Primary Care Urgent Care Women’s Health Lab Pharmacy Mental Health, Substance Abuse &

Sexual Assault Services Allergy/Immunization/Travel Clinic Center for Health & Well-Being Health Promotion X-Ray Acupuncture, Massage Therapy Smoking Cessation, Meditation Physical Therapy Dental (provided by the UMB

School of Dentistry)

General Hours of Operation:• Weekdays 8 am – 6 pm• Saturdays 9 am – 12 noon• There are limited services after 5 pm and on

Saturdays• Appointment based with same-day and urgent care

available• Nurse Call System for after-hours• If your student is under 18, you need to sign a

“parental permit” for treatment (found on the Immunization Form)

Fees/Charges• Fees vary according to service provided; fees typical of community medical practice including co-

payments, co-insurances, and deductibles• The fee for Mental Health Services is $15 • Cancellation fee • Dental clinic charges are separate from the Health

Access to Student Records• Parents do not have access to student

health records without the student’s written permission

• Students may request a copy of their medical records for a visit, etc. by signing a release for that specific visit

Immunization Forms• Immunization forms MUST be submitted

to the Health Center during Orientation• Students will be blocked from

subsequent registration if forms are not turned in

• A fee may be assessed for forms turned in after the first day of class

Meningitis Shots Offered• Students living in residence halls must be

vaccinated against bacterial meningitis or must sign a waiver
