That’s why we offer a comprehensive, integrated wellness program to help you and your family live life to the fullest, now and in the future.

There’s something for everyone in this program, whether you’re looking to maintain or improve your health. The program takes a holistic approach to wellness, with a variety of services on offer. You and your family* can participate in the wellness program at no cost, and some services are available regardless of whether you are enrolled in our Company health plan. *Wellness programs are available for employees, spouses, and dependents through the end of the month following their 26th birthday. With Virgin Pulse, our Wellthy for Life program administrator, you can track your steps and healthy habits, find wellness initiatives, and see your Virgin Pulse and Wellthy points in one place. The program runs Jan. 1-Dec. 31 each year and is 100% confidential. If you have a Virgin Pulse account, sign in to get started. If you don’t have an account, you can create one. New members can order a free MAX BUZZ fitness tracker (similar to a Fitbit) or receive a credit of $24.99 toward a different fitness-tracking device through the Virgin Pulse store. This FAQ can help you successfully navigate the new site. The Virgin Pulse app Virgin Pulse mobile puts the program in the palm of your hand anywhere, any time. Staying connected to your wellness goals is easy with the Virgin Pulse mobile application, available for Apple or Google devices. Confidentiality All personal health information provided through participation in our company wellness programs is completely confidential. Employees will not be able to view their spouse’s information, and vice versa. The company will never see your personal health information in connection with your participation in any of our wellness programs. Wellness Programs and Wellthy for Life Wellness Activities Wellthy for Life wellness programs are designed to help you make changes to improve your health and overall quality of life. Wellness programs are available to all active full-time, part-time, and temporary team members and eligible family members*, regardless of whether you’re enrolled in our Company health plan. *Wellness programs are available for employees, spouses, and dependents through the end of the month following their 26th birthday.

All employees may earn up to 1,000 Wellthy Points per year through Wellthy for Life. For each wellness activity completed, you receive a designated number of Wellthy Points. For detailed information about each activity that earns Wellthy Points, read on. For a quick overview, refer to the Wellthy Points Guide, which is also available on the Programs tab of Virgin Pulse. For employees enrolled in our Company

health plan, Wellthy Points are automatically converted into dollars in a Health Reimbursement Account (HRA) if you’re enrolled in the Choice 80/20 Traditional PPO Health Plan (PPO), or in a Health Savings Account (HSA) if you’re enrolled in the Choice 80/20 High Deductible Health Plan (HDHP). These funds may be used to cover eligible medical, dental, and vision expenses. If you enroll in our Company health plan later in the plan year because of a Qualifying Life Event or an Employment Status Change, you will retroactively receive HRA or HSA contributions for Wellthy Points earned through completed wellness activities in the current plan year. The program runs Jan. 1-Dec. 31 each year. If your spouse or domestic partner is on our Company health plan, they can also create a Virgin Pulse account to sign up for health fairs and participate in special wellness activities and earn wellness points throughout the year. Spouses and partners who aren’t on our Company health plan can still create an account to track their personal wellness goals but will not be able to participate in some of the program’s features.

Access HRA/HSA To view your HRA account balance and details, log in and click Programs Page > HealthEquity, or visit HealthEquity directly. Before you access the HealthEquity site for the first time, you will need to create your account. To view your HSA balance and details, log in and click Programs Page > Fidelity, or contact a Fidelity representative at 1-800-544-3716. Before you access the Fidelity site for the first time, you will need to opt in and activate your account. MyPulse Health Survey (Online Health Assessment) Improving your health begins with an important step: completing your MyPulse Health Survey (Online Health Assessment via Virgin Pulse). This free, confidential health questionnaire is designed to help you understand your overall health, including diet and activity, with suggestions to help you improve or maintain it. The survey is available on both the Virgin Pulse website or mobile app. By taking just five minutes to answer the 43 survey questions, you can begin to have a better handle on your personal health, and you can use the assessment to start a dialogue with your healthcare providers. The survey results will also make suggestions for specific wellness program features that might help you become healthier, and it can be used to track changes from year to year.

