Required Documents – IRS LetterMost Recently Received Letter of Affirmation or Letter yof Determination MUST HAVE:

• Language granting your organization tax-exempt status under 26 U.S.C. 501(c)3

• Your EIN Number• Current physical or mailing address for your organization• To request a new letter from the IRS: 1-877-829-5500

Chapters/Affiliates or Group Exemptions Require Additional Docs

Required Documents – Form 990

Most Recent IRS Form 990 for a fiscal year period ending June 30, 2011 or later MUST:

• Be complete and the same as filed with the IRS• Be signed and dated by an officer of the organization• Include all applicable statements and schedules (all that you

checked off on pages 3 and 4)checked off on pages 3 and 4)

If you file a 990EZ or 990N e-postcard, you must complete a pro forma 990.

Required Documents – Pro Forma 990

The Pro Forma IRS Form 990 MUST:• Be for a complete fiscal period ending June 30 2011 or later• Be for a complete fiscal period ending June 30, 2011 or later• Include the following sections:

• Page 1: Items A-M; Part I lines 1-4 only; Part II (signature block)• Page 7-8: Part VII, Section A (compensation of officers, directors,

trustees, key employees, highest compensated employees and independent contractors)

• Page 9: Part VII, Statement of Revenueg ,• Page 10: Part IX: Statement of Functional Expenses• Page 12: Part XI: Financial Statements and Reporting

• You must also submit a copy of the 990EZ or 990N that you filed with the IRS

Required Documents - Audit

If your IRS Form 990 or Pro Forma 990 shows revenue of $100,000 or more:

• You must submit audited financial statements for the same period covered on the IRS Form 990

• A review is not acceptable• Must include the letter from the CPA conducting the audit, on

the CPA firms letterhead, with a signature.• Must be in compliance with Generally Accepted Accounting

Procedures (GAAP)

Required Documents: Names and DBAs

If your organization goes by a name OTHER THAN the Legal Name on the IRS Letter of Determination

• Provide the certificate showing the ‘Doing Business As (DBA) or Trade Name from the state of incorporation

• Maryland – Department of Assessments and Taxation• Virginia – State Corporation Commission• District of Columbia – Department of Consumer and

Regulatory Affairs

Common Mistakes Handout Provided:

• Name and Address match IRS Letter• Program descriptions are for 2012 and are brief and specific

about services provided.• 990 is SIGNED!• Number of board members on page 1 matches number listed

on page 7-8. • EZ or N Filers: All required sections of the pro forma are

complete!
