Welcome to your 2019 Benefits!The employees of Topeka & Shawnee County Public Library (TSCPL) are our most valuable asset, which is why we are committed to offering a comprehensive benefits package that helps you stay healthy, feel secure and maintain a work/life balance.

This booklet is a snapshot of the benefits available and is intended for illustrative purposes only. Please refer to the Summary Plan Descriptions or Plan Certificates for further details. These can be provided by Human Resources. We hope you find this useful as you make decisions for you and your family.

EligibilityTSCPL defines benefit-eligible employees as:• Full-Time Regular (FTR) - active employee working 30+ hours per week.• Part-Time Regular (PTR) - active employee working 20-29.9 hours per week.

FTR & PTR employees can enroll in any of the offered benefits. For newly eligible employees, the coverage will begin on the first of the month following, or coinciding with first date of hire.

You may elect coverage for your legal spouse (defined as husband or wife) and qualified dependent children (natural, adopted, stepchild or child placed with employee for adoption) through the end of the month in which the child turns 26.

Part-Time Intermittent (PTI) employees are not eligible for benefits, except KPERS 457 deferred compensation.

Coverage LevelsMost benefit options allow you to choose from four coverage level options:

• Employee Only• Employee + Spouse• Employee + Child(ren)• Employee + Family

In order for a spouse or child to be enrolled, you must enroll in the benefit yourself. You may elect different coverage levels for each benefit. For example, you may enroll in Employee Only medical and Family dental.

Payroll DeductionsYour health, dental and vision insurance premiums will be deducted in the month prior to the effective date of coverage.

KPERS deductions begin with your first paycheck.You can begin deferring pay into both of the 457 plans beginning on the first paycheck.

Optional Group Life Insurance (OGLI) will begin when KPERS notifies HR of the deduction.

HSA Annual Contribution Limits for 2019

Health Savings Account (HSA)*Option A (High Deductible Health Plan), is a qualified plan, which means you are eligible to open a Health Savings Account (HSA) to fund with pre-tax dollars to pay for qualified health expenses.

The Library will be providing an employer contribution of $360 ($30 per month) for full-time regular employees with Individual Coverage, or $720 ($60 per month) for full-time regular employees with Employee + Spouse, Contributions cease on the last date of employment.

In addition to the monthly contribution, the Library will be making two special contributions into the HSA in January & July of 2019 of:

• $500 for full-time regular employees with Individual Coverage for a total of $1,000 in 2019.• $1,000 for full-time regular employees with Employee + Spouse, Employee + Children or Full Family Coverage

for a total of $2,000 in 2019.• Part-time regular employees receive a prorated amount.

TSCPL is partnering with Community Bank who will provide you with a debit card and online services to access the HSA. The funds in the account are yours. Any unused funds will remain in the account, even if you cancel the health plan.

Flexible Spending Account (FSA)*FSAs provide you with an important tax advantage that can help you pay health care and dependent care expenses with pre-taxed dollars. By anticipating your family’s health care and dependent care costs for the next year, you can actually lower your taxable income.

The Health Care Reimbursement FSA allows you to pay for certain IRS-approved medical expenses with pre-tax dollars that are not covered by your insurance plan. The annual maximum amount you may contribute is $2,700.

Some samples of eligible expenses include: hearing services (including aids and batteries), vision services (including contact lenses, eyeglasses) dental services, chiropractic services, acupuncture, medical services and/or prescriptions.

The Dependent Care FSA allows you to use pre-tax dollars to pay for qualified dependent care for children under the age of 13 or caring for elders. The annual maximum amount you may contribute is $5,000 (or $2,500 if married and filing separately) per calendar year.

Topeka & Shawnee County Public Library’s claims administrator for FSA plans is First Concord Group.

Your FSA elections are effective January 1 through December 31. Claims for reimbursement must be incurred by March 15 of the following year and submitted to First Concord Group by May 1. Any unused funds will not be returned or carried forward so you will be forfeiting that money to the plan.

*Due to IRS regulations, you and/or your spouse cannot be participating in an FSA & HSA at the same time. This includes spouses participating in his/her employer FSA plan. You can open an HSA once the FSA contributions have stopped and all have been spent.