Biometric Testing Biometric testing is a short health exam that provides individualized feedback about

your risk for Metabolic Syndrome—a group of risk factors that raises your risk for heart disease and other health problems, such as diabetes and stroke. Knowing and understanding your results is the first step to healthy living and encouraging healthy behaviors that decrease the incidence of Metabolic Syndrome. The five indicators that make up Metabolic Syndrome are blood glucose, blood pressure, HDL cholesterol, triglycerides, and waist circumference.

Biometric testing is available for all employees (whether or not they are covered on our Company health plan) as well as spouses (only if they’re covered on our Company health plan). Biometric testing is offered early each year at onsite Wellthy for Life Health Fairs. You can also complete biometric testing at any LabCorp location or at your personal physician’s office (please note that, as with any visit to your personal physician, you may be charged a copay and/or additional charges if other services are performed). All screening results are automatically uploaded to your Virgin Pulse account.

To complete biometric testing at LabCorp or at your personal physician’s office, log in to Virgin Pulse, and click on Programs, WCS Biometrics, and choose your preference. You will be able to download a lab form or physician form and instructions on how to complete it. Note: Some LabCorp locations do not collect all biometric measurements, so be sure to use the LabCorp link provided. For the most accurate screening results, we encourage you to fast (no food) for 9 hours before your appointment. Please drink plenty of water. Continue to take any prescription medications. If you are diabetic or pregnant, please consult your physician before fasting. 2019 Surcharge If you’re enrolled in our Company health plan, completing the survey (along with biometric testing) helps you avoid a $25 surcharge to your health insurance premium each pay period. If your spouse is covered on our Company health plan, they should also complete the survey and biometric testing to avoid the surcharge.* *Team members hired Jan. 1, 2019, or later will not be required to complete the survey and biometric testing to avoid the surcharge until the following plan year, but can still receive Wellthy Points for completion of each. How to Earn Wellthy Points NEW IN 2019: Earn Wellthy Points for achieving Level 3 or higher in Virgin Pulse 30 Wellthy Points per quarter You can earn 30 points per quarter when you achieve Level 3 or higher in Virgin Pulse point rewards. Points are awarded automatically within 7 business days after the close of the quarter. Tobacco Cessation 100 Wellthy Points The Quit for Life® program is a 12-month tobacco cessation program available to all employees enrolled in our Company health plan. It provides telephone-based counseling support, nicotine-replacement therapy (gum, patch, or lozenge), a handy app, web-based support tools, and educational materials. To enroll, call 1-888-768-7848 or visit www.QuitNow.net/Alabama. To complete the program and earn Wellthy Points, you will need to complete five calls with a Quit Coach.

Baby Yourself Maternity Management 100 Wellthy Points Baby Yourself Maternity Management program is available to team members and their

spouses enrolled in our Company health plan and is managed by BlueCross BlueShield of Alabama. Earn 100 Wellthy Points when you enroll by 16 weeks gestation. The goal of the program is to ensure that expectant mothers and their babies receive the best possible healthcare during pregnancy. You’ll receive telephone or email support from an experienced registered nurse throughout your pregnancy, as well as several gifts throughout your pregnancy

to inspire healthy behaviors. The Baby Yourself app (available on Apple and Android devices) provides additional information, trackers, and easy access to your nurse through one-button dialing. Once your baby arrives, the Lactation Program provides encouragement, support and information designed to improve the well-being of infants and their families. You can enroll once you learn you are pregnant by calling 1-800-222-4379. If you complete the program, you’ll earn an additional gift card. Milk Stork is a breast-milk delivery service that provides refrigerated, overnight shipping of breast milk from moms traveling on bank business back to their babies at home. Team members who travel for two or more consecutive nights are eligible to take advantage of this program. Milk Stork and hotel charges must be submitted in the same expense report in the company’s travel and expense system, Concur. Chronic Condition Management 100 Wellthy Points Medical and Mental Health Chronic Condition Management is available to team members enrolled in our Company health plan. Wellthy Points are awarded quarterly, upon completion of the program. Medical Chronic Condition Management: BlueCross Blue Shield offers a personalized approach to managing your health care needs, increasing compliance rates over time, and lowering health risks. The program is designed to help members who have asthma, chronic obstructive pulmonary disease (COPD), congestive heart failure (CHF), coronary artery

