DEFINICIÓN

El tromboembolismo pulmonar es la consecuencia de la migración de un trombo venoso que produce la oclusión de uno o más vasos de la circulación pulmonar

Tanto a la TEP como a la TVP se engloban en un espectro de patologías denominadas TROMBOEMBOLIA VENOSA

EPIDEMIOLOGIA

- Se desconoce la verdadera incidencia a nivel mundial (USA:1era causa de muerte inesperada en hospitalizados y de juicios de mala practica medica).

- Esta estrechamente relacionada con la TVP de miembros inferiores.

- Entre un 86-96 % tiene al menos un factor de riesgo mayor.

SINDROMES CLINICOS DE TEP

1. - Síndrome de Embolia de Pulmón no complicada (15-22%)

2. – Síndrome de Infarto Pulmonar (65%). Cursa con hemoptisis y/o dolor pleurítico.

3. – Síndrome de Colapso Circulatorio (8-19%). La definición de Tromboembolismo masivo se debe

basar más en consideraciones hemodinámicas que en puramente anatómicas. Los TEP con shock tienen una mortalidad 3 ó 4 veces superior.

(70%), aprehensión (63%), tos (50%), diaforesis (36%), hemoptisis

HEPARINA DE ALTO PESO MOLECULAR: • Dosis inicial: Bolo de 5.000 – 10.000

UI o bien 80 UI/kg de peso • Perfusión continua de 20 UI/kg/hora

durante 7-10 días