Cancer InsuranceAll American Marketing will pay cash benefits for expenses such as: positive diagnosis, National Cancer Institute/Cancer Treatment Center evaluations and consultations, transportation and lodging, second and third surgical opinion, medical imaging, treatment planning and monitoring, anti-nausea medication, colony stimulation factor or immunoglobulin, outpatient hospital or ambulatory surgical center, prosthesis, non-local transportation, inpatient and outpatients blood/plasma/platelets, bone marrow donor and stem cell transplant, ambulance, inpatient oxygen, attending physician, inpatient and outpatient private duty nursing, convalescent care facility, rental or purchase of medical equipment, home health care, hospice care, hairpiece expense, physical/speech/audio therapy and psychotherapy. Diagnosis must occur after the plan effective date and must be for Cancer as defined in the policy.

Waiver of premium - All American Marketing will waive the premiums starting on the first premium due date following a 60 day period of total disability. The Insured must be (a) receiving treatment for Cancer for which benefits are payable under this policy; and (b) remain disabled for 60 consecutive days. Premiums will be waived for as long as the Insured remains totally disabled.

Employer Sponsored Basic Term Life InsuranceTopeka & Shawnee County Public Library automatically enrolls you in the Basic Term Life insurance benefit through KPERS. The benefit is 150% of your annual earnings at no cost to you.

Voluntary Life Insurance (OGLI)Employees who want to supplement their library provided group life benefit may purchase additional voluntary Term Life coverage. When you enroll yourself and/or your dependents in this benefit, you pay the full cost through payroll deductions.

You may purchase coverage on yourself in $5,000 increments, up to $400,000. If elected within 30 days of becoming a new member, you may elect up to $50,000 without providing proof of good health.

You may purchase coverage on your spouse in the amounts of $5,000 increments, up to $100,000. If elected within 30 days of becoming a new member he/she may elect up to $25,000 without providing proof of good health. Employees can elect child(ren) coverage for $1.20 per month for $10,000 of coverage or $2.20 per month for $20,000 of coverage.

Beneficiary - it is important to designate the beneficiary(ies) for the basic term and voluntary life insurance coverage by downloading the form at kpers.org/form.html. You are the beneficiary of any spouse or child coverage.

Long Term DisabilityTopeka & Shawnee County Public Library automatically enrolls you in the Long Term Disability benefit through KPERS. The benefit provides financial protection by replacing a portion of your income should you become disabled and unable to work.

For approved claims, benefits will begin the later of the following:• The date you complete 180 continuous days of total disability• The date you cease to draw compensation from Topeka & Shawnee County Public Library.

The benefit amount is 60% of your current annual rate of compensation on the date the disability began, payable in monthly installments. The maximum benefit payable is $5,000 per month.

IDShield IDShield is an optional after-tax benefit that provides full service identity theft protection. It provides services such as: privacy monitoring, security monitoring, social media monitoring, credit monitoring, credit inquiry alerts, monthly score tracker, consultation, mobile services, IDShield Vault and full service identity restoration with a $5 million service guarantee.

Paid Time Off Policy (FTR & PTR Employees)Vacation: Vacation eligibility is computed on the basis of twelve months of continuous employment. Full-time regular and part-time regular employees are entitled to paid vacation time based upon their classification and years of service.

Sick Leave: All full-time regular (FTR) and part-time regular (PTR) employees are entitled to sick leave which accrues at the rate of .0462 per hour paid or 96 hours per year for full-time employees.

Paid sick leave taken will be subtracted from the employee’s total accumulated (unused) paid sick leave eligibility. Unused paid sick leave can be accumulated up to a maximum of 240 days.

This benefit terminates upon separation from employment with the Library, and no cash settlement is made, except to qualified retirees, as a result of accumulated sick leave time.

Holidays: The Library will observe the following holidays:

FTR and PTR employees are paid for these holidays. When a holiday falls on an employee’s regular day off, the employee is entitled to Holiday Leave to be taken within six months following the holiday. Employees will receive the number of hours based on their position as detailed below:

Jury Duty Leave: All FTR and PTR employees are allowed paid leave up to a maximum of 14 days if summoned for jury service in court as a witness. To qualify for jury duty, submit a copy of the summons to your supervisor upon receipt. Proof of attendance must be submitted to your supervisor when your period of jury duty is completed.

• Hours spent on jury duty that occur during an employee’s normal work hours at the Library are considered for pay purposes as hours worked for the Library.

• For regular full-time employees, a full workday on jury duty is considered for pay purposes as eight hours of work for the library.