Medical Case Management: This program is designed to assist team members facing complex or acute health situations, such as medical/surgical conditions, high-risk pregnancy, organ transplant, and physical rehabilitation. Each case is assigned a licensed Registered Nurse who can assist with specialty services, coordinate care and needs, provide community resources, and monitor healthcare needs over time. Mental Health Case Management: This program is designed to assist team members facing complex or acute health situations, such as altered mental states, major depression, and alcohol or substance-abuse withdrawal. visiting your personal physician. Log into Virgin Pulse and click WCS Biometrics on the Programs page. Ideal or Improved Metabolic Categories in Biometrics and Lab Results 75 Wellthy Points (375 Max) Your biometric results are excellent indicators of your current health status. By achieving healthy ranges, you are actively protecting yourself from future disease risks, including heart disease, diabetes, and stroke. You can earn Wellthy Points by achieving healthy or improved ranges in blood glucose, blood pressure, HDL cholesterol, triglycerides, and waist circumference. These Wellthy Points will automatically be awarded and offer an incentive for reaching and maintaining healthy lifestyle habits, like eating better and exercising regularly. Annual Physical/Wellness Exam 125 Wellthy Points Complete an annual physical/wellness exam to earn Wellthy Points. Points will be automatically awarded through a confidential file if the medical claim for the screening is billed appropriately as wellness (if you are enrolled in our Company health plan). Allow up to 60 days to view these points online. If your exam is not billed appropriately by the provider (typically coded as a “physical” or “wellness” visit), the claims issue must be resolved before your Wellthy Points are awarded. Personal Health Coaching Earn up to a Max of 300 points Working with a coach can help you set goals, better understand your health, and ultimately lead to a healthier, happier life. Health coaching is a free, confidential service through Wellness Corporate Solutions (WCS). Personal health coaches are registered dietitians and health and fitness specialists with expertise in areas such as sleep and stress management. Additionally, WCS coaches have credentials such as certified diabetes educator. Health coaches can work with you to address almost any area of your overall health and wellness and will create a sense of accountability and

motivation. In addition to helping you work toward your goals, your coach can also guide you on a variety of topics like weight management, eating habits, and exercising. With the help of a personal health coach, you will have a better chance of achieving your goals and making the health changes you want. You will be able to send a

secure message to your coach via the online platform, as well as opt in for text reminders for coaching appointments. Receive 100 points for your initial call and 100 points for completing 4 calls. To schedule a coaching session, log in to Virgin Pulse,

Stay tuned throughout the year for more information. Wellness Challenges 100 Wellthy Points (Max 200 points) Watch for two Wellness Challenges to be offered by Virgin Pulse in 2019. Points will be automatically awarded upon completion of the challenge. 401(k) Contributions 100 Wellthy Points Thanks to programs like Wellthy for Life, today’s average Americans are living longer than ever before in history. A typical 65-year-old retiree may need retirement income for 30+ years, even more if you retire early. Wellthy for Life rewards you for saving for your future. You’ll receive 100 Wellthy Points if you are a new participant contributing for at least six months during 2019; an existing participant who increases your contribution by 1% for at least six months during 2019; or a participant on schedule to max out your annual contribution for 2019.

Volunteering 100 Wellthy Points Each year team members volunteer thousands of hours collectively to our communities, making a big difference where we live and work. Team members who volunteer 10 or more hours per quarter will earn a max of 100 Wellthy Points for the year.

