Albert Pike Hospital Collection
Albert Pike Hospital Collection. Records 1917–1948. 12 feet. Hospital for women and handicapped children. Admittance records and ledgers (1917–1948) of the Albert Pike Hospital, McAlester General Hospital, and All Saints Hospital, all in McAlester, Oklahoma. Also included in the collection is a certificate of incorporation (1928) granted by the state of Oklahoma to the Albert Pike Hospital. Restriction: Researchers may view the materials in this collection, but copies are not permitted. Researchers who use the collection may not cite or list specific names of children from these records in their research. Boxes 1 and 2 Records for admittance to the Albert Pike Hospital, McAlester, Oklahoma, under the Crippled Children's Assistance Fund. Includes forms used by hospitals in filing claims for services to disabled children with the following information: name of child, commitment number, mane of attending physician of surgeon, date, rate per day, amount claimed, and whether approved. Other forms include commitment orders, notices of commitment and emergency cards. The commitment orders contain case reports with name of child, age, sex, race, name of parent or guardian, phone number, address, occupation, family doctor, address, medical history, and consent verification.
