The Lincolnway Special Recreation Association’s mission is to provide recreation and leisure services for individuals with physical or intellectual disabilities while promoting greater disability awareness in the community.

LWSRA provides recreation, leisure and sports opportunities for individuals with disabilities through a special cooperative agreement that includes Frankfort, Frankfort Square, Manhattan, Mokena, New Lenox, Peotone and Wilmington Island Park Districts as well as special agreements with the Village of Monee and Homer Township. LWSRA offers programs for more than 360 individuals throughout the year from preschool through adult. Contact one of our trained specialists today to learn more!

Registration for in-district residents begins April 30, 2018. Out-of-district and SSSRA Residents begins May 7, 2018.

REGISTRATION INFORMATION: Registration and payment is accepted either by mail, online, or brought to the LWSRC. *Registrations are NOT accepted over the phone or at program sites. Program registration must be received prior to the deadline. Forms of Payment: Checks (made out to LWSRA), cash or credit cards are accepted (Visa/Discover/MasterCard). Confirmation of program registration will not be sent, but you will be notified if a program is cancelled or if placed on a waiting list. Fees will not be prorated for late registration or missed classes. Consideration will be made only if the participant is a new resident. Delinquent accounts: Participation will be denied if a balance remains from a previous season or a late pick-up fee has not been paid.

ANNuAL INFORMATION FORMLWSRA requires all participants to complete an information form annually. This form provides staff with valuable information, which allows LWSRA to conduct its activities in the safest possible manner and to meet the basic needs of the participants. All information is kept strictly confidential and enhances successful participation. Thank you for your cooperation. The form is found in this brochure and online. This form must be updated at the start of the calendar year, or first registration of the calendar year, and whenever information changes.

APPLICATION FOR PARTICIPATION IN SPECIAL OLYMPICS (MED APP)In order to compete in Special Olympic competition, and/or to participate in selected programs, a current Special Olympic medical form must be on file at the LWSRA office and with S.O.I. This form is valid for two years.

ATLANTOAXIAL INSTABILITY RELEASE Individuals with Down syndrome are at risk of having a condition known as atlantoaxial instability (AAI). This condition allows for increased laxity of ligaments involving the top bones of the neck. This results in the abnormally increased movement between the first and second vertebrae and may lead to pressure on the spinal cord which can lead to injury. Certain programs require this release.

INCLuSIONLWSRA understands that not every person with a disability needs to participate in special recreation programs. If you feel that a program offered by your park district (Frankfort, Manhattan, Mokena, New Lenox, Peotone or Wilmington Island) would better meet the needs of your child, follow these steps for inclusion:

Register at your local park district for the desired program as soon as 1. possible. Let them know you will be requesting inclusion assistance. Immediately contact Melissa at LWSRA at (815) 320-3500 ext. *205.

Through meeting(s), observations, etc., it will be determined if inclusion in the 2. program would be appropriate and beneficial, and if an inclusion aide is needed.

For any inclusion needs that cannot be met, the park district will issue a refund.3.

Please understand that we will do our very best to provide an inclusion aide. However, because of staffing restrictions, we are unable to guarantee an aide. Please register early. Last minute requests will not be accepted.

ADALincolnway Special Recreation Association advocates for full participation under the Americans with Disabilities Act (ADA). Every attempt at reasonable accommodation will be made so that individuals may participate.

REFuND POLICYRefunds after a program begins will be allowed for the following situations only: 1) Cancellation of a program or event by LWSRA; 2) Medical reasons upon presentation of a doctor’s request; 3) The participant moves from the area. All refunds will be pro-rated to the time of departure. A $10.00 service charge will be assessed on all refunds unless refund is due to cancellation by LWSRA. There will be no refunds if a participant cancels out of a program, trip, or special event and LWSRA has incurred costs due to purchase of tickets, rentals, deposits, staffing, etc. Accounts with an existing balance will have the refund automatically applied to their existing balance. For NSF checks, a $20.00 fee will be charged. Refunds will not be given due to weather related cancellations.

RESIDENT/NON-RESIDENT FEESResident Fee: Residents are those living in the boundaries of Frankfort, Frankfort Square, Manhattan, Mokena, New Lenox, Peotone and Wilmington Island Park Districts. *Village of Monee: Households that are within Village of Monee boundaries will be allowed to register at in-district rates.*Homer Township: Homer Township offers assistance to its residents with out-of-district fees. Please contact the LWSRA office to learn more.Non-Resident: Fees are 25% to 50% greater than resident rates. The percentage is determined on a program by program basis by the LWSRA Executive Director.*Non-Resident Exceptions: Participants who are taxpayers of the Lincoln-Way High School District 210 shall be able to attend LWSRA programs held at Lincoln-Way High School at an in-district rate. *Verification required prior to registration. Not eligible for scholarships, door-to-door transportation or SSSRA cooperative agreement.

