On behalf of UAP District A3 Chapters, I am pleased to submit this Accomplishment Report in accordance with UAP MEMORANDUM CIRCULAR NO.02,

S. 2016 for the months of August-September 2016.

The District Chapters accomplishments are summarized as follows:

5 projects/activities for BAYANIHANG ARKITEKTURA (CSR PROJECT/S OR

PROGRAM/S)

Furthermore, the above mentioned activities/accomplishments were highlighted by the following District Council activities and accomplishments:

August 19,2016 – Wreath laying at Manuel L. Quezon Statue and Tour at

QCDRRMC in commemoration of the 138th Birth Anniversary of Manuel L.

Quezon ( ANNEX A) August 20, 2016 - The P15, 000.00 payment was received by the UAP

Finance Manager Ms. Alma Valencia, as the District’s support to the JAA

CSR Activity in Iloilo. ( ANNEX B) Sept 2, 2016 – A3 District Council Meeting ( ANNEX C)

September 8, 2016 – Attended the Davies Paint Showroom Inauguration

to show support to our industry partner. ( ANNEX D)

September 29/Oct 3, 2016 – Ceremonial and Official Turnover of the P90,

000.00 District contribution to the UAPSA Scholarship Program. ( ANNEX E)

September 30, 2016 – Signing of MOU with Ways and Means Committee

Chairman Arch. Dante Aguirre for the improvement of NP’s Bedroom at

UAP Building. (ANNEX F) 100% compliance to UAP Memorandum Circular No. 02, S.2016 for the

month of July 2016 ( ANNEX G)
