Health System (StarCare) iscurrently seeking to expand its
network of providers for thefollowing Community First Choice
and Community Support services toindividuals with intellectual and
developmental disabilities residing inthe community: • Personal
Attendant Services/HabilitationServices • Supported Home
Living/Transpiration ServicesContracted service providers will
provide assistance to individuals toparticipate in age appropriate
community activities and services.The services will be provided face toface and will include provision of as-sistance, training and support neces-sary for the individual to completepersonal care, health maintenance
and independent living task. All inter-ested candidates meeting minimum
qualifications, inclusive of back-ground checks, will be admitted intothe network of providers upon the
successful execution of their individu-al contracts. Contracted providers
will receive reimbursement of $14 perhour for the provision of services.
There is no guarantee of utilization ofany contracted provider as each indi-vidual receiving services will select
their service provider. Hours are con-tingent upon authorization based on
