Conclusion: i) The common reason for 2nd attempt in phlebotomy was vein slip. ii) Incomplete documentation

The cause for the vein slip may have been:1) Poor technique2) Patient gives jerk during the prick. (especially Children ) 3) Very thin vein.4) Counter puncture.Corrective Action: i) Training to be done for the technicians .
