Overview

This document is intended to provide Missouri users at both the school and district levels with instructions on setting up

InformationNOW for the collection and reporting of state-required data.

Latest Changes to this Document

This document has been revised to incorporate changes for the 2017-2018 school year. Recent changes are highlighted in

yellow throughout the document where applicable.

Data Entry and Setup

Enrollment System Settings

Due to the nature of attendance reporting, certain enrollment settings should be selected to ensure correct attendance

reporting.

Go to School/District | District and select the Enrollment tab.

• In the Withdrawals section, bullet The first day of non-attendance for Withdrawal date school enters is student’s...

• In the Enrollment section, check Allow students to enroll and withdraw from the same school in a single day.

• In the No Shows section, enter the date to Disable button on. Enter starting date for No Show to be inactive

and Academic Year affected.

Note: MO schools should not use NO-SHOW.

• In the Withdrawals section, check Withdraw from all sections.

Note: This should be done prior to the start of the year.

First Day of School

On the first day of school, students who do not return to school must be withdrawn from school.

• Daily Attendance will be taken.

• Go to Attendance | Reports and click Daily Absence Listing.

• Print the report for the first day of school.

If the student transferred out during the summer after completing the regular academic session:

• Go to Students | Student Maintenance and search for the student.

• Go to Schedule. Select all classes and delete all sections.

• Open the student’s file and browse to the Enrollment tab.

• Withdraw the student on the first day of school with the appropriate Exit Code.

For any student who dropped out or withdrew during the last regular academic session (not including Summer School):

• Go to Students | Student Maintenance.

• Search for the student.

• Open the student’s file.

• Under Manage | Select Schedule, delete the student’s schedule.

• Click the Enrollment tab.

• Select the enrollment record and Delete it.

Failing to delete the student schedule before the entry record is deleted could cause issues later in the school year.

To confirm all students are reported correctly:

• Go to Attendance | Reports and click ADA/ADM by Hours.

• Set the beginning and ending date range to include the first day of school only.

• Print the report.

• Full-time students’ member hours should match the school hours noted at the end of the report.

Schedule Setup for Aides (Position Code 80)

Aides are to be reported with the same course information as the teacher being assisted on the Course Assignment

extract.

Perform the following steps to do this:

• Go to Scheduling | Master Schedule and search for the section in question.

• Open the section by clicking the Course.Section number link.

• Click Teacher, then click Add.

• Search for and select the teacher.

• Confirm the selection.

• DO NOT change the Primary selection from the primary teacher.

• Click OK to save.

Schedule Changes for Students

As the year progresses, a student’s schedule may be changed for any reason. It is important that schedule changes are

made properly for students so that the Enrollment Attendance Extract will report the correct hours for all students.

Please refer to the InformationNOW – Scheduling Quick Reference Guide for full details.

Some points to remember when making schedule changes during the school year:

• If the student is moving from one class to another, the withdrawal date and the enrollment date must be

consecutive dates. For example, the student withdraws from one section on 10/01/YY and enrolls into another

section on 10/02/YY.

• If the student never attended a class or was only in a class a few days and you do not want to keep any

info/history for that class. Then you can delete the class from the student’s schedule. This will make it look like

the class never existed for the student and delete anything related to that class.

To make sure the schedule change was done properly:

• Go to Attendance | Reports and click ADA/ADM by Hours.

• The beginning date should be the first day of school and the ending date should be the date on which the

student’s schedule was updated.

• The hours membership for the student should match the school hours reported at the end of the report.

o If a student is reporting too many hours, there may be a schedule change error or too many classes may be

scheduled.

o If a student is reporting too few hours, there is most likely an incomplete schedule.

Note: It is the best practice to print the ADA/ADM by Hours report weekly to help locate errors early

enough for simple correction.

Physical Fitness Tests and Records

Note: InformationNOW Health must be used for MO State Reporting Fitness Tests.

• Go to Health | Setup and look up Fitness Area. Create the Fitness Area Codes based on the codes provided in

the InformationNOW MO - Code Setup 2017.xlsx document, available on Chalkable's State Guidelines page at

http://support.sti-k12.com/documentation/stategdocs.asp.

