I hereby confirm that I have been issued with the “Pre-Travel Advice” from the College, outlining all medical, safety, insurance, registration and other requirements in order to participate in the Program. I agree that, as a participant in the Program, I have already completed or will duly complete in accordance with the Pre-Travel Advice, the following steps:

(1) Attendance at compulsory pre-departure information session(s) to obtain information about the Program and to make academic, international travel and logistical preparations for my participation in the Program;

(2) Provision to the College of Arts, Law and Education of my detailed emergency contact details and travel itinerary;

(3) Advice to the University’s insurance office of any pre-existing medical conditions (if applicable);

(4) Completion of “My Trips” online registration with International SOS and downloading/installing of International SOS travel card or smart phone application;

(5) Registration with SmartTraveller and checking of Department of Foreign Affairs and Trade (DFAT) website’s warning levels;

(6) Completion of such other steps as reasonably required by the University.
