We are delighted that you are here to celebrate with us! Can you believe it has been 20 years already? Once Upon a Time, this organization existed only as a thought in the mind of some insightful, driven and caring individuals. In 1998, these volunteer doctors, nurses and lay people began providing free health care to those in the community who needed it most. From the Falmouth Free Clinic to Community Health Center of Cape Cod, we’ve come a long way over these two decades.

We’ve written many chapters, and are so grateful to all of the individuals and organizations who have gotten us to where we are today. Our donors, staff, patients, volunteers, Board Members, founders and friends are the fabric of this Health Center, and have helped us grow to one of the largest and most respected non-profit organizations on the Cape. For that, we thank you and we so appreciate you being here to celebrate with us tonight.

The story continues. Even though we are thrilled to celebrate such an amazing milestone, we have no intent on slowing down or letting up. We will continue to expand services and locations, while taking a careful and strategic look at how to best care for our community. All of our efforts are aimed at best serving our patients, and your generous participation in a night like tonight makes that possible! Please enjoy your evening, and know how much we appreciate your support. Enjoy the food, the music, the view, the venue and the company. 1. In 1997, Community Health Center of Cape Cod is founded by volunteer doctors, volunteer nurses and lay people as the Falmouth Free Clinic.

2. In 1998, we began seeing patients in basement of Falmouth Hospital once a week, completing 600 patient visits the first year.

3. In 1999, volunteers started working out of Town Hall Square in Falmouth.

4. In 2004, the Health Center opened a location in Mashpee Commons, and the number of patients grew to over 3,000.

5. In 2005, CHC launched their Veterans Partnership and started implementing Electronic Health Records.

6. In 2006, CHC opened a satellite location in Bourne to expand its patient reach.

7. In 2006, the Health Center received designation as a Federally Qualified Health Center (FQHC).

8. In 2007, the Town of Mashpee gave 4.2 acres to the Health Center for their new building.

9. In 2008, a 10,000 square foot facility in Mashpee facility opens, and the Health Center surpasses 5,000 patients.

10. In 2008, CHC begins offering dental services.

11. In 2009, CHC opens a Falmouth office at 210 Jones Road in Homeport for greater capacity.

20 Health Center Milestones over 20 Years! 12. In 2010, the Health Center starts an Office-Based Opioid Treatment (OBOT) program, offering Suboxone and Vivitrol.

13. In 2011, Health Center enrollment staff assists more than 3,000 patients and members of the community enroll for health insurance annually.

14. In 2012, a 22,000 square-foot extension in Mashpee opens, with expanded offerings of pharmacy, walk-in, and imaging. 15. In 2013 (and then again in 2016), the Health Center is awarded Patient Centered Medical Home Recognition at Level Three from the National Committee for Quality Assurance – the highest level possible.

16. In 2015, optometry and nutrition education services are added to the Health Center’s continuum of care. 17. In 2016, the Health Center begins offering telehealth services, including remote patient monitoring, video interpreting services and telepsychiatry.

18. In 2016, Health Center care teams complete more than 75,000 patient visits.

19. In 2017, 1 in 5 Mashpee residents and 1 in 6 Falmouth residents are Health Center patients.

20. In 2017, CHC adds dental and pharmacy services to the Bourne office, and expands addictions services.
