WellStyles™ is a comprehensive lifestyle management program designed to put YOU in

charge of your health & wellbeing. Knowledge, mo va on & follow‐thru are key factors

in successfully changing or maintaining healthy habits. The WellStyles program incorpo‐

rates a variety of wellness op ons & goals giving YOU the ul mate choice as to which

healthy habits you are willing to explore. By prac cing & recording your par cipa on in

these programs, you earn points that can be redeemed for rewards.

there is no limit on the number of prizes that can be requested.

To redeem your prizes, complete the Prize Request Form (all forms can be obtained

online: www.wellstyles.org) and e‐mail or fax to your WellStyles Representa ve. Upon

confirma on of available points, in‐stock prizes will be awarded within 30 days of the

Earn 200 points for your Annual Wellness Visit!

It’s important to establish a rela onship with your primary care physician. Seeing your primary care doctor at least once a year for your annual Wellness Visit is essen al in providing you and your doctor an overall “snapshot” of your current state of health. For Sierra Vista benefited employ‐ees, your annual wellness visit is at no cost to the member and typically only takes about ten to fi een minutes to com‐plete.

What can you expect at your visit?

Pa ent History:

Your doctor will ask you some ques ons to determine how you are currently feeing and if you have any concerns. Typically he or she will also ask you ques ons that relate to your habits (diet, exercise, smoking etc.) as well as your vaccina on status.

Vital Signs:

Your doctor will check your blood pressure, heart rate, respira on & temperature.

Heart & Lung Exam:

Your doctor will listen to your heart and lungs.

Physical Examina on:

This would include checking your throat, mouth, tonsils, nose, sinuses, eyes, lymph nodes and thyroid

Neurological Exam:

This includes tes ng your nerves, strength, balance and reflexes.

Dermatological Exam:

Some doctors will check the condi on of your skin and nails.

Men:

Depending on your age, most doctors will check for hernia, tes cular cancer and prostate health during your annual exam.

Women:

If you do not see your OBGYN for your annual cervical and breast exam, your primary care doc‐tor can perform these exams at the me of your annual physical.

Lab Tests:

Doctors will some mes order a metabolic blood panel to include glucose (blood sugar) levels, cholesterol and lipids, etc. as well as a urinalysis.

All program forms and further instruc ons can be obtained online at:

www.wellstyles.org under your District Sign‐In. Catalog, prizes, forms and

points are District Specific. Make certain the catalog you are using is listed as

CUSD’s for 2013‐2014.

Once enrolled, members can provide documenta on for points earned as

you par cipate, monthly, or annually. Forms or EOBs can be faxed, mailed or

e‐mailed.

Your District’s current Fitness Sponsored program is the Work it Off program.

Exercise log forms can be obtained online. 30 minutes of con nuous exercise

10 hours per month is required for members to earn 25 points per month.

Forms will be accepted July 1, 2013 through the end of June 2014.

An annual calendar of events can be accessed online. Any changes, dele ons

or addi ons will be included on the calendar.

Members may request prize(s) whenever sufficient points are available.

Qualifying preven ve exams can be submi ed for points from July 1, 2013

thru June 30, 2014.

For UHC members comple ng the online Health Assessment at

www.myuhc.com, no comple on forms are necessary. A monthly report will

be provided to Valley Schools, and points will be automa cally added to

your total.

EOBs (Explana on of Benefits) can by obtained by logging on and registering

at www.myuhc.com.

1. WellStyles membership is offered at the sole discre on of WellStyles VSMG © 2013‐

2014. WellStyles retains the right to change, limit, modify or cancel program, regula‐

ons and rewards at any me, with or without no ce.

2. Points will expire upon employment termina on as well as at the end of each plan

year. Point balances must be redeemed within 30 days at the end of each plan year.

Membership is automa cally renewed each year unless member no fies WellStyles in

wri ng of their inten on to quit the program.

3. Despite efforts to ensure accuracy in point calcula on, tracking and prin ng errors

occasionally occur. WellStyles reserves the right to correct mistakes at any me.

4. WellStyles prizes will be mailed via USPS to the address indicated on the membership

form. WellStyles is not responsible for lost, stolen, damaged or misdirected mail and/

or prizes.

5. Points have no cash value and cannot be exchanged for cash or credit.

6. WellStyles rewards & prizes may be subject to tax liability. Any tax liability, including

disclosure connected with the receipt of prizes is the sole responsibility of the mem‐

ber.

7. In the case of fraud or abuse involving the WellStyles Lifestyle Management System,

WellStyles has the right to take appropriate administra ve and/or legal ac on, and all

points earned through the program may be forfeited.

8. Members may be provided with usernames or passwords to access restricted por ons

of the WellStyles site. The content contained in such areas is confiden al and is provid‐

ed to members for personal use only.

9. Points earned by a member are for the member's benefit only and may not be sold,

traded or transferred. Viola on of this rule will result in the termina on of the mem‐

bership and forfeiture of all points.

10. If a member changes his or her address, no fica on must be provided to

[email protected] before prizes are requested.

11. WellStyles may subs tute prizes or rewards of equal value at any me prior to or a er an

order has been placed. Should a specific reward or prize be discon nued, it will be replaced

with a reward of equal or greater value or, if no suitable subs tute is available, WellStyles

may require the member to make an alternate selec on.

12. WELLSTYLES MAKES NO PRODUCT REPRESENTATIONS OR WARRANTIES, EXPRESS OR IM‐

PLIED, AND DISCLAIMS ANY AND ALL LIABILITY AS TO THE CONDITION, QUALITY, MERCHANT‐

Schools personnel & your districts' management staff. All reasonable ac ons are taken to

secure both online & hard copy documenta on on our members. Your informa on will not

requirements such as a law, regula on, search warrant, subpoena or court order; or (2) in

special cases, such as a physical threat to you or others. In the event that we are legally
