• Lysosomal storage diseases are a group of
approximately 50 inherited metabolic disorders that
result from defects in lysosomal function.

• โรคโกเชร (Gaucher's disease) เปนโรคความผดปกตของการ
สะสมไขมน (lysosomal storage disease) เปนโรคทางพนธกรรม
ทถายทอดทางออโตโซมลกษณะดอย มลกษณะเฉพาะคอมการ
สะสมของ glucocerebroside ในเซลลแมคโครฟาจและโมโนซยต
• โรคนเปนผลจากภาวะพรองเอนไซม lysosomal hydrolase หรอ

GAUCHER DISEASE: Classification
• Type 1 is the most common and is non-neuronopathic, Type 2 and 3
are rare and neuronopathic.

• Gaucher disease is caused by a deficiency of glucocerebrosidase that
is located within the lysosomes and is responsible for breaking down
glucocerebroside into glucose and a fat called ceramide.

• In people with Gaucher disease, as a result of
glucocerebrosidase deficiency, glucosylceramide
collects in macrophages.
• Cells containing this undigested lipid become
swollen. These cells, called Gaucher cells, most
commonly accumulate in the liver, spleen and bone
marrow.

cytoplasm.

• Gaucher cells are characterized as large, pale, polyhedral shaped cells
possessing a single, relatively small, eccentric nucleus and weakly
eosinophilic cytoplasm containing indistinct striations which impart a
"wrinked tissue paper" appearance.

• Upper leg and knee - Radiograph
Erlenmeyer flask deformity of the distal femur.
• Widening of the distal femur and thinning of cortical bone,
especially in the long bones and vertebrae, are
characteristic of Gaucher disease.
• The bony abnormalities are considered to be related to the
large numbers of lipid-laden macrophages (Gaucher cells) in
the bone marrow, but the mechanism of bone deformation
and destruction in this disease remains unclear.

สะสม glycogen ทตบและกลามเนอมากกวาปกต
เนองจากขาดเอนไซมทเกยวของกบ glycogen

• GSDs are usually first diagnosed in babies and children. The common
features, particularly for type I, include:
• Abnormally slow growth
• Low blood sugar levels which can cause symptoms such as sweating,
tremor, drowsiness, confusion and sometimes convulsions.
• Enlarged liver
• Noticeably weaker muscles
• Obesity
• Problems with bleeding and blood clotting
• Kidney problems

• Phenylketonuria (PKU) is the first metabolic disorder which
has been shown to be treatable through dietary restriction
with a great measure of success.
• PKU occurs in approximately 1 in every 20,000 live births in
North America.

• Phynylketonuria is a disease very similar to alkaptonuria in
that an enzyme is non-functional, in the same metabolic
pathway, but there the similarity ends.
• Whereas alkaptonuria is not life threatening, PKU is.
• PKU is defined by a mutation in the enzyme Phenylalanine

• Without this enzyme Phenylananine will build up in the
blood stream.
• Phenylalanine is non-polar, and so is not easily excreted in
the urine, one of the minor derivatives of phenylalanine -
phenylpyruvic acid - is toxic to the cells of the Central
Nervous System (CNS).

• Symptoms of PKU can range from mild to severe that can appear
normal for the first few months of the life. If the child is not treated
for PKU during this time, he or she will begin to develop symptoms as:
• Intellectual disabilities or mental retardation
• Seizures, tremors or jerky hand and leg movements
• Hyperactivity
• Stunted growth
• A distinct odor in breath, skin, or urine that is often described as
musty
• Eczema, lighter skin, hair, and eye color than their family members

It is very important
to avoid the sugar
substitute aspartame
(Equal, Sweetmate).
Aspartame contains
high amounts of Phe.
It can quickly raise
the blood levels of
Phe in people with
PKU.

PHENYLKETONURIA
• In 1965 an American doctor, Dr. Robert Guthrie, created a
relatively simple procedure to test for phenylketonuria in
newborns.
• The Guthrie test has been in use worldwide since then, with
many children born with PKU growing up to lead normal,
healthy adult lives.

• ปจจบนพบวายนทเกยวของกบการเกดโรคนคอ CF gene อยบน
โครโมโซมคท 7 ผปวยไดรบยนจากทงพอและแม สวนผทเปน
พาหะจะไดรบจากพอหรอแมคนใดคนหนง
• อาการของเดกเกดขนกอนอาย 3 ป พบวาเอนซยมจากตบออนซง
ท าหนาทยอยสารอาหารไขมนลดนอยลง เรยกภาวะนวา
pancreatic insufficiency ท าใหเกดปญหาการดดซมไขมนจาก
ล าไสบกพรอง เดกจงขาดวตามนทละลายในไขมน ไดแก วตามนเอ
ด อ เค สงเกตเหนไดวาอจจาระของเดกจะมนเยม ปรมาณกาซใน
ทองเพมมากขน มองเหนลกษณะทองโปงขน ผปวยมอาการปวด
ทองหรอจกแนนทอง

The sweat electrolyte test is performed to determine the amount of chloride that is excreted in sweat from the body during a certain period of time. The test may be performed on infants to determine if cystic fibrosis is present. Children with cystic fibrosis have increased sodium and chloride concentrations in their sweat.

ความผดปกตเกดขนกบยน fibrillin 1 (FBN1) บนโครโมโซม 15q
ซงมผลท าใหมการสราง microfibrillar fiber ลดลง เปนผลท าให
การสราง elastin ลดลงดวย ท าใหปรมาณเนอเยอเกยวพนใน

ACQUIRED METABOLIC DISORDERS
• Gout (also known as podagra when it involves the big
toe) is a medical condition usually characterized by
recurrent attacks of acute inflammatory arthritis—a red,
tender, hot, swollen joint.
• The metatarsal-phalangeal joint at the base of the big toe is
the most commonly affected (approximately 50% of cases).
The Gout (James Gillray, 1799) depicts the pain
of the artist's gout as a demon or dragon.

• Iit may also present as tophi, kidney stones, or urate
nephropathy. It is caused by elevated levels of uric acid in
the blood. The uric acid crystallizes, and the crystals deposit
in joints, tendons, and surrounding tissues. characterized by
deposition of urate crystals in joints and other tissues.
• Transient acute episodes of arthritis may progress to chronic
gouty arthritis.
