GENERAL INFORMATION
If there is anything you think the club should be aware of please indicate in the space allowed. You should include in this space if the player has a preference for a particular team or wishes to play with a particular player.
Please note all endeavors will be made to meet such requests but this cannot be guaranteed.

DECLARATION
I hereby give my consent for the above-mentioned player to train for and play Australian Cricket with the Officer Cricket Club. I also agree to abide by the Clubs Code of Conduct and other policies/codes as applicable.
I understand and on behalf of the registered player, accept the risks associated with playing and training for Australian Cricket. I agree to indemnify the club and/or its servants, officer, players and members should any claim/s be lodged or issued against the club and/r its servants, officers, players or members now or in the future. By participating in a game or training session I agree, on behalf of the player, to be deemed to have accepted voluntarily all and any risks associated with playing sport on the playing surface and its adjacent areas and its associated equipment. By signing this document, I agree on behalf of the player to the terms and conditions outlined above.
I also consent to the provision of medical treatment to the registered player and the transfer of the registered player to a medical facility where deemed necessary by a sports trainer and/or club official of the club, if all reasonable efforts to contact me or other contact person/s as nominated on the registration forma have been unsuccessful.
PRIVACY
The Officer Cricket Club will collect, store and use this personal information and, at all times, observe the confidential nature of the information. In an emergency where, after our best endeavours to obtain your consent or the consent of the emergency contact person (as stated in this document) or where it is not possible or reasonably practical that such consent be obtained we may disclose this information without consent so we may provide medical assistance to the player should an injury be sustained and medical assistance is needed

Code of Conduct
I confirm that I have read the Officer Junior Cricket Club Code of Conduct and will ensure that any participants that I have given permission to play under this registration, myself and any of my associates will abide by such code at all times. I also understand that a breach of the code of conduct the matter is referred to the Senior Committee for determination. The code of conduct can be found on our website: officer.vic.cricket.com.au and is also attached to this form.

W.G.C.A.PARENT CONSENT FORM
BL6.7. A junior aged player must have written parental permission to play in any junior grade, orin any senior grade, higher than they are eligible for under their registration. This permission mustbe in the hands of the Record Secretary PRIOR to the commencement of play. Failure to complywill result in the loss of match points for senior grades. PENALTY: $10 fine + LOSS of all matchpoints and 1st innings match points to be awarded to the opposing side. (see also BL 1.5)

Officer Junior Cricket Club abides by the behaviors that are expected of every person involved in community sport and active recreation as se out by the Victorian Code of Conduct for Community Sport. Implementation of the Code will ensure that community sport remains safe and inclusive for all.
The Code States:
Every person, spectator, player, club member, official, participant, administrator, coach, parent or member of the community involved with the sport, should work to ensure:
Inclusion of every player regardless of their age, gender or sexual orientation Inclusion of every person regardless of their race, culture or religion Opportunities for people of all abilities to participate in the sport and develop to their full
potential Respect is shown towards others, the club and the broader community A safe and inclusive environment for all Elimination of violent and abusive behavior Protection from sexual harassment or intimidation
Further, Officer Junior Cricket Club expects that every participant, official, coach, parent or member of the community should abide by the following codes at all times:
Abide by the rules of the game and rules set down by your coach, club and league and other governing bodies
Never argue with an official or umpire. If you disagree, have your captain, coach or manager approach the official during a break or after the competition. If you disagree with an official or umpire raise the issue through the appropriate channels rather than questioning the officials judgment and honesty in public
Control your temper. Verbal or physical abuse of officials, umpires, spectators or other players, deliberately distracting or provoking an opponent is not acceptable or permitted
Work equally hard for yourself and for the club Be a good sport. Applaud all good players whether they are on our team, opponent or the other
team Treat all participants as you would like to be treated. Do not interfere with, bully or take unfair
advantage of other players or officials Co-operate with your coach, club members and team mates and respect the ability of your
opponents Avoid use of derogatory language based on gender, race or religion Smoking and/or consuming of alcohol is totally forbidden by Juniors whilst involved in any

Recognise the value and importance of volunteer coaches and/or officials. They give their time and resources to provide recreational activities for your child and deserve your support
Support your club officials in maintaining the highest standard of behavior both on and off the field for the betterment of the league and your family. Offer your assistance to the team that your child is playing in so that every opportunity is being provided for the very best supervision and support. Your involvement will give both yourself and your child far more satisfaction
Ensure that equipment and facilities of the club, club members and other league participants is treated with respect and be aware that any damage may incur a cost to be paid by the person/people inflicting the damage
Ensure that a sun smart policy is adhered to at all times, ensuring the adequate drinks, hats and sun screen is brought to all games and/or training
Ensure that all governing rules set out under Officer Cricket Club’s Liquor License and local by-laws in relation to the consumption of alcohol at Junior sporting events are followed at all times
