We want you to be as healthy as can be, so we’ve listed the top five things that will help

you get the most out of your Anthem benefits.

Healthy Indiana Plan Startup Guide!

Your doctor or primary medical provider (PMP) is the first person you call about your health care needs. Your PMP will help you with things like:

Checkups and vaccines.

Referrals to specialists.

Referrals for tests and services.

Admission to a hospital.

To make sure you get care right away, we’ll choose a PMP for you. If you want a different PMP, log in or register at www.anthem.com/inmedicaid, click on Change Your PMP and follow the steps to choose a new one. Or download the free Anthem Medicaid app from the App Store or Google Play to find doctors, view claims and more. If you need help, call us at 1-866-408-6131 (TTY 711).

Be sure to make an appointment with your PMP even if you’re not sick. Your PMP needs to get to know you to give you the best care.

Seeing your PMP within 90 days of joining Anthem is the first step to good health.

Complete the Health Needs ScreeningThe Health Needs Screening (HNS) is very important.It’s a list of questions that helps us know what kind of care you may need. We’ll call you to help you take the HNS. Plus, we’ll reward you for it!

Once you complete the HNS, you’ll earn $10 as part of the Anthem Rewards program to make Walmart purchases.

If you’d like to complete the HNS on your own, you can: Visit a Pursuant Health kiosk in a nearby

Walmart pharmacy. Go online to www.anthem.com/HNS. Call 1-866-408-6131 (TTY 711).

To get your reward, you’ll need to

complete the HNS within 90 days

of joining Anthem. So don’t wait!

You can also earn other rewards

in the future. Visit www.anthem.com/AnthemRewards to learn

about other incentives like quitting

smoking and wellness visits.

Use your POWER AccountWith HIP, you have a special savings account called a Personal Wellness and Responsibility Account, or POWER Account for short. It gives you $2,500 to pay for certain health care costs.Use your POWER Account to get the health care you need. After $2,500 is used in your POWER Account, Anthem will begin paying for your care.

If you’re a HIP Plus member, you must make a monthly payment or POWER Account contribution. Keep your HIP Plus benefits by following these steps:

1. Pay your POWER Account contribution every month.

2. Make sure your contribution is not late. See your member handbook for the many ways you can make your contribution.

Here’s another benefit: Preventive care could help save you money.If you get certain types of preventive care and there’s money left in your

POWER Account at the end of your benefit year, HIP Plus members may pay

less. HIP Basic members who choose to move to HIP Plus can pay less and

get more benefits, too. See your member handbook or call Member

You pay a low monthly payment based on your income, but you have no copays for doctor visits or prescriptions. If you’re a HIP Basic member, you can choose HIP Plus when you renew your benefits.

Get preventive careGood health starts with preventive care.This is the care you get when you go to your PMP before you get sick.

You should see your PMP at least once a year for a wellness checkup. This checkup gives your PMP the chance to get to know your health care needs. You should also see your PMP for shots and exams, like mammograms for women or prostate cancer screenings for men. We’ll send you reminders about your wellness checkups and exams so you don’t miss them.

What is redetermination?It’s the time of year when you have to make sure you’re

still eligible for benefits. When you enroll in HIP, you have

benefits for 12 months. The state will send you a letter

and may ask you for information. You must send this

information on time or, you could lose HIP for six months.