LWSRA/SSSRA AGREEMENTLWSRA has a cooperative agreement with the South Suburban Special Recreation Association (SSSRA). Each association offers programs to the other association’s residents at in-district rates with the following exceptions: 1) Special Services such as scholarships and door to door transportation are not included. SSSRA’s “First Time Participant Discount” does not apply to LWSRA residents. 2) Participants who wish to register for day camps, vacation trips, away camps and weekend trips may do so at non-resident rates. 3) Early bird discounts or any other discounts do not apply under this agreement. 4) When programs are offered free to either association’s own residents because of grant funds or sponsorships, the program or event will be offered to residents of the other association at the regular fee for residents.

All other agency policies and guidelines apply to residents who register for programs under this Agreement.

Persons residing in these SSSRA member districts are considered residents: Country Club Hills Park District, Hazel Crest Park District, Homewood-Flossmoor Park District, Lan-Oak Park District, Matteson Parks & Recreation Department, Oak Forest Park District, Olympia Fields Park District, Park Forest Recreation & Parks Department, Richton Park Parks & Recreation Department and Tinley Park - Park District.

PAYMENT POLICY FOR REGISTRATION FEESAll fees must be paid in full upon registration for LWSRA programs, events and tournaments. If full payment is not possible then a deposit of at least 25% must be made at time of registration and a payment schedule will be generated at LWSRA’s discretion. Payments must be made by the designated date each month. If a payment is not made on time, a late fee of $15.00 will be assessed and the account considered delinquent. New registrations will not be accepted until ALL HOUSEHOLD outstanding bills are paid. LWSRA reserves the right to deny participation to any individual who becomes delinquent until paid in full.

SCHOLARSHIPSThe scholarship program is funded by donations only. Therefore, availability of scholarships may vary. Scholarships are available for in-district residents who qualify due to financial hardship situations. An individual may qualify for up to 1/2 of program fees for two programs a season. (Maximum of $400 a year.) The remainder due will follow the payment policy stated above. Please call (815) 320-3500 to request a scholarship application or find it online at lwsra.org/forms.

DESIGNATED SCHOLARSHIPS/GIFT CERTIFICATESDesignated scholarships allow both residents and non-residents to hold money in a designated fund to assist with payment of future program fees. Gift certificates are available and make a great gift idea for family and friends to purchase. Individuals with money in their designated fund will be notified prior to each registration season. IMPORTANT: To use your funds, you must either submit the gift certificate or contact the LWSRA office and request the funds be applied to your current balance. LWSRA will not automatically apply any of these funds.

DROP-OFF POLICY If a participant is brought to a program/drop-off site earlier than five (5) minutes before the designated time, please understand that supervision will not be provided and you must stay with the participant until the staff is ready to accept that person. In the event an individual is present more than five (5) minutes before the scheduled drop-off time (ex: participant uses public transportation), LWSRA staff are not responsible for the participant until the scheduled drop-off time. For safety reasons, parents or caregivers MuST check in with the program leader when dropping off or picking up a participant.

LATE PICK-uP POLICY Each time a participant is picked up late, a $10.00 fee will be charged for every 15 minutes or portion thereof. Payment is due to LWSRA within one week of notification. Registration for other programs will not be accepted until all outstanding fees are paid.

GuIDELINES WHEN ILLNESS OCCuRSIn consideration of other participants and staff, and to prevent the spread of contagious illnesses, LWSRA strongly urges participants to refrain from attending programs when any of the following conditions exist:

Fever of 100 degrees or higher• Vomiting within the last 24 hours• Diarrhea within the last 24 hours• Persistent cough and/or cold symptoms• “Pink eye” (conjunctivitis) or discharge from the eye• Symptoms of mumps, measles, chicken pox, strep throat, flu, impetigo, or • coxsackie virusRunny nose with yellow or green discharge• Lice or mites•

Notify the LWSRA office of any contagious disease that will affect attendance. Participants can return at doctor’s recommendation, or when symptoms have clearly passed.

PLEASE NOTE: LWSRA staff has the right to deny participation to any individual exhibiting symptoms of a contagious illness.

BEHAVIOR EXPECTATIONSLWSRA encourages and promotes the concept of fun for everyone. However, LWSRA participants are expected to exhibit appropriate behavior at all times. The LWSRA basic behavior code insists that participants shall:

1. Show respect to each other and staff and take direction from staff.2. Refrain from using abusive or foul language.3. Refrain from causing bodily harm to self or others.4. Treat equipment and facilities with care and careful consideration.

Additional rules may be developed for specific programs as deemed necessary. LWSRA applies a caring, positive approach to discipline. Staff are willing to work with parents to develop behavior modification programs if necessary. LWSRA reserves the right to dismiss a participant whose behavior continues to be inappropriate or whose behavior endangers the safety of him/herself or others.