Funerals: In the event of the death of a member of the immediate family, a full-time regular or part-time regular employee is eligible for up to four consecutive workdays of paid leave. Members of the immediate family include: parents, spouse, children, brother, sister, grandchildren, mother and father-in-law, brother and sister-in-law, and longtime companion living in the same household. One day of leave will be allowed for funeral leave for death of other members of the family. Consideration may be given for additional leave that can be charged to available vacation time if conditions warrant and the employee’s supervisor consents.

Education Leave: Full-time regular and part-time regular employees, except shelvers, may be granted time off with pay to attend approved classes, conferences, seminars, training programs, or workshops as determined by the Library administration. Educational leave time is considered as hours worked when computing overtime premium pay.

Unpaid Time Off PolicyPROTECTED LEAVEMilitary Leave: A full-time regular (FTR) or part-time regular (PTR) employee who enters the armed forces of the United States will be entitled to a leave of absence without pay for the period of service required and will be granted re-employment rights as specified under USERRA. Any such leave will freeze salary and benefits.

A FTR or PTR employee who is a member of any federally recognized reserve component of the armed forces shall be granted time off, not to exceed fifteen working days in any one fiscal year, when called for training. The Library will pay the difference between the employee’s daily military pay and the employee’s daily regular pay for the leave period provided military pay does not exceed the regular pay and a military pay stub is presented for verification. Such leave will not affect benefits.

Family Medical Leave Act (FMLA): Under FMLA, eligible employees are entitled to a total of 12 work weeks of leave during any 12 month period under conditions set by the Act and in accordance with Topeka and Shawnee County Public Library Policy.

To be eligible for coverage, an employee must:• Have been working for the Library a total of 12 months before the leave request (these need not be consecutive)• Have worked at least 1,250 hours during the previous 12 months.

An eligible employee may be granted leave for one or more of the following reasons:• For the birth of a son or daughter and to care for a newborn child;• For the placement with the employee of a child for adoption or foster care, and to care for the newly placed child;• To care for an immediate family member (spouse, child or parent) with a serious health condition; and• When the employee is unable to work because of a serious health condition.

EMPLOYEE ASSISTANCE PROGRAM (EAP) - New Directions

• A free service, separate from your health insurance. Topeka & Shawnee County Public Library pays 100%.

• A confidential service. No information, including your name, is released without written permission. Topeka & Shawnee County Public Library will never know if you use this program.

• A service available 24/7 to you and all immediate family members that live in your household.

What is an Employee Assistance Program?

How and when should I use the EAP?The EAP provides phone and face-to-face assessments and consultative sessions when you need help with, but not limited to, stress at home or work, relationships, parenting, financial advice, grief, depression, anxiety, and substance abuse.

Life After Work: Retirement BenefitsKPERS Retirement: KPERS 1& 2 are defined benefit plans. Retirement benefits are calculated using a formula based on years of service, final average salary, and age multiplier. KPERS 3 is a cash balance plan. For all 3 programs, employees pay 6% and the Library pays 8.39% or more.

Salary Deferral (457 Plan): Defined contribution plan, your contributions & investment decisions. The Library currently offers two plan options, KPERS 457 and OneAmerica Securities.

Retiree Health Insurance: TSCPL will pay a portion of the group health insurance premium of a qualified retiree and the dependents, should the qualified retiree elect to participate in the health insurancepackage available through TSCPL. A qualified retiree is one who:

• on the date of retirement has been employed by TSCPL and eligible for Plan benefits in the aggregate of not less than ten years;

• is receiving retirement or disability benefits under the Kansas Public Employees Retirement System (KPERS) for service with TSCPL;

• is under age 65;• retires or has retired from TSCPL; and• has given the Human Resources Department written notice of his or her application to continue receiving Plan

coverage for the qualified retiree and his or her dependents.

More information is available from the Human Resources Department.

TSCPL reserves the right to change, amend, or terminate this program or to require increased employee premium contributions toward any benefits. This reserved right may be exercised in the Library’s discretion and in the absence of financial necessity.

Retiree Sick Leave Pay-Out: Retiring employees will be compensated for 1% of accrued sick leave per year of employment up to twenty-five years. The rate of compensation will be based on the hourly rate of the employee at the time of retirement.

As we welcome nearly 3,000 visitors every day, and serve more than 100,000 registered borrowers, it is clear the library is valued.
