The Year of 2006 was a successful one for the Foundation of Physical Medicine and Rehabilitation. Diff erent aspects of the Foundation’s activities contributed to this success. The Foundation benefi ted from an expanded base of support, strength-ened its operations and continued to implement programs to advance phys-ical medicine and rehabilitation.

The Foundation’s Board wishes to thank all who contributed to the Foundation in 2006. The number of contributors and those who gave at the leadership level in-creased signifi cantly. The Foundation cont-inued to receive support from the Academy as part of a fi ve-year plan that will result in the Foundation’s fi nancial independence, makingindividual donors vital to our ongoing existence. The Foundation was able to add to its endowment this year even as it continued a number of current programs.

The Foundation is able to identify more projects than its existing funding can support, so it may seek external grants. During 2006 HealthSouth provided a grant to support a study to compare the Local Coverage Determinations (LCDs) of the Medicare Fiscal Intermediaries to the Academy “Standards for Assessing Medical Appropriateness Criteria for Admi� ing Patients to Rehabilitation Hospitals or Units”.

The current Director, Phyllis Anderson, MA, has provided consistent, energetic and creative leadership and support to the Foundation. During 2006 the Foundation adopted procedures that have signifi cantly reduced its overhead expenses. Most work of the Board and commi� ees is accomplished through conference calls and e-mails. The majority of communications (like this one) are done via e-mail and through our Web site (www.foundationforpmr.org). The overlap in the management of the various funds of the Foundation has been simplifi ed.

This Annual Report includes a complete listing of the goals and sub-goals of the Foundation. The Board decided to allocate most of the Foundation’s programmatic resources towards its Research goals for now. 2006 research programs included the ERF grants and awards, Research Summit follow up planning, the LCD Evaluation Project, planning the Musculoskeletal Research and Educational Grant and participation in the development of the State-of-the Science Symposium of Post-Acute Care: Se� ing a Research Agenda and Developing an Evidence Base for Practice and Public Policy.

The Foundation plans to continue to fund specifi c pilot research projects and participate in collaborative eff orts to expand the evidence base of physical medicine and rehabilitation. It intends to leverage its resources through grants and participation with other organizations with similar interests. The Foundation is designed to serve the entire fi eld of physical medicine and rehabilitation. One of the most visible of the Foundation’s eff orts is the annual awards program. Several awards were presented by the Foundation in 2005.

NEW INVESTIGATOR AWARDThe Education Research Fund (ERF) New Investigator Award is a short-term seed grant for research in topics related to the fi eld of physical medicine and rehabilitation. Eligible applicants for award include any resident in an ACG-ME-accredited residency training pro-gram in physical medicine and rehabili-tation or a physiatrist within fi ve years of completion of an ACGME-accredited residency training program in physical medicine and rehabilitation. Applicants must be members of the American Academy of Physical Medicine and Rehabilita-tion (AAPM&R) and/or the Association of Academic Physiatrists (AAP).

Two 2006 ERF New Investigator Awards of $10,000 each were pre-sented. Yejia Zhang, MD (le� ), from Thomas Jeff erson University, Philadelphia received an award for her project “Role of Human Um-bilical Cord Blood-derived Mesenchymal Stem Cells in Intervertebral Disc Regeneration.” This study seeks to confi rm the hypothesis that cell therapy with human neonatal umbilical cord blood-derived mes-enchymal stem cells (hUCB-MSC) is likely to represent an eff ective tool for slowing or potentially reversing intervertebral disc degenera-tion.

Jerry Chuang, MD (right), from the Univer-sity of Pi� sburgh also received an award for the study, “The Eff ects of Early and Daily Administration of Dilantin on Neuroprotection and Neuroplasticity in an Experimental Model of Traumatic Brain Injury.” The purpose of this study is to use a clinically relevant animal model of experimental traumatic brain injury (the controlled cortical impact model) to determine how early dilantin treatment and daily dilantin treatment aff ects both neuroprotection and neuroplasticity assessed through cognitive and motor skills as well as neurorecovery.

