Township High School District 214 Registration Information 2016-2017 Welcome to the 2016-2017 school year. The first full day of classes will be Monday, August 22, 2016. Township High School District 214 uses an On-Line Registration and Fee Payment Process. You may complete your registration at home or at any location where you have Internet access. Please plan to allow at least 30 minutes to complete the registration. On-Line Registration will begin Monday, June 13, 2016, 8:00am central time. Please go to your home school website for a quick link or to https://www.d214.org/onlineregistration/ On -Line Payments will be accepted using a credit or debit card (MasterCard, Discover, or Visa). You will need the Student’s ID number, student’s home school name, phone number, and birth date to log in. Note that payment for Drivers Education Behind the Wheel is not available online. Please follow the directions on the website and verify that demographic information, especially your email address, is current and up to date. If you do not have access to a computer, or if you prefer to pay using cash, check, or money order, please go to your home school main office to complete the registration process. The Board of Education approved registration fee for the 2016-2017 school year is $360. However, if paid by July 31, 2016 the registration fee is $335. Parents are encouraged to take advantage of the early registration fee. After July 31, 2016, or if you have been denied a fee waiver and approved for a payment plan, the registration fee will be $360. There will be a $65 fee per student for those who participate in any of the identified cocurricular activities which will cover their participation for the entire year. A link to the list will be available during the on-line registration process or in your school office. Particular cocurriculars may have additional fees charged by the sponsor/coach to ensure the program continues to run successfully. Students on fee waivers will have these fees waived. Any optional fees (not included on-line), health and immunization records, athletic registrations, and other information included in this packet will need to be mailed in, or dropped off in the main office. Transportation is free for students living beyond 1.5 miles from school. Students not eligible for free transportation may purchase this service ($540 for the 2016-2017 school year), based on seat availability on regular routes. Please contact the main office for more information. If you are applying for a fee waiver, you must go to your home school main office with the required documentation for approval and to register. The required documentation includes: a copy of your 2015 Income Tax Return (1040), W-2 forms, and 4 current paystubs for each person working within the household. Reminder to all Freshmen and New Students : You must provide proof of residency and original birth certificate. Letters have been sent out with detailed information. You must submit a current physical and the appropriate immunization records in addition to On-Line Registration in order for your enrollment to be considered complete. You can download and print these medical related forms from the registration website. Returning Students : Schedules, IDs, bus schedules, and other information will be available after you have completed on-line registration and cleared any outstanding obligations. Students attending Alternative Schools or Programs Outside the District are required to register, pay fees, and apply for fee waivers at their respective home school main office. It is imperative that all students, regardless where they attend school, register at their home school. Failure to register within the appropriate timelines will result in the suspension of all services including transportation. Student Accident Insurance enrollment forms for coverage through Markel Insurance Company are available in the Main Office or on the registration portal website. If you elect this additional coverage for your student, follow the instructions outlined in the brochure. Markel Insurance Company is not affiliated with Township High School District 214. 2016-17 Physical Exam & Immunization

Requirements

Now is the time to make an appointment if you have not already done so!

FRESHMAN A health examination is required for all freshman students. The exam must be completed within one year of the first day of school. The required physical examination form can be found at: http://www.d214.org/assets/1/6/child-health-exam-form-11-15.pdf Please note: • A parent or guardian must complete and sign the health history section. • A physician’s signature, clinic’s stamp, date, immunization record, BMI, B/P, and diabetic risk assessment are required in the appropriate places. • Please schedule your doctor appointment early. Summer appointments fill quickly. • Any missing component will delay your child’s school registration until the information is obtained from the physician. To process registration a completed physical and immunization record is necessary. SENIORS All 12th grade students shall show proof of having received two doses of meningococcal conjugate vaccine prior to entering the 12th grade. The first dose shall have been received on or after the 10th birthday, and the second dose shall have been received at least eight weeks after the first dose. If the first dose is administered when the child is 16 years of age or older, only one dose is required. Please submit the required documents with registration fees by June 15. Please note that your student will NOT receive their class schedule in August until this required health information is submitted. GENERAL INFORMATION

ATHLETICS Athletic Permit: All parents must agree to the online Township High School District 214 Athletic Permit if their student(s) wish to participate in interscholastic athletics. This agreement can be checked during the online registration process.