• Go to Health | Setup and look up Fitness Test Type. Create the Fitness Test Types based on the codes

provided in the InformationNOW MO - Code Setup 2017.xlsx document, available on Chalkable's State

Guidelines page at http://support.sti-k12.com/documentation/stategdocs.asp.

• Go to Health | Setup and look up Fitness Results. Create the Fitness Results based on the codes provided in

the InformationNOW MO - Code Setup 2017.xlsx document, available on Chalkable's State Guidelines page at

http://support.sti-k12.com/documentation/stategdocs.asp.

Fitness results for 5th

, 7th

and 9th

graders may be entered on an individual basis or for multiple students by mass-entry.

To enter fitness result individually:

• Go to Health | Student Health and search for the students by Grade Levels of 5, 7, or 9.

• Select all the students and click View.

• Open the Screenings Tab in Health, select Fitness, and click Add.

• After clicking Add, select the fitness test and the fitness result, making sure to mark Report to State.

To enter fitness results for multiple students by mass-entry:

• Go to Health and select Mass Assign Fitness Screenings.

• Search for the students by grade level and click Next.

• Select all students who earned the fitness result and click Add, then Next.

• Select assign results to all students or assign individual results

• Select the Date, Fitness Area, Fitness Test Type, Administered By, Check Box Report to State, and Fitness Result. Click Assign.

Repeat for each Fitness Area. Note that a student cannot be assigned results to the same test more than once.

Omit Fitness Records

To omit fitness records:

• Go to Health | Student Health and search for the student(s).

• Select a student from the results list and click View.

• Screenings tab in Health.

• Select the test and view it. Mark it completed and click OK. Once it has been completed, you may view again

and select an Omit Date and Reason.

Home Rooms

Due to requirements on the PreCode export, any student taking the MAP assessment must be placed in a home room.

The home room may be assigned based on actual home room teacher, grade level or other criteria. Please refer to the

Quick Reference Guide entitled InformationNOW – Homerooms, available on the Documentation page at

http://support.sti-k12.com/documentation/default.asp.

MO State Reports

Prior to State Reporting

To help with data for MOSIS reporting, Quick Entry/Edit may be used to populate required fields. Please refer to the

InformationNOW – Quick Entry Edit Quick Reference Guide for information on using the data entry method.

The following fields should be checked prior to state reporting to confirm that all students have the proper values:

• Demographics Area (Main and Misc. tab of student file):

o Migrant Family (Migrant)

o Reporting School

o Residency Status

• School Area (School tab of student file):

o Reported FTE

• Custom Area (Custom tab of student file):

o Truant

Quick Entry/Edit may be used for other state reported fields not listed here. These fields are listed because the majority

of the students have the same value.

State Reports

To access state reports, go to Reports | State.

Note: The term export is used in the State Reports screen because the data drawn by these reports will

be exported to a designated file format.

Click any report in the Export Type list to view the menu options for that report.

Report Criteria

• Academic Sessions Year: The current year will be selected by default. Use the drop-list to choose a

different academic session’s year if desired. The sessions shown below will change, depending on the year

selected here.

• Academic Sessions: All sessions for all schools will be selected by default. To select a specific academic

session for a specific school, use the scroll bar on the right side of the browse box to view the schools/sessions

and then click the appropriate one. The Ctrl and Shift key functions may be used to select multiple

schools/sessions.

• Start Date and End Date: (Not available for all state reports). To change the date range for the report, click

the button to the right of the start and/or end date field and use the popup calendar to choose the new date.

Make sure to see what dates to enter for each cycle that requires shots.

• Select Report Format: (Not available for all state reports) Reports are by generated, by default, in comma-

delimited-format. However, the report format may be changed to fixed width by clicking the drop-down and

selecting FixedWidth for the fixed-width format.

• Cycle: (Not available for all state reports) Some exports are submitted during various reporting cycles, so the

correct reporting cycle must be selected.

• Staff Classifications: (Not available for all state reports) A Staff Classification is a required filter option that

filters the staff members who are exported in this report. The Ctrl and Shift key functions may be used to select

multiple Staff Classifications.

MOSIS Reporting

Below are the data elements required by each report. The data elements are sorted by common data elements that are

required by several reports and then by report-specific data elements. Each data element contains the MOSIS definition

as defined by DESE and the location of the element in InformationNOW. Not all data elements are listed. Some data

elements are defined in such a way that InformationNOW will automatically export the expected value.

