4. Name and List of Equipments and Machinery alongwith the number,installed capacity and horse power used .

5. Photo 1.0. and address proof issued· by Government authori ty ofProprietor/Partn erlDirector(s)/Authorised Signatory.

6. List of food category desired to be manufactured. (In case ofmanufacturers).

7. Authority letter with name and address of responsible personnominated by the manufacturer alongwith alternative responsibleperson indicating the powers vested with them viz assisting the officersin inspections, collection of samples, packing and dispatch.

8. Analysis rcportjChemical and Bacteriological) of water to be used asingredient in food from a recognized! publ ic health laboratory toconfirm the portability indicating the name of authorized representativeof Lab . who collected the sample and date. of collecting sample .