BEST RESEARCH PAPER AWARDThe ERF Best Research Paper Award recognized published work by a physiatrist in any peer-reviewed journal on topics related to the fi eld of physical medicine and rehabilitation. One award is given to a young investigator, the other to an established investigator.

Preeti Raghavan, MD (right) received the young investigator Best Paper Award as the lead author of a paper titled, “Impaired Antici-patory Control of Fingertip Forces in Patients with a Pure Motor or Sensorimotor Lacunar Syndrome,” that was published in Brain, in April, 2006.

The established investigator Best Paper Award was given to “Ran-domized Controlled Trial of Nocturnal Splinting for Active Workers with Symptoms of Carpal Tunnel Syndrome,” published by Robert Werner, MD (le� ) et al in the Archives of PMR 2005; 86: 1-7.

BUILDINGRESEARCH CAPACITY

In 2006, Foundation President Dr. John Melvin appointed a Research Summit Follow-up Commi� ee to direct the Research Capacity Building Initiative to follow through on the fi ndings of the 2005 Research Capacity Building Summit.

Dr. Walter Frontera, commi� ee chair,has solicited the Summit ProgramCommi� ee members; their initialgoals are to review the recommenda-tions made at the Summit; identifythose that have been implementedin one way or another; and proposeand prioritize projects to implementother recommendations.

The commi� ee met regularly throughout the second half of 2006; their initial eff orts included reviewing, updating and prioritizing the recommendations made at the 2005 Research Capacity Building Summit. Many synergies were found in the four primary areas of Researchers; Environment, Infrastructure and Culture; Funding; and Metrics. All of the original Summit participants were surveyed to determine which recommendations have already been implemented, and to start assembling a compre-hensive database of physiatric research resources.

Some activities being considered by the commi� ee include reviewing the current curricula available for training investigators in how to conduct clinical trials, using this infor-mation to develop a current curriculum or white paper on best practices, and identifying exist-ing courses and workshops on conducting research to create a comprehensive database of re-search resources on the Foundation Web site. The commi� ee is making fi nal recommendations to the Foundation Board of Directors to consider for implementation. Progress reports will appear in 2007 editions of the Foundation UPDATE newsle� er.

Concerned about the disparity in access to rehabilitation hospitals and units for Medicare patients, the American Academy of Physical Medicine and Rehabilitation (AAPM&R) assembled an expert task force to develop a statement regarding who are appropriate candidates for admission to these facilities. This Medical Inpatient Rehabilitation Criteria Task Force (MIRC) worked to develop balanced standards that refl ect the consensus of a diverse group of expert physicians on the factors that should infl uence the admission of patients to rehabilitation hospitals and units. Their fi ndings were assembled into the AAPM&R “Standards for Assessing Medical Appropriateness Criteria for Admi� ing Patients to Rehabilitation Hospitals or Units” (www.aapmr.org/hpl/legislation/mirc.htm.)

In 2006, the Foundation initiated a project to compare the standards of the MIRC document to those in the dra� and fi nal Local CoverageDeterminations (LCDs) of the Medicare Fiscal Intermediaries. These included Mutual of Omaha, Riverbend, AdminaStar, BCBS of Alabama, BCBS of Georgia, Cahaba, Medicare Services of Arkansas, Palme� o - North Carolina and Veri-tus. The MIRC standards were also compared to Section 110 of the MedicareBenefi ts Policy Manual. Dr. Irma Fiedler was selected as the lead researcher for the study, which was conducted under the guidance of the LCD Review Project Task Force (see next page.) The project was initiated in July 2006; its main investigative focus was to identify how consis-tently the elements of the LCDs and Section 110 matched those of the MIRC document.