Common Data Elements

Make sure that all used code sets have the appropriate code in the State Code field. If the State Code is missing or

incorrect, either no information or the incorrect information will be extracted in the export file.

• Attending District Code: DESE-assigned 6-digit county district code for the district of attendance.

Location: Attending District under the Custom tab of the student’s Maintenance form.

Note: If the student’s Attending District and School are the Reporting District and School, leave this

field blank.

• Attending School Code: DESE-assigned 4-digit school code for the school of attendance Location:

Attending School under the Custom tab of the student’s Maintenance form.

Note: If the student’s Attending District and School are the Reporting District and School, leave this

field blank.

• Reporting District Code: DESE-assigned 6-digit county district code for the district reporting. Location:

Main tab of the District form under School/District.

• Reporting School Code: DESE-assigned 4-digit code for the school from which the student is reported.

Location: Main tab of the School form under School/District.

Note: If a student is enrolled into more than one school for the reported academic year, the Reporting

School Code is based on the school selected as Reporting School on student Misc tab.

• Resident District Code: DESE-assigned 6-digit county district code for the district of residence. Ex.: K-8

Resident II student attending high school in a different district would have Resident District Code populated

with xxxxxx and the Attending District Code populated with yyyyyy, representing the district of enrollment.

Location: Resident District under the Custom tab of the student’s Maintenance form.

Note: If the student’s Resident District and School are the Reporting District and School, leave this

field blank.

• Resident School Code: DESE-assigned 4-digit school code for the school of residence. Location: Resident

School under the Custom tab of the student’s Maintenance form.

Note: If the student’s Resident District and School are the Reporting District and School, leave this

field blank.

• Teacher Name: Used for sort order. May be teacher/examiner/class name. If the student is assigned to a home

room, the home room will be exported. If no homeroom exists, this will use the Reporting School name.

• MOSIS student ID: State-assigned student identifier. Location: Misc tab of the student’s Maintenance

form.

• Legal Last Name: Legal last name as it appears on certificate of birth. Location: Main tab of the student’s

Maintenance form.

• Legal First Name: Legal first name as appears on certificate of birth. Location: Main tab of the student’s

Maintenance form.

• Legal Middle Name: Legal middle name as appears on certificate of birth. Location: Main tab of the student’s

Maintenance form.

• Legal Name Suffix: Legal name suffix as appears on certificate of birth. Ex.: Jr, Sr. Location: Main tab of the

student’s Maintenance form.

• Date of Birth: Date of birth as appears on certificate of birth. Location: Main tab of the student’s Maintenance

form.

• County: County in which the student resides. Location: Physical Address under the Main tab of the

student’s Maintenance form. If County is blank, student is either missing a county on the Main tab or the

Same as Mailing option is not checked where there is not a Physical Address entered.

• Student Grade Level: Grade level as of the time data is being submitted unless otherwise specified.

Location: Main tab of the student’s Maintenance form.

• Gender: Location: Main tab of the student’s Maintenance form.

• Race/Ethnicity: Location: Main tab of the student’s Maintenance form.

Civil Rights Data Collection (CRDC) Part I The Civil Rights Data Collection (CRDC) is a federally mandated biannual export. Refer to Quick Reference Guide for

CRDC information.

Student Core

Make sure that all code sets used have the appropriate codes in the State Code field. If the State Code is missing or

incorrect, either no information or the incorrect information will be extracted in the export file. To make sure all

applicable students are included on the report, run an ADA/ADM in Hours for the same date range as the state report and

include withdrawn students. The number of students on this report should match the number of students on the Student

Core. If students are missing, this would normally be because their Reporting Schools are blank or incorrect (on

Student’s Misc tab).

Notes about Each Cycle

• October Cycle: Count Date = Last Wednesday in September

• December Cycle: Count Date = Dec 1.

Export a single record for each currently enrolled student where:

SpEd Exit Date is Null or is > Reporting Period End Date

AND

IEP Start Date = or < Reporting Period End Date < or = IEP End Date

Date Enrolled in LEA < or = Reporting Period End Date

NOTE: By using this field, the user should be able to get all current special education students when

the file is generated after Dec 1; with the ASSUMPTION that the student has been served in special

education since the Date of Enrollment in the LEA. However, since there is no history on Student

Exceptionality, we can only send what is currently assigned to the student. If the Exceptionality is

changed after Dec 1, it will have to be manually edited in the file.

