To whom it may concern: This constitutes the annual self-monitoring report for the above-referenced facility. This report is for calendar year 2017. ANNUAL COMPLIANCE SUMMARY TABLE OF TREATMENT PLANT PERFORMANCE
This is not required as Lehigh has submitted the year’s monitoring results to CIWQS. COMPREHENSIVE DISCUSSION OF TREATMENT PLANT PERFORMANCE AND COMPLIANCE The West Final Treatment System (West -FTS) was in place and functional as of October 1, 2017. To augment the treatment capacity, Lehigh is constructing the East- FTS, which is targeted to be completed by the end of April 2018. Lehigh began discharging from the West -FTS on December 6, 2017. A total of 8.3 million galls (MG) was discharged during December. Results from the five sampling results collected throughout December demonstrated good performance where all permit limits where met except for one result for selenium, which was above the daily permit limit. The cause of the elevated selenium was noted and corrected on the same day. No other detections above the permit limit have been noted. Below is a summary of select metals results of the FTS effluent (EFF-001).

Prior to October 1, 2017, the interim treatment system (ITS) was in operation and performed well by consistently achieving over 50% removal for metals. There was a two week period where faulty nutrient-feed to the bioreactor led to less than optimal ITS performance. As noted in the November 10, 2018 report, Lehigh completed a root cause investigation and taken measures to prevent from happening again. TABULAR AND GRAPHICAL SUMMARIES OF MONITORING DATA

Lehigh does not have a laboratory that is certified for any analyses. Below is a list of analyses that are conducted by outside, certified laboratories.

Lehigh has no “waived” analyses.

EFFLUENT CHARACTERIZATION The annual effluent characterization sampling results were collected in April and reported with the April SMR. The sample was analyzed for the additional priority pollutants not already sampled as part of the routine monitoring at Discharge Point 001. Antimony was detected at 6.3 ug/L, which is above the water quality criteria as listed in the Permit Fact Sheet Table F-6. 4,4-DDE (0.0028 ug/L), 4,4-DDT (0.011 ug/L), and Dieldrin (0.0047 ug/L) were also detected above listed water quality criteria. The sampling will be repeated during the first quarter of 2018. AMBIENT BACKGROUND SAMPLING Ambient background sampling at RSW-001A was conducted in January 2017 (wet season) and May 2017 (dry season) for the priority pollutants listed in Attachment G, Table C, plus pH, salinity, hardness, temperature, turbidity, and total dissolved solids (TDS). The results were reported with the associated monthly SMRs. No organic compounds were detected in either sample. For the two samples, pH ranged from 7.76 to 8.15, salinity from 13 mg/L to 16 mg/L, hardness from 250 to 320 mg/L, TDS from 280 to 300 mg/L. Several metals were also detected including selenium at an estimated value of 0.585 J ug/L and hexavalent chromium up to 0.65 ug/L. RESULTS OF ANNUAL FACILITY INSPECTION
No major changes were made to the stormwater pollution and prevention plan (SWPPP) as a result of the facility inspection on September 9, 2017. The following minor changes were made:

The Best Management Practices (BMP) plan and Facility Reliability Assurance Plan (FRAP) were reviewed on January 9, 2017.
The Spill Prevention, Control, and Countermeasure (SPCC) plan was reviewed in 9/28/2017. The facility contact information was updated, the materials inventory was updated, and other minor revisions were made.
Lehigh continued to work on updating Hazardous Material Business Plan (HMBP) maps in the CERS database. Representative of the Santa Clara County Department of Environmental Health have requested several revisions.
