relation to grade-level expectations at a point in time rather than an average over time

FYI Flu Vaccine
• Flumist (intranasal live influenza vaccine) will be the only vaccine provided
• Vaccines will only be provided to students with parental consent • Parental consent forms will be sent home later this week.• Consents must be returned by Friday, September 27th
• Parents will be notified if student is not eligible for FluMist• Any child under the age of 9 that did not receive any doses of the flu
vaccine last year will require a 2nd dose of the flu vaccine in 4 weeks. 2nd dose will be administered at school.
• Vaccine is being provided by the Vaccines For Children (VFC) program. VFC requires consent forms ask if the child has health insurance but insurance is not billed.
• Vaccination will be completely free of charge.