• February Cycle: Count Date = Last Wednesday in January

• April Cycle: Start and end date are MAP testing start and end date

• June Cycle: Begin Date is the 1st day of school and the End Date is the last day of school.

• Lunch Status: Located on Main tab of the student’s Maintenance form.

• Gifted: Designation for students who have been identified for and/or have participated in the district's formal

Gifted program. Location: Gifted under Custom tab of the student’s Maintenance form.

• Homeless: The primary nighttime residence is the basis for identifying homeless children and youth. Provide

code that identifies primary nighttime residence. Location: Student Homeless Status on the Misc tab.

Note: If Homeless is not selected for the student, export NH.

• Migrant: A Migrant is a student who has moved across school district boundaries within the preceding 36

months to seek or obtain (or to accompany or join a parent, spouse or guardian who is seeking to obtain)

temporary, seasonal employment in agriculture or fishing, or to work in a beef, poultry or pork processing plant.

Location: Migrant under Misc tab of the student’s screen.

• NotFAYSchool: Y for any student who was not enrolled the last Wednesday in September OR any student

who was enrolled the last Wednesday in September, transfers out of the Academic Session for one more than

half of the in-school/membership days between the last Wednesday in September and the Report Parameter End

Date’, and is re-enrolled on or before Report Parameter End Date Otherwise ‘N’.

• NotFAYDistrict: Y for any student who was not enrolled the last Wednesday in September OR any student

who was enrolled the last Wednesday in September, transfers out of the District for one more than half of the

in-school/membership days between the last Wednesday in September and the Report Parameter End Date, and

is re-enrolled on or before the Report Parameter End Date. Otherwise ‘N’.

• Voluntary Transfer Student: Designation for students who reside in the St. Louis City school district but

who voluntarily enroll in a St. Louis County school district or a student who resides in a St. Louis County

school district but attends the St. Louis City school district. The transfer must have been a result of the

desegregation settlement agreement. Location: Voluntary Transfer under Custom tab of the student’s

Maintenance form.

• A+ student: Required if student is in grade 09, 10, 11 or 12. DESE-assigned A+ codes will designate if a

student is an A+ participate or completer. Location: A+ Tracking under Manage options in the student’s

Maintenance form.

Note: Export ‘Y’ if student has an A+ Program Start Date and A + Program End Date is null. Export

‘N’ if A+ Program Start Date or A+ Program End Date are both null. Export ‘C’ if ‘Eligible’ is

selected on the student’s A+ Program Record.

• Months in USA: Location: 1st Date in US on student custom field.

• Export number of months calculated by subtracting student custom field_1st Date in US from April 1 of the

current school year.

• If null, export blank spaces.

• Round to nearest whole number.

• April (Conditional) - Required if ‘Immigrant’ = Y.

Note: Immigrant must be checked under Misc tab of the student’s Maintenance form.

• Immigrant: Designation for students who are aged 3 through 21, were not born in any U.S. state; and have not

been attending one or more schools in any one or more states for more than 3 full academic years. Immigrant

children and youth will include the children in lawful permanent resident aliens, refugees, asylees, parolees,

persons of other immigrant status, and immigrant residents in the United States without proper documentation.

They will exclude children of foreign diplomats, United States citizens, children who were born abroad, and

children of foreign residents temporarily in the United States for business or pleasure. Location: Misc tab of

Student Maintenance form.

• ELL Primary Language: Required if student is reported LEP status = MY1, MY2, RCV OR NRC.

Location: Misc. tab of the student’s Maintenance form.

• LEP/ELL: DESE-assigned LEP/ELL code set. This code set is used to declare if a student is LEP, ELL

Receiving, first year monitoring or second year monitoring. Location: Misc tab of the student’s Maintenance

form.

Note: If student LEP field is empty, this will export NLP.

• ELL Exit: Method used to re-classify the student as non-LEP. Location: ELL Exit under Custom tab of the

student’s Maintenance form. If null, export blank spaces. If LEP/ELL is MY1 or NLP then ELL Exit is not

allowed

• ELL Exit Test: Language Proficiency Test used to re-classify the student as non-LEP. Data element may only

be reported if the ELL Exit is LPT (Language Proficiency Test). Location: ELL Exit Test under Custom tab

of the student’s screen.

