Instructions

Vessel Name:

Enter vessel name as listed on the Certificate of Documentation (COD), state registration, or MISLE.

VIN:

Enter Vessel Identification Number as listed in Certificate of Documentation (COD), state registration, or MISLE.

MISLE #:

Enter MISLE activity number after opening an Activity.

Hailing Port:

Enter hailing port as listed on Certificate of Documentation (COD).

For state registered vessels, enter primary port of operation.

Call Sign:

Enter vessel call sign as listed on vessel’s FCC License.

Validate in MISLE: “Vessel Description Summary” > “Communication”.

Year Built:

Enter build year from Certificate of Documentation (COD) or state registration.

Propulsion:

Check propulsion type based on observation. If “other,” specify type.

Validate in MISLE: “Vessel Description Summary” > “Propulsion”.

Hull:

Check hull material type based on Certificate of Documentation (COD), state registration, and/or direct observation. If “other,” specify type.

Validate in MISLE: “Vessel Description Summary” > “Hull”.

# of Engines:

Enter # of engines by observation.

Validate in MISLE: “Vessel Description Summary” > “Propulsion”.

# of Shafts:

Enter # of shafts by observation.

Validate in MISLE: “Vessel Description Summary” > “Propulsion”.

Reg GT: NT:

For documented vessels, enter registered gross and net tonnage.

For state registered:

• If known, enter exact tonnage.

• If not known, enter “< 5 net tons.”

Validate in MISLE: “Vessel Description Summary” > “Tonnage and Dimension”.

ITC GT: NT:

If applicable, enter International Tonnage Certificate (ITC) gross and net tons.

Validate in MISLE: “Vessel Description Summary” > “Tonnage and Dimension”.

Total HP:

Enter vessel’s total brake horsepower.

Validate in MISLE: “Vessel Description Summary” > “Propulsion”.

Length:

Enter length overall as listed on Certificate of Documentation.

For state registered:

Use Vessel Information System (VIS) to access registration information.

Validate in MISLE: “Vessel Description Summary” > “Tonnage and Dimension”.

# Fuel Tanks:

Enter # of fuel tanks by direct observation and information provided by operator.

Validate in MISLE: “Vessel Description Summary” > “Hull”.

Fuel Cap (gal):

Enter fuel capacity based on information provided by vessel operator.

Validate in MISLE: “Vessel Description Summary” > “Hull”.

CEMS:

Verify if operator/vessel has all 5 critical elements of a Crew Endurance Management System (CEMS) per NVIC 02-08.

Removed from updated form.

Lube Oil Cap (gal):

Enter lube oil capacity based on information provided by vessel operator.

Validate in MISLE: “Vessel Description Summary” > “Hull Details”.

Hydraulic Oil Cap (gal):

Enter hydraulic oil capacity based on information provided by vessel operator.

Validate in MISLE: “Vessel Description Summary” > “Hull”.

SMS:

Check type, if any, of safety management system. If “other,” specify type.

Determine if the vessel operator has a Safety Program in place, and if so, note this next to “Other” in this block.

Removed from updated form.

Route:

Check route based on COD and determine endorsement. Establish primary route from operator. If no COD, determine primary route from vessel operator.

Validate in MISLE: in “Vessel Description Summary” > “Vessel Routes and Conditions”.

Class Certification:

Check appropriate box.

If classed, enter classification society/type on form.

Validate in MISLE: “Vessel Description Summary” > “Certificate”.

Service:

Determine primary service of vessel via operator.

Check appropriate box.

If “other,” specify type.

Location of Exam:

Enter Lat/Long of exam location based on GPS coordinates.

If applicable, enter River and Mile Marker.

Boundary Line:

Check appropriate box based on location at time of exam.

Navigation Rules:

Check appropriate box based on location during exam.

Owner:

Address:

Enter name of vessel owner and address.

Operator:

Address:

Operator Phone:

Enter managing operator’s name, address, and phone number.

Master/Mate/Pilot:

Enter name and applicable information about the vessel’s primary person in charge during exam.

Enter phone number where the vessel’s primary person in charge present during exam may be reached.

Examination Results:

Check appropriate box based on the outcome of the examination.