Transportation of ChildrenThe Child Passenger Protection Act of the Illinois Vehicle Code requires that children under the age of 8 be transported in an approved Child Safety Seat in certain vehicles. Parent/Guardian will be notified if there is a need to provide a Child Safety Seat for your child. Children under 8 years of age will not be transported if the parent/guardian does not provide a Child Safety Seat. For further information, please contact Abby Billips 815-320-3509.

MEDICATION DISPENSINGIMPORTANT! If you are requesting that medication be dispensed to your participant during an LWSRA program, these procedures MUST be followed:

Each form of medication must be in the original container or in a 1. clearly marked envelope provided by LWSRA. A. The envelopes can be picked up at time of registration or anytime up to one week before the program begins.* B. For weekend, multi-day events and S.O.I. tournaments, medications MUST be brought to the LWSRA office, no later than one day before the program/event. 2. The container must include doctor’s name, patient’s name, pharmacy, medication, strength, dosage, date and on envelope, initials of parent, guardian or care giver.3. No more than the proper dosage should be in each container/envelope for the length of time medication will be given. A. If medication is given more than once per day additional envelopes (unless using the original container) are needed for each subsequent dose. B. Other forms of medication such as liquids, creams, etc. must be in the original container and include doctor’s name, patient name, pharmacy, medication, strength, dosage and date.4. The Permission to Dispense Medication Form must be completed and signed prior to the distribution of any medication. If any medication information changes, notify LWSRA and complete a new Permission to Dispense form.*Please be sure to pick up enough envelopes for the duration of the event

CANCELLATION DuE TO WEATHERCancellation of a program, event or transportation is possible due to adverse weather conditions, especially if it is an outdoor activity or the roads are considered unsafe for driving. The program may or may not be rescheduled. The following guidelines will be used to determine cancellations (Special Olympics programs will follow Special Olympics Illinois weather guidelines):

SEVERE COLD WEATHER CONDITIONS: Outdoor programs: A temperature of 10 degrees or lower or wind chill of 0 *

degrees or less. A temperature of -5 degrees or wind chill of -15 degrees or less. * All programs: Snowstorm or blizzard warnings or conditions, when driving *

restrictions and emergency accident plans are in effect.Programs with transportation: In some situations a program will still be *

offered without transportation.

SEVERE HOT WEATHER CONDITIONS: Outdoor programs: Lightning conditions and 30 minutes after lightning has *

ceased. Outdoor programs/indoor sites without air conditioning: Temperature of 95 *

degrees or higher or a heat index of 105 degrees or higher. Programs with transportation: Heat index of 115 degrees or higher. * All programs: Weather situations when a Tornado Warning is in effect.*

LWSRA will use discretion when determining program cancellations due to other weather conditions, watches and warnings. Refunds will not be given due to weather related cancellations.

PERSONAL ITEMSLWSRA is not responsible for lost, damaged or stolen items at LWSRA programs. Lost and found is located at the LWSRC. Items will be held for three months.

MANDATED REPORTERAll LWSRA staff are mandated by the State of Illinois to make a report to DCFS hotline if they observe a situation that provides reasonable cause to suspect child abuse or neglect. Workers in certain professions, including recreational program personnel, have this legal mandate. Our staff is trained on signs and symptoms of abuse and neglect and procedures for making a report.

within this brochure. However, errors can occur and circumstances may require adjustments to fees, schedules, locations, or other aspects. LWSRA

reserves the right to make such adjustments and apologizes for any inconvenience these errors or adjustments may cause.

YOuNG ATHLETES Ages: 3-7 Through Young Athletes, staff will introduce young children to the world of sport, with the goal of preparing them for Special Olympics sports training and competition when they get older. We’ll focus on the basics that are crucial to cognitive development: physical activities that develop motor skills and hand-eye coordination and the application of these physical skills through sports. NOTE: This program runs in conjunction with summer camp. If you have registered for summer camp afternoon sessions all six weeks, you do not need to register for Young Athletes.

INDIVIDuAL MuSIC LESSONS Ages: 3 & up Music education is a gift that enables students to create, communicate, and express emotions in a positive and encouraging manner. Nadine Kahl has over 10 years of experience playing piano and 3 years teaching youth. Nadine is excited to bring individual music lessons to LWSRA. All lessons are 30 minute private sessions offered on a monthly rolling enrollment. Once registered please contact Abby Billips to select lesson time and set-up initial meeting with Nadine. TITANS is a structured, 5 day a week program that runs 49 weeks out of the year and is designed to meet the needs of persons 18 years and older with a disability.

The TITANS program incorporates recreation and basic life skills to enhance social skills, improve independence and help maintain a leisure and recreational lifestyle for the individuals who participate in the program.

