TRANSCRIPT FORMATTING GUIDELINES

COVER PAGE:

• Court/County centered in all caps at the top of the page.

List the state of Georgia on the second line, top center

of the page.

• Parties to the case at left margin, Plaintiff v. Defendant.

NOTE: When the State is the prosecutor in a criminal

action, list the state as the plaintiff but do not

include the label of Plaintiff underneath.

• Close parenthesis centered, separating parties to the case

and the case number, type of hearing, volume number, et

cetera.

• List the case number on the right-hand side of the page

parallel to the plaintiff(s).

EXAMPLE: Case No.: (In a criminal case.)

Civil Action File No.: ((In a civil case.)

• List the type of proceeding on the right-hand side of the

page parallel to the defendant(s).

EXAMPLE: Final Divorce Hearing, Motion to Suppress,

NOTE: If you're transcribing an excerpt, just a portion of

the transcript, put: E X C E R P T.

A TRANSCRIPT IS ALWAYS 25 LINES PER PAGE, with a possible

63 characters per line. This is according to Georgia law.

Q&A FORMATTING:

• The question and answer markers, (Q) or (A), are indented

five spaces from the margin, with the first word of the

question or answer starting on the fifth space after the

(Q) or (A) marker.

• The second line of a question or answer comes back to the

left-hand margin.

NOTE: If a question or answer has more than one

paragraph, the first line of the new paragraph is

indented ten spaces, so as to line up with the

first word of the first paragraph.

COLLOQUY FORMATTING:

• Colloquy is any dialogue outside of Q&A and is formatted by

putting the speaker’s name in all caps, indented ten

spaces.

NOTE: Each line in the same paragraph of that

speaker’s colloquy is indented five spaces.

• Each new speaker must be designated by their name in all

caps. Again, the first line is indented ten spaces with

subsequent lines only indented five (spaces.

TRANSCRIPT FORMATTING GUIDELINES

PARENTHETICALS:

• A transcript should allow the reader to understand exactly

what took place in a hearing, without having been present

at the hearing, just by reading the transcript. Therefore,

parentheticals are used to show when some action has

occurred in the courtroom. Parentheticals represent

nonverbal yet relevant information.

• Parentheticals must be centered on the line enclosed in

parentheses, capitalizing the starting word, and end with a

period(.) inside the parentheses.

• Parentheticals that are not standing alone on its own line and

are embedded within text the showing nonverbal communication

does not include the punctuation within the parenthesis. For

example:

A. The pain originates from here (indicating).

• Parentheticals should be concise and not contain

unnecessary wording. Below are some examples of

parentheticals and their proper wording:

(The witness was sworn.)

(Plaintiff's Exhibit l was tendered and

admitted into evidence.)

(Defendant's Exhibit 2 was marked for

identification.)

(A bench conference was held outside the

presence of the court reporter.)

(The following bench conference was held: )

NOTE: If a bench conference is taken down by the court

reporter, you would end the above parenthetical with a

colon still inside the parentheses and then transcribe

the bench conference in colloquy format with proper speaker

designations. In this example you would also put another

parenthetical at the end of the bench conference as

follows:

(The bench conference was concluded.)

CHANGING FROM COLLOQUY BACK TO Q&A:

When questions and answers are interrupted with colloquy (i.e.

objections, et cetera), format the colloquy as described above for

colloquy formatting. Once Q&A resumes by the same questioning

attorney, By Line goes after the Q inside parentheses.

BY LINES IN ALL CAPS AT THE LEFT MARGIN ARE FOR INITIAL DESIGNATION OF

EXAMINATION, CROSS-EXAMINATION, ET CETERA, WITH A CHANGE OF ATTORNEY.

FORMATTING PARENTHETICALS WHEN A WITNESS IS SWORN:

• If a witness is sworn and the attorney goes right into Q&A,

then the parenthetical should be as follows:

NOTE: You will delineate the type of examination: Direct,

Cross, Redirect, Recross, depending on the situation.

FORMATTING PARENTHETICALS WHEN A WITNESS IS SWORN:

• If a witness is sworn and there is colloquy that takes

place after the witness is sworn but before questions and

answers begin, you would use the following parentheticals:

NOTE: Again, you would delineate the type of examination

that is taking place.

