notebook pages in the correct order (back to back) in the approved notebook folder. A limited number of used “loaner” notebook folders are available in the Career Center. At SHS, additional folders may be purchased at the Explorer’s Club School Supply kiosk during lunches. At GPHS, they can be purchased at the Grizzly Den. All submitted scholarship notebooks must have the same approved notebook cover.