The adapted sports program started at LWSRA in 2007 in an effort to provide recreational opportunities for people with physical disabilities. Participants come from our member park districts as well as many communties beyond our districts. Since 2009, the program has grown immensely including players receiving Team USA tryouts and college scholarships to play wheelchair basketball.

WHEEELCHAIR BASKETBALL Wheelchair basketball is a Paralympic sport designed for individuals with a primary physical disability affecting his/her lower body. Athletes with a spinal cord injury, cerebral palsy, spina bifida, amputation or other diagnosis that affects one’s ability to play competitive stand-up basketball may qualify. To play wheelchair basketball you do NOT have to be an everyday wheelchair user. Sports wheelchairs are provided to allow everyone to compete on the same playing field. New athletes and seasoned athletes are welcomed to play. Season runs October - April.

LWSRA offers 5 wheelchair basketball teams • Minis - Ages 3-9 (using a 6’ rim for beginner athletes) • Prep - Ages 8-13 (uses 8.5’ rim) • Varsity* - High School level using 10’ rim *(Advanced Prep players may qualify)

WHEELCHAIR SOFTBALL Wheelchair softball is designed for individuals with a primary physical disability affecting his/her lower body. Athletes with a spinal cord injury, cerebral palsy, spina bifida, amputation, limb length difference, ankle fusion, or other diagnosis that affects one’s ability to play competitive stand-up baseball/softball may qualify. To play wheelchair softball you do NOT have to be an everyday wheelchair user. Sports wheelchairs are provided to allow everyone to compete on the same playing field. LWSRA has its own accessible wheelchair softball field consisting of painted asphalt. Program is offered May through August.

BOCCIA Boccia is a Paralympic Sport that is a precision ball sport like bocce. The sport is open to individuals with cerebral palsy, traumatic head Injury, stroke, spinal cord and muscular dystrophy. Adaptive equipment is available for those who need assistance in propelling the ball onto the court. Program is offered year round.

VI/PI BOWLING This program is offered for individuals with physical disabilities, blind and visual impairments. It’s a chance to improve bowling skills in a social setting. Program is offered year round.

WHEELCHAIR FOOTBALL For individuals who use a manual wheelchair. Game is played 6 on 6 and two hand touch above the waist. Program runs October to November.

WHEELCHAIR LACROSSE LWSRA is teaming up with the VPVA to offer Wheelchair Lacrosse. If interested, please contact Keith Wallace at [email protected]. Program is offered April to August.

SNOW SKIING Adaptive skiing uses specialized equipment for people with physical disabilities or visual impairments to experience skiing. Riders will have the option to try stand-up, sit-down, and ski bikes. Program is offered January to February.

WATER SKIING Adaptive water-ski equipment offers an opportunity for individuals with mobility, visual, or other impairments to enjoy water sports. Program is offered June to July.

Door-to-Door Transportation ProceduresLWSRA reserves the right to limit transportation. 1. Transportation is limited to the programs indicated with the symbol.2. If requesting door-to-door transportation, the application form must be submitted at least two weeks before the program starts. 3. Door-to-door transportation is limited to residents of our seven member districts and is only available to those with NO transportation alternatives.4. If door-to-door transportation is approved, 5. a fee of $8.00 per one-way trip will be assessed.

We will continue to provide limited door-to-door service for select programs, however we strongly urge you to try local transportation resources first. Some Dial-a-ride services do transport outside of their boundaries in some instances and some areas have more than one transportation service.

The following procedures will be followed for the optimal safety of everyone riding in LWSRA vehicles and vehicles leased for LWSRA programs and events.

All passengers must be seated with seat belts securely fastened. Those in wheelchairs must wear shoulder and lap belts. The wheelchair must be secured using 1. the four-point restraint system. The driver will not proceed until all passengers are properly secured.All wheelchairs must have brakes that are in good working order and can stop the chair from moving.2. LWSRA staff are responsible for determining whether or not a participant can be transported safely. If it is determined that it would be dangerous to transport an 3. individual, the driver may refuse to service that individual.Persons riding in Amigo-type wheelchairs must transfer into a van seat and must use a seat belt. 4. No object or part of the body is to protrude from the vehicle, nor should any object be thrown from the vehicle at any time. 5. Upon request, LWSRA staff will assist participants to and from their homes, but will not enter homes. Walkways must be clear of debris, snow and ice. The LWSRA bus 6. will not pull in driveways.LWSRA staff will not leave any person unattended after any program unless verbal permission has been received by said individual’s parent/guardian.7. LWSRA reserves the right to refuse or discontinue transportation services to any individual at any time. 8. The Child Passenger Protection Act of the Illinois Vehicle Code requires that children under the age of 8 be transported in an approved Child Safety Seat in certain 9. vehicles. Parent/Guardian will be notified if there is a need to provide a Child Safety Seat for your child.