PAGE AND LINE NUMBERING:

• All pages should be numbered sequentially in the bottom,

right-hand corner with the exception of the first page.

• On transcripts with more than one volume, the page numbers

are sequential from one volume to the next.

EXAMPLE: If the last page number of Volume 1 is 101, then

the first page of Volume 2 would be 102.

• All lines within the transcript are to be numbered, except

for the title/cover, index, certificate and disclosure

pages.

NOTE: 25 lines per page.

CERTIFICATE PAGE:

• All transcripts should contain a certificate page

containing the following:

State and County at the top left-hand side.

Date the proceedings were taken down.

Date the transcript was produced.

Reporter’s full name and certificate number.

Reporter’s court reporting seal.

NOTE: The County listed on the certificate page is the

county in which the court reporter has obtained their

notary public seal, not the county where the proceeding

took place.

INDEX PAGE:

• Must list all witnesses' names and the page number their

testimony begins on direct, cross, redirect and recross­

examination.

• Deposition examination titles can be limited to

examination and further examination.

• List of exhibits introduced by each party and the page

number they were tendered and/or admitted in the index is

included in the transcript. They should include a short

description and the corresponding page number. Deposition

exhibits are only marked for identification.

• The page number on which any relevant segment begins, such as jury

charge, opening/closing statements, motions, et cetera.

DISCLOSURE PAGE:

• All depositions must contain a disclosure page. You will

use the appropriate disclosure page depending on if you

have a contract with one of the attorneys or not.

• If you're using the contract disclosure page, the disclosure form must be signed by attorneys for both sides. If you're

using the "no contract" disclosure page, signature by the

attorneys is optional.

• The disclosure page must be included in the original and

copies of the transcript.

ERRATA SHEET:

• A deponent has the option to read and sign their deposition

and note any corrections on the Errata Sheet.

• If the deponent has chosen to read and sign, the Errata

Sheet is mailed to counsel for the deponent along with a

copy of the deposition and instructions for notarizing and

returning to the court reporter.

• The deponent has 30 days in which to read the deposition,

note corrections, and have the Errata Sheet notarized

and returned to the court reporter for inclusion in the

transcript.

• The Errata Sheet becomes part of the deposition and is inserted as the last page of the transcript.

NOTE: The transcript itself is not changed or

corrected. The Errata Sheet becomes part of the

record. It must be notarized and returned within the

designated time.

• If the deponent asked to read and sign but doesn’t return

the Errata Sheet within the 30 days given, then a

Certificate of Non-Signature is included in the transcript

to let the parties know the Errata Sheet was not returned.

2 THE COURT: I will prepare this order. The court

3 reporter will send a jump drive to my staff attorney who

4 will prepare the order from my oral pronouncement.

5 I’m going to grant a divorce for the plaintiff on the

6 grounds that this marriage is irretrievably broken. It's

7 unfortunate. 19 years is a long time. They have three

8 children. And it's very painful to watch the oldest

9 daughter, as Ms. Patterson said, the father-bashing, that

10 she wanted to get up here and - - that's the reason I let

11 her have her say, because she wanted to get up here and

12 bash Dad. That's very sad, and I think there's blame to

13 go around on both sides.

14 I think the mother is to blame in some regard for

15 enabling the children and promising them the moon and then

16 the dad having to work so long to be able to buy them the

17 moon. So I think there's blame. And the children, I

18 think their reality is skewed by all that, by the parents'

19 lack of, you know, realizing reality here, that they just

20 can't afford all these things.

21 So it's unfortunate the way this went down. You

22 know, it sounds like Mr. Foster's anger resulted in the

23 breakdown of the marriage. And his working all hours -- I

24 mean, working all the time also resulted in the breakdown

25 of the marriage.

Style pages vary from state to state and even within

states themselves. The flow of information remains fairly

consistent, but margins and general appearances will differ.

It is less important to concern yourself on the style being

used than it is to be able to make changes in your CAT files

to reflect the differences. You may find the following

templates to be generally accepted when you go to work, but be

prepared and able to edit the style in your software when

asked to do so.