• ESOL Instructional Model: Most frequent LEP Instructional Model used. LEP/ELL must be reported as

"RCV" (Receiving Services) in order for ESOL Instructional Model to be allowed. Location: ESOL Instr

Model under Custom tab of the student’s screen.

• Missouri Option Program: Student who has or is participating in the Missouri Option Program. Must be

reported for students in grades 09, 10, 11 or 12. Location: School Programs under Manage options in the

student’s Maintenance form.

Note: The value of N will export for all students who are not in Missouri Option Program.

• H.S. Career Ed Student: Designation for students in Grades 9-12 who have completed or are currently

taking a Career Education course approved by DESE. Data element not allowed to be reported unless student is

in grades 09, 10, 11or 12. Location: HSCareerEducation under Custom tab of the student’s Maintenance

form.

Note: If HSCareerEducation is blank, the value of NHS will be exported.

• Title I: This identifies a student who is receiving Title I services. In a targeted assistance program, only students

receiving direct Title I services should be designated. All students in a Title I school-wide program should be

designated as Title I. If the school building does not receive Title I funds, none of the students in that school

building should be designated as Title I. You must enter a Service under each student for your Title I to be

reported. This can be done thru Mass Manage under Students. Students must have a Begin and End date that fall

within the current Academic Session. Location: Services under Manage options in the student’s Maintenance

form.

with Service Type state code = T1 start and end dates (student is active in T1 on Report End Date/count

date). Else, N.

o February (Required): Export Y if Report Parameter end Date is on or between Student District Service with

Service Type state code = T1 start and end dates (student is active in T1 on Report End Date/count date).

Else, N.

o April (Required): Export ‘Y’ if Report Parameter Start and End Dates are on or between Student District

Service with Service Type state code = T1 start and end dates (student is active in T1 during MAP testing

date range). Else, N.

o June (Required): Export Y if Student District Service with Service Type state code = T1 start or end date

falls within the Acad Session Date Range (student was served in T1 at least one day during the Acad

Session). Else, N.

o Valid Values: Y (yes), N (no)

• Title III: This identifies an ELL student who is receiving services funded through Title III. Location: Services

under Manage options in the student’s Maintenance form.

o October (Required): Export Y if Report Parameter end Date is on or between Student District Service with

Service Type state code = T3 start and end dates (student is active in T3 on Report End Date/count date).

Else, N.

o December (Required): Export Y if Report Parameter end Date is on or between Student District Service

with Service Type state code = T3 start and end dates (student is active in T3 on Report End Date/count

date). Else, N.

o February (Required): Export Y if Report Parameter end Date is on or between Student District Service with

Service Type state code = T3 start and end dates (student is active in T3 on Report End Date/count date).

Else, N.

o April (Required): Export ‘Y’ if Report Parameter Start and End Dates are on or between Student District

Service with Service Type state code = T3 start and end dates (student is active in T3 during MAP testing

date range). Else, N.

o June (Required): Export Y if Student District Service with Service Type state code = T3 start or end date

• Career Cluster: Required for Secondary Career Education students who are identified as Perkins

Concentrators (please review the HS Career Ed code set for the definition of a Concentrator). 16 Career

• Non-Traditional Student (Secondary): Required for Secondary Career Education students. Classification

is defined as persons entering a Career Education training program or occupation nontraditional to his/her

gender. An enrollment of 75% of one gender is considered traditional. Location: Non Traditional under the

Custom tab of the student’s Maintenance form.

• Single Parent (Secondary): Required for Secondary Career Education students. Classification is defined as

an individual who is unmarried or separated from his/her spouse and is pregnant or has sole or joint custody of a

• Displaced Homemaker (Secondary): Required for Secondary Career Education students. Classification is

defined as an individual who has worked primarily without remuneration to care for a home and family and for

that reason has diminished marketable skills; or has been dependent on the income of another family member

but is no longer supported by that income; or is a parent whose youngest dependent child will become ineligible

to receive assistance under Social Security; and is unemployed or underemployed and is experiencing difficulty

• CTE Technical Skills Attainment: Require for secondary, post-secondary and adult technical education

