2018 Snohomish School District Local Scholarship Notebook (LSN) Program
Completed notebooks are due in the Career Center no later than 3:00 PM on Thursday, February 9, 2018.
Step 1: Review the document and application below to determine the information you will need to provide. Input on the fillable pdf (blue data boxes showing) once you have all of your information. PRINT the four pages. DO NOT SAVE your completed application to your desktop as it will not save the data you have input.
Pages 1-4 are forms provided in this link; pages 5-8 are pages to be provided by the student. Carefully follow the directions below for completing each page.
• PAGE 1 - Attach a wallet-sized picture that captures your spirit. Do not decorate the page.• PAGE 2 - Application Page. Complete all sections and make sure all appropriate signatures are included. Fill in blanks
with N/A for non-applicable information. Be sure to include “Intended Major.” After your notebook has been turned in, keep the Career Center updated with your college decisions and scholarship awards.
• PAGE 3—Student Activities and Work Experience. In the Student Activities section, be complete and include clear descriptions of your activities, leadership positions and/or accomplishments. In the Work Experience section, list most recent job experiences first.
• PAGE 4—Honors and Awards and Community Service. Include clear descriptions in the Honors and Awards section. In the Community Service section, include your most recent service activities first and obtain verification initials.
• PAGE 5—Student Essay. Type a one-page, one-sided document discussing the following topic: What education program do you plan to study and why?
• PAGE 6—Teacher Evaluation Form. Request that a teacher complete the LSN Teacher Evaluation Form - staff can access this form online. Submit your teacher request well in advance to ensure sufficient time for preparation. Pick this up from the teacher well before the due date.
• PAGE 7—Employer or Community Member Recommendation. Request a one-page, one-sided letter of recommendation from an employer or community member (may not be employed by the school district, Sno -Isle or R/S college). Submit this request well in advance to ensure sufficient time for preparation.
• PAGE 8—Financial Need Statement. The typed statement may be no longer than a one-page, one-sided document. Express in your own words why/how the scholarship money could help you reach your education goals. This statement needs to be written by the student from the student’s point of view. No personal information such as taxes, income or FAFSA info please. You can include family hardships or personal challenges that might help express your need. This information is important for the committee to consider your specific needs when reviewing notebooks.
• PAGE 9—Transcript Page. Career Center Assistant will provide the transcript in the notebook for you.
Step 2: Assemble Notebook. Starting with the first inside document sleeve, place completed notebook pages in the correct order (back to back) in the approved notebook folder. A limited number of used “loaner” notebook folders are available in the Career Center. At SHS, additional folders may be purchased at the Explorer’s Club School Supply kiosk during lunches. At GPHS, they can be purchased at the Grizzly Den. All submitted scholarship notebooks must have the same approved notebook cover.
Step 3: Sign Up for scholarships in the Career Center between January 9 and February 8th. A list of the LSN scholarships will be found on the high school website. You must sign up for each scholarship that you wish to be considered for. If you do not sign up—your notebook will not be viewed by scholarship committees. Do not sign up for scholarships if you do not meet the criteria—your notebook may be disqualified from all scholarships.
Step 4: Submit your LSN to the Career Center before 3:00 PM on February 8th, 2018. As always, turning it in early is advised. Late or incomplete notebooks will not be accepted.
Step 5: Wait patiently. The scholarships in the LSN are provided by various donors throughout the community. Donors are invited to review their scholarship applicants through April. Scholarship recipients are selected by the donors or their assignees. Notification of recipients will begin in late April and will continue until all donors have made their selection.
