Amputa*on Sta*s*cs
• In the US, there are approximately 2 million people living with limb loss
• 185,000 amputa*ons in US each year – Congenital Amputa*ons: ~ <1% – Tumor: <2% – Trauma&c: ~ 45%
• Males > Females – Dysvacular causes such as peripheral vascular disease (PVD), Diabetes Mellitus (DM) or Chronic Venous Insufficiency (CVI) : ~ 54%
• Over half of these are diabe*c

Reasons for Trauma*c Amputa*ons
• Machinery • Power tool/appliance • Motor cycle • Lawnmower • MVA • Gunshot wounds • Natural disasters, war,
and terrorist a<acks can also cause trauma&c amputa&ons.
![Page 5: 0930 Payne lecture Early amp recovery Trauma conf 9-13.pptx ......0930 Payne lecture Early amp recovery Trauma conf 9-13.pptx revised 9-12pptx.pptx Author Candance Van Vleet Created](https://reader035.vdocuments.mx/reader035/viewer/2022070215/6117491d1e850d2d8d169a64/html5/thumbnails/5.jpg)
Limb Salvage vs Amputa*on
• Age old trauma dilemma • Obviously, in some cases there is no choice • Considera*ons:
– First & foremost…….pa*ent survival & safety – Time: how long will salvage take, months? Years? – How many surgical procedures? – Pa*ent’s lifestyle…gebng on with one’s life – What limb is involved and what level of poten*al amputa*on?
![Page 6: 0930 Payne lecture Early amp recovery Trauma conf 9-13.pptx ......0930 Payne lecture Early amp recovery Trauma conf 9-13.pptx revised 9-12pptx.pptx Author Candance Van Vleet Created](https://reader035.vdocuments.mx/reader035/viewer/2022070215/6117491d1e850d2d8d169a64/html5/thumbnails/6.jpg)
Decision-‐making in Trauma
“a salvaged upper extremity oeen func*ons befer than currently available prosthe*c replacements, while a salvaged lower extremity is oeen worse than prosthesis unless it can tolerate full weight-‐bearing, is rela*vely pain-‐free, and has durable skin and soe *ssue coverage that does not break down when walking is afempted”
– Dr. Douglas Smith, University of Washington
![Page 7: 0930 Payne lecture Early amp recovery Trauma conf 9-13.pptx ......0930 Payne lecture Early amp recovery Trauma conf 9-13.pptx revised 9-12pptx.pptx Author Candance Van Vleet Created](https://reader035.vdocuments.mx/reader035/viewer/2022070215/6117491d1e850d2d8d169a64/html5/thumbnails/7.jpg)
Correct surgery, excellent nursing care, immediate rehabilita6on/training, and a well-‐fi;ng prosthesis are all equally
important!
Rehabilita&on should be both: • Physical • Psychological

Post Trauma*c Stress Disorder (PTSD)
• Post-‐trauma*c stress disorder (PTSD) is a mental health condi*on that's triggered by a terrifying event. Symptoms may include flashbacks, nightmares and severe anxiety, as well as uncontrollable thoughts about the event.
• Resolves with *me-‐ Many people who go through trauma*c events have difficulty adjus*ng and coping for a while. But with *me and taking care of yourself, such trauma*c reac*ons usually get befer.
• Unresolved-‐ In some cases, the symptoms can get worse or last for months or even years. Some*mes they may completely shake up your life. In a case such as this, you may have post-‐trauma*c stress disorder.

Amputa*on Surgery/Dressing
• Edema Control • Wound healing • Eleva*on for several days, proper loca*on of pillow, not under the joint
• Surgical management – retain the knee when possible – Requires less energy to walk – Prosthe*c fibng easier to manage
![Page 14: 0930 Payne lecture Early amp recovery Trauma conf 9-13.pptx ......0930 Payne lecture Early amp recovery Trauma conf 9-13.pptx revised 9-12pptx.pptx Author Candance Van Vleet Created](https://reader035.vdocuments.mx/reader035/viewer/2022070215/6117491d1e850d2d8d169a64/html5/thumbnails/14.jpg)

Wound Healing • I. Primary: heals without open areas, infec*on, or wound
complica*ons. • II. Secondary: small open areas that can be managed, and
ul*mately heal with dressing strategies and wound care. • III. Requires minor surgical revision: skin and subcutaneous
*ssue (no muscle, no bone). • IV. Requires major surgical revision involving muscle or
bone: but heals at ini*al amputa*on "level.” • V. Requires revision to a higher amputa&on level: for
example, a trans*bial amputa*on that must be revised to either a knee disar*cula*on or a transfemoral amputa*on.

Acute Post-‐Surgical
• Pain control • Peer visitor • Joint ROM and strength of both lower and upper extremity
• Wound healing • Phantom limb pain/sensa*on management • Func*onal mobility training • Equipment needs-‐ walker/crutches/wheelchair
– If Acute Rehab, they will order equipment

• Starts on admission • Surgeon-‐ decisions for best level, saving most func*on, healthiest *ssue
• Nursing-‐ posi*oning, pain management • Discharge planning
– Acute Rehab-‐ inpa*ent, 3 hours of 2 therapies daily (PT/OT)

Peer visit • “I remember thinking that
living life with one leg was worse than not living life at all. I really, honestly did…[Then] a young boy with an ar*ficial leg came into my hospital room for a visit. I don't remember what he said. I don't remember what he looked like. I was so transfixed on his prosthesis as he walked into my room. That's all I needed to give me the personal strength to go on…” Ted Kennedy, JR