(CTE) students identified as Perkins Concentrators. Provide the Technical Skill Attainment result or

• School Choice: Identify if a student is eligible, has been offered, has applied for, or has received school

choice. Children enrolled in Title I schools are eligible for school choice when their schools are in the first and

subsequent years of school improvement, corrective action and restructuring. Any child attending such a school

must be offered the option of transferring to a public school in the district not identified for school

improvement, unless such an option is prohibited by state law. No Child Left Behind requires that priority in

Local Tax Effort

Section 167.126, RSMo, requires:

• The domicile school district (not a special school district) to pay an amount to another school district providing

education services to a student placed from the domicile district into that district by the Department of Mental

Health, Department of Social Services, or a court of competent jurisdiction, equal to its average sum produced

per child by the local tax in the domicile district.

• A special school district to pay an amount not to exceed the average sum produced by child by the local tax

efforts of the domiciliary districts.

The educating school district must bill the domicile district for LTE or the educating school district would be subsidizing

the education of non-domicile students at the expense of their own students. The domicile school district of a student is

defined as the school district in which the student’s parent(s) or legal guardian(s) reside.

The average sum produced by local tax effort, more commonly referred to as Local Tax Effort (LTE) is a calculation of

local tax revenue behind each child in a school district. The local tax effort per child is computed by adding the prior

year tax revenue the district of domicile received from property taxes, Intangible (Financial Institution) Taxes, M & M

surtaxes, In Lieu of Taxes, and State Assessed Railroad and Utility Taxes, then dividing that sum by the prior year

resident average daily attendance of the district of domicile. The local tax effort per child in a special school district is

the average sum produced per child by the local tax efforts of the component districts.

Identifying Students Eligible for Local Tax Effort Bill Back The educating school district might identify the following students potentially eligible for LTE bill back:

• Students having a domicile address outside of the district.

• Students who are eligible for the Public Placement Fund may also be eligible for LTE bill backs.

• Students who are placed into DMH facilities.

Direct Certification

This export is used to submit student data for the purpose of determining Direct Certification eligibility. Per State

documentation, Direct Certification is valid from July 1st – June 30

th of the following year.

The Direct Certification process is as follows:

• All parents fill out free/reduced lunch application

• The district submits the applications to the appropriate agencies who determine if the student qualifies for

Targeted Assistance (TANF) or Food Stamps (SNAP)

• DESE/MOSIS retrieves TANF/SNAP eligibility

• District EXPORTS currently enrolled students to MOSIS via Direct Certification state report

• MOSIS produces the Direct Certification Lookup Report (excel file) with TANF and SNAP eligibility indicated

by Y or N

• District retrieves the Direct Certification Lookup Report and optionally IMPORTS to InformationNOW which

updates student lunch program and certification.

Before importing:

• Delete data in Student Address column because it causes problems with the comma count when retrieving the

SNAP/TANF values.

• If the file is in Excel format, it must be converted to a CSV.

Referrals

The main driver behind this is the schoolwide positive behavior supports (SW-PBS) program which has been collecting

this information from participating schools for quite some time via the FormHog system. The collection is being moved

from FormHog to MOSIS.

The data elements in this collection are/could be useful to schools in evaluating their programming and looking at

students’ time spent out of the classroom and services/supports to students beyond the traditional classroom.

While these data have been collected/analyzed in SW-PBS schools, it is good information to look at for any/all schools,

so we are encouraging schools that are implementing a state-supported program to report/analyze the data.

MO Lookup (Codes)

Code Entry and Maintenance

All MO state codes are stored under System Preferences | Setup, along with any non-state-specific codes used for

other reporting purposes. The Lookup drop-list provided here is used to access all types of codes.

Some lookup banks (for example, Special Education Status codes) will be pre-defined by the application and will not

allow codes to be added, changed or deleted. Some individual codes within code banks will be predefined and cannot be

modified or deleted.

User access to and maintenance of Lookup banks may be secured through Permission Maintenance.

Code Name and Description are required fields (marked with asterisks within the program display) but are user-defined.

The corresponding State Code field, however, is used for state reports and must be entered exactly as it appears below.

For example, user may enter a Weapon Type with a code of G and a description of Gun, but the state code of H must be

entered for data to be accurately reported to the MO DESE.
