Support to Take Your Health to a New LevelThe Bellin Health Care Team is a resource offered to employees to support healthy lifestyles. It provides you with convenient, cost-effective access to a full suite of quality health and wellness services. Staffed and managed by Bellin Health, bringing Sandy Treichel RN, CHWC, Sharon Maon RN, CHWC, Jeremy Nelson PTA, CEES and Meghan Hawley OT onsite to compliment the care you receive from your physician regardless of health system affiliation. Your Care Team is backed by a team of qualified, caring professionals who bring a strong preventive care philosophy, proven expertise, experience and clinical excellence to Bellin Health employees.
Health & Wellbeing ServicesFree, confidential appointments scheduled for:• Health coaching for lifestyle related issues (i.e. nutrition, exercise, life balance)• Care coordination• Acute care• Chronic care management (i.e. diabetes, cholesterol, blood pressure)• Information on age-appropriate screenings and tests• RAS owners• System navigation/referrals• Provide wellness classes (i.e. Walk Wisconsin, Healthy U, Get Ready for Your HRA)
Physical Therapy• Ergonomic consultations/evaluations• Early Intervention for discomfort• Muscle or joint discomfort from work or nonwork injuries and illnesses• Questions regarding worksite ergonomics or equipment recommendations• Exercise and stretching advice• Advice regarding physical fitness level prior to pursuing a new work or

Privacy and Confidentiality are Number OneYour Care Team is committed to your privacy, and pledges all interactions, services, and medical records at the Onsite Clinic will be held in strict confidence. Your records will be maintained by Bellin Health and in accordance with their stringent security standards, as well as the Health Insurance Portability and Accountability Act (HIPAA).
