About Us

1978—Corporation established as Jasper County Sheltered Facilities Association

1978—Opened First Group Home

1981—Established Leisure and Summer Camp Program

1985—Established Day Program Services

1989—Established Community Advocacy Supports Program

1994—Established Individualized Supported Living Program

2000—Separated from Jasper County Tax Board

2003—Established First Steps Program in Southwest Missouri

2003—Changed name of agency to Community Support Services of Missouri

2004—Received CARF accreditation for 3 years

2005—Carl McConnell Activity Center Opens

2007—Received CARF accreditation through 2010

2008—Celebrating 30 years of “Making Opportunities Happen!” 1978-2008

2009—First Non-Profit Developmental Disabilities Agency in Missouri to be awarded contract for Targeted Case Management Services with the Division of Developmental Disabilities

2011—F-5 Tornado hits Joplin, three Individuals served died, two heroic staff hurt

2012—Alcott and CMAC rebuilt, Texas Duplex purchased

Community Support Services of Missouri offers a wide array of programs designed to support children and adults

with developmental disabilities. We also offer in-home assistive services to persons of all ages throughout the

Southwest Missouri area.

Infants & Toddlers

First Steps, Missouri’s Early Intervention system, is an important program for young children with developmental

delays and their families. First Steps provides seventeen therapy services for children from birth up to 3 years old,

with developmental disabilities or diagnosed conditions associated with developmental delays. Serving 10 counties

in Southwest Missouri: Barton, Dade, Cedar, Vernon,

St. Clair, Lawrence, Jasper, Newton, Barry, and McDonald.

Children & Adolescents

Recreation & Leisure Services

Activities and Programs geared toward school age Children and adolescents with developmental disabilities.

GROWING OPPORTUNITIES AND LIFE SKILLS (Known as our “G.O.A.L.S.” program)

This program offers a wide variety of age appropriate activities designed to promote growth and independence. Pro-

grams range from art and craft projects to recreational activities, to learning about personal hygiene and social rela-

tionships.

WEEK END AWAY PROGRAM (Known as “Short Term Community Living” )

This program offers a chance for children and adolescents to spend time away from their families to work on skills

needed for independence.

SUMMER CAMP

This program runs during the month of July each year. It is designed to enhance and promote progress made by par-

ticipants during the school year by providing educational and therapeutic opportunities.

Adults

SUPPORTED LIVING

This program offers highly qualified and trained staff to provide support based on each person’s unique needs.

Support provided for people in their own home

Up to 24 hr a day based on need

Assist with all aspects of independent living from cooking to budgeting

GROUP HOME LIVING

We have two group homes in Jasper county that specialize in caring for individuals with increased medical needs.

Each home can support up to 6 people

Staffed with highly qualified and trained staff 24hrs a day

Nurse support available 24hrs a day

Each home is equipped with a State of the Art Lift system for safety

Targeted Case Management

Case Management assists individuals eligible for services from the Division of Developmental Disabilities in

finding needed medical, social, educational, and other specialized services.

Case Managers are professionals who are trained in the field of mental health and/or closely related fields.

A Case Manager will get to know the individual and assist with developing a person centered plan. This plan

will address specific needs and goals that the person has in his or her life.

The Case Manager will work to locate resources and monitor services that will meet the person’s needs and

assist the individual in achieving personal goals.

Other services a case manager can provide include:

Advocacy

Linking to leisure and recreational activities

Requesting services from local, state, and federal programs

Employment opportunities

Community Support Services provides Case Management in the following counties:

St. Clair

Carl McConnell Activity Center

The Carl McConnell Activity Center is a joint project between Community Support Services and Challenger

Sports. Both organizations support sporting opportunities for children and adults with

developmental disabilities. This center hosts the CSS Summer Camp program and our recreation

program for young children with disabilities, as well as a variety of leisure activities for our G.O.A.L.S. pro-

gram.

Our many thanks to the McConnell family, our local sponsors, and our board and staff who have joined us to create a

legacy that will provide opportunities for a lifetime!

Inclusive Playground It's time to play! Thanks to the support of our partners at KaBOOM!, the McDonald's Foundation, and community vol-

unteers, we now have a great place for children of all abilities to play together! Our inclusive playground is located di-

rectly behind our main office at 2312 Annie Baxter Avenue in Joplin and is open to the public.

BEHAVIORAL FITNESS (BFit) PROGRAM

CSS's Behavioral Program is designed to support individuals exhibiting behaviors that are adversely affecting their daily

routines. We are committed to the principles of Applied Behavioral Analysis and Positive Behavioral Intervention &

Supports.

Applied Behavioral Analysis (ABA) Services

CSS ABA Services - Our BFit Field Consultants specialize in children and adults with behavior disorders caused by a

variety of developmental disabilities including autism. We work with families in diverse settings and in partnership with

many service providers and members of an individual’s support team. Techniques can be used in structured situations

such as a classroom lesson as well as in "everyday" situations such as family dinnertime or the neighborhood play-

ground.

BFit Center

The BFit Center will support individuals in crisis as an alternative to acute care hospitals and emergency rooms. It is a

safe and secure environment where trained staff and an individual can focus on why the BFit Center’s services were nec-

essary and what can be done to prevent future behaviors of concern. The BFit Center is available 24/7, 365 days a year

and the individuals’ stay is 8 to 23 hours to ensure the best outcomes and quick stabilization.

Satisfaction Survey Results Summary

During 2016 a satisfaction survey was randomly conducted for the Targeted Case Manage-ment, Residential and Community Living programs. Early Childhood Services are surveyed through DESE. Surveys to family members/guardians and direct care staff were mailed with a self addressed, stamped envelope to return them. Surveys to individuals served, and case managers were hand delivered with a self addressed, stamped envelope to return the survey.
