This Program is effective for the period Janu-ary 1, 200 9 through December 31, 200 9. The program is for existing electrically-heated 

homes (not new homes or additions) located in Blachly-Lane's service area.

Instructions: To receive your weatherization incentive from Blachly-Lane, please pro-vide the following information and include a copy of your invoice. A rebate will be paidor credit will be applied to your electric account. The maximum insulation plus windowrebate is $350 per customer per year. This offer is valid only for purchases and

installations made during the period of January 1, 2009 - December 31, 2009.

(U= .30) rated brand. Rebate is $3 per square foot of window area*.
