• Each Year, Medicare-certified dialysis facilities are required to complete the CMS-2744 Annual Facility Survey.

• Facilities should not begin the CMS-2744 form completion process in CROWNWeb until instructed by CMS.

• When completing the CMS-2744 Annual Facility Survey via CROWNWeb, facility representatives must select “2013” for the Survey Year.

Purpose of the CMS-2744

• The CMS-2744 is designed to collect information regarding:

Treatment trends

Utilization of services

Patterns of practice in treating ESRD patients

• The CMS-2744 form is the only Office of Management and Budget survey of all facilities that provides valid census and dialysis patient movement information.

• The information is used to assess and evaluate the local, regional and national levels of medical and social impact of ESRD care.

• The CMS-2744 Annual Facility Survey:

Captures facility, treatment, and staffing data for the survey year (January 1 – December 31)

Historically, provided data to be uploaded onto CMS’ Dialysis Facility Compare website

Helps identify the top 50 kidney transplantation centers in the nation

• The CMS-2744 form in CROWNWeb is identical to the paper form.

The fields and CMS instructions are the same.

The submission process is the same as last year.

• The CMS-2744 form will go through various stages before it is complete. Facilities must monitor their forms’ status.

Network Responsibility

• ESRD Networks will have local submission deadlines to help ensure that CMS-2744 forms are FINALIZED.

• CMS will announce the national deadline for the 2013 CMS-2744 form.

• CROWNWeb gathers data from the Facility Details, Admit/Discharge, Treatment Summary, and Patient Attributes screens to populate most of the CMS-2744 form.

• The CMS-2744 form will present warning and error messages if the counts do not balance.

• Facilities can utilize CMS-2744 reports to determine where a correction may be needed.
