Intake

1. Enter CAP Module

2. Once in CAP Module, find +Add Household on left, and click to expand. Click * General.

3. Working TOP to BOTTOM of page, fill out all required fields. These fields are in light pink and have red asterisk next to them. You are encouraged to capture other easily captured information (like phone number, address, etc). Items below in yellow indicate required fields.

Comments can be added to Comment section if desired (Case Note areas in upcoming training).

Once all info is filled out, button on top changes from to . Click Save!

Next page captures Household Information. Again, fill in required fields (yellow below).

Once data is saved, user is directed to Household Member Info page (same as first page). Please fill out one Household Member Info page per Household Member. If only 1 Household member, or when finished adding, then click Cancel.

Income

Users will be directed to enter income immediately after intake. Click on Quick Add link to easily add multiple (or 1) income source.

Fill out highlighted fields in Yellow. *HINT: If you cannot find the income source which matches change the Type Dropdown*

Once information is entered, click Save. Then at the top of the screen you will now see Annual Income:

Case Management

After income is added, user will be director to Case Information page. Here, you will manage their programs. To add new program, click Add Case link highlighted below:

When adding the case, fill in the below yellow highlighted fields. IF THE FAMILY INCLUDES MORE THAN 1 MEMBER, CHECK OFF MEMBERS IN THE HOUSEHOLD AT BOTTOM UNDER APPLICABLE HOUSEHOLD MEMBERS. If only 1 member, those checkboxes will not exist (see purple circle of “0” below). Save.

Users will be prompted to Add Income to the case by checking off incomes listed, then click Save:

Once income is added, user will be directed to Case Summary page. Number in Household and Income should have appropriate information included on top of Case Information.

Scroll down to Available Services and apply the direct service given to client. HINT: If the service is not available, it is probably under another Program and you must enter the client into said program.

Click Add General Service and select service received (from dropdown), status, date, quantity and unit. Funding Source will be “No Funding Attached”. Save.

Once saved, user can see when the last service was provided:

Next, user will enter the outcome achieved once service is given. Click Add Goal.

Enter date, year, goal type and who it applies to. The year selected will prompt accurate CSBG outcomes in checkable list below. Check off applicable outcome and if the client has already achieved the outcome, indicate the date at bottom of the screen. HINT: you can add achievement date later on. Save.

After saving, you will see outcome listen under Goals. If you need to modify the outcome, or add achievement date, select Pencil.

Add Follow-up/Case Note/Referral

Users can add follow-ups, notes and referrals and bottom of the Case Summary page. Click Add Follow-up/Appointment/Referral and indicate Event type:

All events other than referral require the below highlighted fields prior to save:

Date, titles and comments are encouraged.

If a referral is chosen in Event Type, Referral Information below is required:

Once “Event” is entered click Save.

Front Desk

Users can record all contacts (phone calls, emails, walk-ins, etc) of both clients and non-client in Front Desk Module. Click Add.

Fill out date, agency, comment and topic called about. Other information is not required to save.

You can also make a referral through Add Non-client Referral.
