E ach year, on October 27, people world-wide raise awareness of a severe, painful inflammatory disease that affects about 125 million people worldwide — psoriasis.

It’s normal for skin cells to mature and shed over the course of a month. But in a per-son who has psoriasis, autoimmune signals cause

skin cells to form in three to four days — pil-ing up on the skin and causing thick, red,

and scaly patches of varying sizes. Dur-ing a flare-up, which can be triggered by

stress and cold weather, the skin can be itchy, painful, and tender.

Noticeable symptoms of a chronic skin condition like psoriasis can often lead to some patients suffering from so-cial isolation and even clinical depres-

sion. “Just imagine what it would be like to see someone cross the street to avoid

interacting with you,” says Dr. Lyn Guen-ther, a professor of dermatology at Western

University in London, ON. Topical therapy, which includes creams,

gels and foams, is viewed as a frontline therapy to treat psoriasis. Almost all patients use a topical treatment either on its own or, in more severe cases, in combination with treatments such as photother-apy, oral, or injectable medications.

The various topical treatments work by slowing down or normalizing cell reproduction and reducing inflammation of the psoriasis patch or plaque. The

most commonly used contain a steroid or synthet-ic forms of vitamin D, or a combination of both. Ster-oids reduce inflammation and vitamin D slows the growth of skin cells that build up in psoriasis patients.

Other topical therapies include coal tar, which helps slow the growth of skin cells, and salicylic acid, which smoothes the skin by promoting the shedding of psoriatic scales. There are also prod-ucts that contain aloe vera, jojoba, and other in-gredients that help remove psoriatic scales as well as moisturize, soothe, and relieve itching.

There is an effective treatment for almost every psoriasis patient. “If older therapies have not been effective previously ask about new ones,” says Dr. Chih-ho Hong, a board-certified dermatologist and founder of SkinFitMD, a cosmetic dermatology clin-ic in Surrey, BC. “There are a lot of options out there.”

Advances in treatmentsAlthough topical formulations can be effective, they can be inconvenient to apply. But thanks to con-tinued efforts to develop effective, safe, and conven-ient therapies, new topical treatments have recently been introduced. Foam preparations of corticoster-

oids, for instance, are well accepted by patients, and may lead to improved adherence to therapy.

One such addition to the available topical treat-ments for psoriasis is a spray foam formulation that combines a steroid and vitamin D. This treatment provides good penetration of the active ingredients into the skin, and improved control of psoriasis symp-toms, compared to the same ingredients in other for-mulations. The foam delivery system provides a pa-tient-friendly and convenient application; it works in much the same way a spray sunscreen does.

There are also other new approaches to topical psoriasis medication in development, including the use of nanostructured lipid carriers loaded with the psoriasis medication, methotrexate, to deliver the medication right to the skin.

Dr. Ron Vender, a dermatologist based in Hamilton, ON, anticipates the options for psoriasis patients will continue to grow. “I predict, down the road, we’ll see a variety of new, safer, and more effective therapies.”

Finding the right therapy through conversationTo find the best treatment, patients should sched-ule an appointment with a board-certified derma-tologist and ask him or her informed questions. Finding the best medication can be a matter of trial and error for someone with psoriasis, but it’s well worth the effort to improve quality of life.

Everyone loves the holiday season — what with the carols, the big man in a red suit, along with festive feasts and holiday parties. But if you want to feel beautiful and confident this holiday season, there are some skincare myths worth nut-cracking.

➊ You don’t need UV protection in the winter

It’s freezing cold and the sky is gunmetal grey, but don’t be fooled. The sun is shining and UV radiation is contributing to wrinkles, fine lines, and even age spots and discoloration. “If you can see your hand in front of your face there is enough sun for you to be wearing sun-screen,” says Dr. Julia Carroll, a dermatologist at Compass Dermatology in Toronto.

Specialists recommend a sunscreen with broad-spectrum protection from both UVB and UVA. 5 WINTER TIPS FOR ECZEMA + DRY SKIN

W inter is coming — and as the sea-sons change, so does our skin.

However, Toronto derma-tologist Dr. Julia Carroll says the dry, dull skin that often ac-

companies colder months isn’t necessarily con-nected to the weather.

“The big thing that affects skin in the winter is humidity,” she says. “A lot of people blame it on the harsh outdoor conditions, but it actually has a lot do with the indoor conditions: dry, forced air-type heating, which is low in humidity, can real-ly affect our skin.” She adds that these conditions can also exacerbate eczema symptoms.

