Mantener niveles DNA-VHB indetectables en pacientes AgHBe (-) o AgHBe (+) sin seroconversión antiHBe 2.- Seroconversión antiHBe en pacientes AgHBe (+) 3.- Pérdida AgHBs +/- aparición antiHBs La normalización de las transaminasas es un objetivo adicional que se alcanza en la mayoría de los pacientes con supresión viral

Recomendación
Pacientes naïve • AN de elevada barrera genética de manera prolongada,
independientemente de la gravedad de la lesión hepática
• Preferentes: ETV, TDF y TAF en monoterapia
• No se recomiendan LAM, ADV y LdT
Monitorización • Evaluación periódica de ALT y HBV DNA
• Control de función renal y fósforo en sangre
• Pacientes con TDF y enfermedad renal u ósea subyacente, cambiar a
ETV o TAF

Suspensión del tratamiento
• No se recomienda retirada de tratamientos en pacientes
cirróticos
• Suspender los AN tras la confirmación de la ausencia de
HBsAg con sin seroconversión anti-HBs
• Se pueden suspender en hepatitis crónica HBeAg+ no
cirróticos que seroconvierten y tienen ADN-VHB indetectable
tras al menos 12 meses de tratamiento de consolidación
• Se puede considerar en HBeAg- no cirróticos que tienen ADN-
VHB indetectable durante al menos 3 años en tratamiento con
AN si se asegura un seguimiento cercano