The LCDs were analyzed in a two-step process of: 1) comparing the narratives of the MIRC document vs. the LCDs, and 2) creating an index of items about which they agreed or dis-agreed, or did not mutually address. These fi ndings were displayed on grids that compared each LCD with the MIRC standards and Section 110. The fi nal step will include analysis of individual LCDs and their comparison with the others.

The study found variation among the LCDs. Half of the 10 analyzed diff ered signifi cantly from the MIRC standards. These diff erences uniformly involved restrictions on admissions based upon diagnostic categories. Even Section 110 of the Medicare Benefi ts Manual does not completely meet the MIRC standard of not restricting admissions solely on diagnoses. Section 110 identifi es patients with ankle and hip fractures as not meeting its criteria for admissions.

Dr. Fiedler presented the preliminary fi ndings on Monday, February 12, 2007 at the “State-of-the-Science Symposium on Post-Acute Rehabilitation: Se� ing a Research Agenda and Devel-oping an Evidence Base for Practice and Public Policy” (h� p://www.foundationforpmr.org/programs/postacuterehab.html). Complete results of the study will be submi� ed for publica-tion in a peer-review publication in 2007. They will also be available to the Academy for any future revisions of the MIRC document. The Foundation will consider these fi ndings when planning its support for future research projects.

JUSTUS LEHMANN FUNDJustus F. Lehmann MD (February 27, 1921 - April 28, 2006) was a prominent leader in physical medicine and rehabilitation, both na-tionally and internationally. Born in Königsberg, Germany, he stud-ied medicine at Johann Wolfgang Goethe University in Frankfurt and at the University of Leipzig. Dr. Lehmann was initially trained in internal medicine but an interest in physical modalities led him to study at the Max Planck Institute of Biophysics. In 1951 he moved to the United States to further pursue a study biophysics and physical medicine and rehabilitation (PM&R) at the Mayo Clinic, under the tutelage of Frank H. Krusen, MD.

Dr. Lehmann was recruited to serve as the founding chairperson of the Department of Rehabil-itation Medicine at University of Washington School of Medicine. He headed the department from 1957 to 1986; it was consistently ranked among the top PM&R departments in the United States under his leadership.

During his distinguished career, Dr. Lehmann was the recipient of American Congress of Re-habilitation Medicine’s Gold Key Award in 1971, the American Academy of Physical Medicine and Rehabilitation’s Frank H. Krusen Award in 1983, and the Association of Academic Physiat-rists’ Distinguished Member Award in 1993.

In memory of the contributions Dr. Justus Lehmann made to the fi eld, Dr. Kristjan Ragnarsson and other members of the Foundation Board are establishing a Lehmann Fund. Donations are currently being solicited; we hope to provide an education/research grant or award through the fund in the future. Over $45,000 has been collected in contributions and pledges already. A comprehensive list of donors will appear in future issues of the Foundation UPDATE news-le� er and next year’s Annual Report.

Dr. Joel Press, AAPMR President and Medical Director of the Spine and Sports Rehabilitation Center at the Rehabilitation Institute of Chicago, is undertaking a 3,861-mile cross-country bicycle ride from San Francisco to Boston on June 12, 2007 to build awareness of the role of physiatry and to raise funds for rehabilitation out-comes research. The Ride will take him from San Francisco to Boston, site of the 2007 AAPM&R Annual Assembly.

The Foundation is coordinating with the Rehabilitation Institute of Chicago to promote Dr. Press’ Ride for Rehab through a national media campaign. The Ride for Rehab 2007 will promote awareness of the critical role physiatry and rehabilitation play in restoring function and improving quality of life for patients.

As part of the Ride for Rehab campaign, the Foundation for PMR is seeking donations from physiatrists and their patients, as well as corporate supporters. The Academy Resident Phy-sicians Council is coordinating local fundraising events in conjunction with their medical schools. All funds raised by the Ride for Rehab 2007 will be used for rehabilitation outcomes research.

Please support this eff ort by making a contribution to rehabilitation outcomes research that will continue to determine the best course of treatment for individuals who have suff ered a loss of function due to illness or injury.