In order to keep your skin feeling soft and smooth all winter long, Dr. Carroll offers the fol-lowing top tips.

Get a humidifier “Our skin has high water content, so when the air is dry it’s trying to balance itself by pulling water from anywhere it can, including our skin,” ex-plains Dr. Carroll. In order to retain your skin’s moisture and keep eczema from worsening, she recommends using a humidifier, particularly in the bedroom, since it is a confined space where we spend (hopefully) around eight hours per night.

Cool it with the heat It can be tempting to crank the heat in the win-ter, but Dr. Carroll explains “higher temperatures are also going to dry out the air more.” If possible, she advises turning down the heat before turn-ing in for the night.

Moisturize daily The best way to combat dry skin is to replenish lost moisture by applying hydrating creams — or creams specific to sensitive or eczema-prone skin — regularly. These lotions can help reduce eczema flare-ups and act as a protective barrier when ven-turing out into harsh outdoor conditions. “The best time to moisturize is when you first get out of the shower and still have a bit of the water left on your skin,” says Dr. Carroll. “Pat dry and apply moisturiz-er on top and it will lock in some of that water.” She adds that exfoliating once a week can get rid of dry skin and help moisturizers be better absorbed.

Shorten showers The shower can be a welcome respite from the cold weather, but standing under hot water for long periods of time can further dry out skin and irritate areas with eczema. Keeping your routine short and the temperature warm — not hot — will go a long way for your skin.

Don’t forget sunscreen Summer isn’t the only time you need to protect your skin from the sun’s UV rays. While UVB levels go down in the winter, UVA rays stay con-sistent and can damage your skin. “It’s import-ant even on the bleak days of winter to wear sunscreen,” says Dr. Carroll.

Winter has not yet hit in full force, but it’s never too early to take advantage of these tips. In fact, Dr. Carroll advises starting your cold-weather skin-care regimen as soon as you bring out the winter coats, in order to keep skin from drying out, par-ticularly for those with eczema: “It’s always hard to catch up, it’s better to prevent the issue.”Ishani Nath

A s the years change, so does our skin — but thanks to new anti-aging treat-ments, dermatologists can now turn back the clock for patients.

Toronto dermatologist Dr. Sheldon Pollack explains that there are both internal and ex-ternal factors that can cause our skin to wrinkle and sag — starting with the loss of the skin’s key struc-tural components, such as collagen and elastin.

“This loss causes the skin to be both less plump and less tight,” says Dr. Pollack, adding that sun damage also contributes to the loss of elasticity and further aging.

As we enter our 40s, we begin to lose fat in our skin. The result is that our face appears to “deflate like a mattress,” says Dr. Pollack, which causes our skin to fall lower. Gravity will pull sagging skin low-er with time — which can mean more pronounced lines around the corners of the mouth and chin, deeper furrows, and cheeks that turn to jowls.

Filling in what’s missing “Volume loss has been identified as one of the ma-jor signs of aging, and it leads to thin, sagging, tired looking faces,” says Dr. Pollack. But he explains that treatments, such as injectable dermal fillers, can help skin bounce back. Belotero is a range of dermal fillers that can be used to help reduce the look of finer lines, folds, and furrows and also safely help to fill out deeper wrinkles and combat the deflated

look of aging skin. He compares it to re-inflating the cheeks that have lost fat, which helps return any sagging skin back to its original position, “just as putting air in a mattress lifts it higher off the floor.”

Depending on the patient, Dr. Pollack sometimes opts to use Radiesse, another form of dermal filler.“We use Radiesse for facial contouring of cheeks, jaw lines and temples, where significant sagging and volume loss require superior filling and lifting effects,” says Dr. Pollack, adding that he has also used this dermal filler to help rejuvenate the look of aging hands.

At his downtown practice, Dr. Pollack uses Ra-diesse and Belotero dermal fillers to combat signifi-cant sagging and volume loss — consistently bring-ing his patients’ faces back to their natural, youth-ful shape and look.

Doctor’s advice When it comes to protecting against the effects of aging, Dr. Pollack says taking care of your skin by using sunscreen (SPF 45 or higher) daily and meeting regularly with a dermatologist will help you put your best face forward for years to come.

“Do not wait until the skin is aged beyond re-pair,” he advises. “At the first sign of worrisome lines, folds and wrinkles, consult a skin expert for advice on how to proceed with keeping the skin looking as young and natural as possible.”