SmartDollar is owned by well-known financial expert Dave Ramsey. Every time you use SmartDollar, you’ll earn SmartDollar points, which convert to Wellthy Points. Other Wellthy for Life Wellness Program Features ReThink ReThink is a free benefit that provides support to parents raising children with learning or behavioral challenges, or with developmental disabilities. Parents receive 14 hours of free, live teleconsultations per plan year with behavioral experts who can answer questions, provide guidance, and offer resources. These can take place over the phone or via video chat, day or night, weekday or weekend. Behavioral experts hold a master’s degree or Ph.D. and have many years of experience supporting parents. Common teleconsultation topics include:

● Teaching new skills ● Addressing problem behaviors at home ● Troubleshooting lack of progress ● Collaborating with school and other providers ● Coping with the stress of a new diagnosis or ongoing daily struggles as home

Additionally, Rethink provides parents with a variety of resources, training and support in a simple web-based format. Parents have access to printable materials such as flashcards, worksheets, schedule templates, and token boards as tools that complement the web-based training to help their children in reaching their potential. They can access over 1,500 easy-to-follow videos from an extensive library depicting behavioral experts and educators teaching children skills such as:

Life Balance by American Behavioral Employee Assistance Program Life Balance by American Behavioral is designed to meet a wide range of employees’ needs, both within and beyond the workplace. Life Balance reflects Wellthy for Life’s comprehensive wellness approach and commitment to finding solutions for the whole person in mind, body, and spirit. Life Balance is designed to complement, rather than replace, services available under other programs, including health insurance. Long-term services are not covered by Life Balance. You may elect to continue counseling sessions at your own expense if you are not on our Company health plan. Pursuing long-term services is entirely your decision. Assessment and Counseling Services We are committed to providing support and resources when you have personal difficulties. Personal or emotional problems can overwhelm almost anyone and affect job performance. At such times, Life Balance can play a significant role by providing access to confidential, short-term professional counseling, as well as resource and referral assistance. The company covers the cost of this service. 24/7 Access to Master’s Level Clinicians The program offers convenient, toll-free telephone and online access to Life Balance’s Employee Assistance Specialists, Child/Parenting Specialists, and Adult/Elder Specialists, around the clock, 365 days a year. Face-to-Face Counseling Brief goal-oriented counseling for any concern is available through a network of credentialed, licensed clinicians. The first six sessions with a professional counselor per year per employee (or covered eligible dependents) are free. During these sessions, the counselor will talk to you, decide what kind of counseling or treatment is needed, and begin work to help resolve your issue. Adult/Elder Support Services This feature includes personalized search services to provide referrals for elder care, as well as resources and materials on retirement, housing concerns, grief and loss, disaster support, Medicare/Medicaid, and respite care. Connection to Community Support This service provides referrals to thousands of community resources from the Personal Advantage website, which houses information on 12-step groups, community mental health agencies, support groups, and more. Legal Counseling Services Services include a free 30-minute consultation with an attorney on a variety of legal matters (excluding employment disputes), including documentation review (up to six pages free of charge), and complimentary simple will preparation. This is in addition to the six counseling sessions, three identity theft consultations, and the one financial consultation. Financial Counseling Services This feature includes a free 30-minute consultation with a credentialed financial professional and/or local community referrals to other financial advisement resources. Identity Theft Counseling Services This feature includes up to three 30-minute scheduled telephone consultations with an Identity Theft Counselor. This is in addition to the six counseling sessions, the one legal and one financial consultation. While Life Balance addresses everyday living challenges, the mental health benefit addresses clinical diagnoses such as anxiety, chemical dependency, depression, bipolar disorder, and other serious disorders. Behavioral healthcare is an employer-sponsored program designed to provide disorder identification, clinical treatment referrals, and crisis intervention for employees and their family members who experience mental/behavioral conditions. BlueCross BlueShield through New Directions Behavioral Health assists employees and families by identifying and working through conditions that may be a focus of clinical attention such as adjustment disorders, ADD/ADHD, anxiety disorders, impulse control disorders, mood disorders (such as depression), schizophrenia and other psychotic disorders, and substance-abuse disorders. From information on “what type of program do I need” to “crisis information” and “help with my diagnosis,” New Directions focuses on finding you the right care at the right time. Then under ManageMyHealth, expand the list by selecting See All. Now select Behavioral Health Services.
