IMPORTANT INFORMATION

The City of Takoma Park Department of Recreation strives to maintain the highest quality programs and professional standards. Part of those standards are having accurate participant profile and medical in-formation on every child. All forms are available at www.takomaparkmd.gov/recreation. Forms must be completed and submitted prior to your child attending camp.

Participant Information FormThis form can be downloaded from the website or picked up at the Recreation Department office during regular business hours. Completed paperwork must be submitted prior to the start of each camp ses-sion. This form has emergency and general contact info, health info, release information and a behavior agreement.

Authorization for MedicationIf your child will be taking medication during camp time, specific medical forms must be completed priorto attending. The forms may either be down loaded at www.takomaparkmd.gov/recreation or by calling 301-891-7290. Forms must be signed by your physician even for over the counter medicines.

ImmunizationsIf your child is not currently attending a school in the state of Maryland, you must provide a copy of prop-er immunizations upon registration. Note: this will apply particularly to those entering Kindergarten.

For your convenience, you may choose to pay in full at the time of registration or select the pay-ment plan option. This payment plan option, which must be chosen at the time of registration, di-vides the total amount due into three equal payments: 33% of the total fees will be due at the time of registration, with the remaining payments due on or before May 1 and June 1. You must regis-ter early to take advantage of this opportunity as payment due dates will not be changed. For exam-ple, if you register for programs in May 66% of the fees will be due, with the remaining installment due by June 1. Payment plans will not be available after June 1. All payment plans will be assessed a $25 late fee if payment is received more than five days after the due date of May 1 and June 1. Out-standing balances as of June 5, 2013 could result in the child being withdrawn from the program.

Registration Policies* The Recreation Department reserves the right to cancel any program/class due to insufficient enroll-

ment or to substitute teachers without participant notification.* Classes will be canceled on school holidays, vacations, snow days or any other dates noted in the guide.

If schools close midday due to a weather or other emergency, children’s classes will also be canceled. Every attempt will be made to reschedule classes canceled due to weather emergencies, but refunds cannot be guaranteed.

* A resident of Takoma Park is any person who lives within the 2.8 square mile corporate limits. A non-resident lives outside the corporate limits. If you are not sure of your residency status, please call us and we will be happy to assist you.

* In the event that a program has more interest than available space, we will keep a waiting list. Indi-viduals on the waiting list will be accommodated on a first-come, first-served basis. In the event that another class is created, individuals from the waiting list will be contacted.