Sports Physical: An athlete must have a yearly physical that is within 13 months of the sport in which he/she is participating. Please remember that the “Health History” section of the physical must be completed and include a parent/guardian signature. If a student’s physical has expired, remember that a new physical is due by the expiration date in order to continue to be eligible. The required physical examination form can be found at http://www.d214.org/assets/1/6/child-health-exam-form-11-15.pdf Athletic Eligibility: To try out, a student must be academically eligible, have a current physical on file at the school, and agree to the online Athletic Permit Agreement. Coaches will receive from the Athletic Office on the first day of practice, a list of eligible students. Only students whose names appear on this list will be eligible to try out.

Fall Sport Try-Outs: A student trying out for a fall sport should have a current physical turned in by June 15th and checked the online Athletic Permit Agreement. Attention to these details will help to avoid a delay of your student’s registration:

Athletes who wait until the first day of practice to turn in their materials, or whose registration is incomplete because of any of the above reasons, should not expect to participate on that day. In order to make the first day of try-outs go smoothly for your student, please make sure that athletic and school registration is completed by the due date. BUS TRANSPORTATION Bus transportation is free for students living beyond 1.5 miles from school. Bus stop number and route description will be included in the packet pick-up. The bus stop number appears on the student's identification card. Students not eligible for free bus transportation may purchase this service at a cost of $540 for the first student and $475 for each additional student, for the 2016-17 school year. Bus service is available on regular routes only. Students who request this service after the beginning of the school year will be served according to seat availability. If you are not eligible for free bus service and wish to pay for it, please send a separate check for the total bus purchase payable to Township High School District 214 prior to August 1st so that students may be included on buses during the routing process. Indicate “bus service” and student’s name and ID number on the front of the check. Bus stops are assigned by the district office based on the number of students on routes. FEES In addition to the registration fee, other fees may apply as indicated on the following fee sheet. Various booster/parent organizations and school publications are not payable online and must be paid by separate checks. Information and forms for these organizations and publications are available in the packet. FEE WAIVER FORMS A fee waiver form must be completed and submitted for each school year. To request a form for the 2016-17 school year, please call 847-718-5213. Financial verification must be returned with the completed fee waiver application. Completed 2015 federal 1040 tax forms, 2015 W2 forms, and 4 current pay stubs from all working members in the home are required for fee waiver approval. Fee waiver income guidelines as well as fee waiver application and appeal forms can be found at http://www.d214.org/about-district-214/fee-waivers/. IDENTIFICATION CARDS (School photo ID cards) Each student will receive a new identification card for the current school year. Students must carry the card in order to board buses, utilize the library and technology lab, gain access to school events, and to obtain a yearbook at the close of the school year. If you do not receive an ID card when your schedule is returned to you, please come to the Main Office between the hours of 8 a.m. and 3 p.m. beginning August 15, 2016 so that we may photograph you and process your card. Be sure to bring your printed copy of the online registration confirmation page.

INSURANCE School accident insurance is available for purchase. The information can be found at http://www.d214.org/onlineregistration/ then scrolling to the top of the page. LOCKERS Hall Lockers - All students will be assigned lockers. Students will receive the combination for the built-in locks. The locker number will appear on the student's schedule that will be in their pick-up packet. Physical Education Lockers - Locks for all physical education lockers will be distributed during physical education class. SCHEDULES - Schedules will be available for pick up: August 10, 2016

Dear Parents, Your student is enrolled in an English course for the upcoming school year. Township High School District 214’s English courses provide instruction and skill development focused on the areas of reading, writing and critical thinking skills. In the area of reading, English teachers will provide contextual and literary analysis experiences that enhance student’s personal connections to the text. A specific reading strategy included in this instruction is annotation. Annotation encourages critical reading through the use of symbols, marginal notes, essential questions, and connections both to and in the text. The process of annotation allows students to maximize these personal connections in order to create meaning, analyze, and interact with text in a way that is vital to their understanding. District 214 students will be afforded the opportunity to examine text closely and critically through the use of their own paperback novel(s). The attached list includes the title and ISBN number for the novels required for purchase in each English course at your student’s high school. Each listing indicates the semester in which the novel will be read. This will assist with your purchasing timeline. Students eligible and approved for a fee waiver will have the materials provided for them. If a course indicates “Not Applicable” there are no materials required for purchase.

Students and parents are free to purchase the listed paperback novels at any retail or online vendor, i.e., Amazon.com, in order to access the best available pricing. In an effort to provide you with options for purchase we have worked with two book vendors who have worked with the district to make inventory available for all listed materials. Multiple options are available which include in store purchase and online purchase with delivery to your home. We hope that this information is of assistance to you.
