Ziram (Ramipril Tablets), is an angiotensin-converting enzyme (ACE) inhibitor, which is used to treat high blood pressure (hypertension) and congestive heart failure. ACE inhibitors decreases your bodys production of substances that could raise your blood pressure and thus relax the muscles around small arteries (arterioles). The arterioles expand and allow blood to flow through more easily. This reduces blood pressure.

Ziram tablets, manufactured by FDC Limited, India to treat high blood pressure (hypertension), to reduce the risk of you having a heart attack or stroke, to reduce the risk or delay the worsening of kidney problems, to treat your heart when it cannot pump enough blood to the rest of your body (heart failure) and as treatment following heart attack (myocardial infarction) complicated with heart failure.

Ziram (Ramipril Tablets) have to be stored at controlled room temperature i.e. from 20C to 25C (68F to 77F) with excursions permitted between 15 to 30C (59 to 86F). Keep this as well as all other medicines away from children and pets.

Hypertension: The recommended starting dose of Ziram is 2.5 once daily in patients not receiving a diuretic. Dosage must be adjusted according to the blood pressure response.
Reduction in Risk of Myocardial Infarction, Stroke and Death from Cardiovascular Causes: Ziram (Ramipril Tablets) must be administered at an initial dose of 2.5 mg, once a day for 1 week, 5 mg once a day for the next 3 weeks and then increased as tolerated, to a maintenance dose of 10 mg once daily. In case the patient is hypertensive or recently post-myocardial infarction, it can be given as a divided dose.

Heart Failure Post-Myocardial Infarction: For the treatment of post-myocardial infarction patients who have shown signs of congestive heart failure, the recommended initial dose is 2.5 twice daily (5 mg daily). A patient who becomes hypotensive at this dose may be switched to 1.25 twice daily. After one week at the starting dose, patients should be titrated (if tolerated) toward a target dose of 5 mg twice daily, with dosage increases being about 3 weeks apart.

Contraindications
Ziram is contraindicated in patients with a hypersensitivity to Ramipril or any other ACE inhibitor or any other component of this medication. Do not co-administer Ramipril tablets with aliskiren containing medicinal products in patients with diabetes mellitus or with moderate to severe renal impairment.

The most common Ziram side effects for patients with hypertension include include headache, dizziness, fatigue, and cough.
The most common reasons for discontinuation were: cough (1.0%), dizziness (0.5%), and impotence (0.4%)
Other side effects include headache, non-productive, tickling cough, angioedema, bronchitis, sinusitis, dyspnoea, gastrointestinal inflammation, digestive disturbance, rash in particular maculo-papular, muscle spasm, myalgia, hyperkalemia, neutropenia/ agranulocytosis, chest pain and orthostatic BP decrease.

Limited data on human Ramipril overdosage are available. The most likely clinical manifestations would be symptoms attributable to hypotension.

Due to the fact that the hypotensive effect of Ramipril is achieved through vasodilation and effective hypovolemia, it is reasonable to treat Ramipril overdose by infusion of normal saline solution.

Ziram (Ramipril Tablets) has been classified by the US FDA as Pregnancy Category D. Use of medicines that act on the renin-angiotensin system during the second and third trimesters of pregnancy reduces fetal renal function and increases fetal and neonatal morbidity and death. There is positive evidence of human fetal risk based on adverse reaction data from investigational or marketing experience or studies in humans, but potential benefits may require use of the medicine in pregnant women despite potential risks.
