DEAR DR. DONOHUE: After an endoscopy, the doctor told me I have a hiatal hernia. He said nothing could be done for it and that I will have to live with it the rest of my life. He gave me no medicines or advice. I am anxious about this and would appreciate any info you can give me. -- Anon.

ANSWER: The swallowing tube -- the esophagus -- begins at the throat, travels down the chest and finally ends by attaching to the stomach. To reach the stomach, it must go through an opening in the diaphragm, the muscle that separates the chest from the abdominal cavity. That opening is the hiatus. A hiatal hernia is a bulging of part of the stomach through the hiatus and into the chest cavity. Many times, a hiatal hernia causes no symptoms and needs no treatment.

At other times, a hiatal hernia produces GERD -- gastroesophageal reflux -- an upward spurting of acid and digestive juices from the stomach into the esophagus. That brings on heartburn, a common problem with many treatments. People without such a hernia also develop GERD.

Here are some tips to handle heartburn. Lose weight, if need be. Don’t eat within three hours of going to bed. Elevate the head of your bed using blocks that are 6 to 8 inches tall and placed under the bedposts. This keeps stomach juices in the stomach while you sleep. Sleep on your left side for the same reason. Don’t wear constricting garments around the abdomen or too tight a belt. Take antacids as needed -- Tums, Rolaids, Mylanta and Maalox. Eliminate any food that gives you heartburn. Usual offenders are citrus fruits, tomatoes, onions, spicy foods, fatty and fried foods, chocolate, carbonated beverages, mints and caffeinated drinks.

If these steps don’t solve the heartburn problem, try medicines called acid blockers: Tagamet (ci-metidine), Zantac (ranitidine), Pepcid (famotidine) and Axid (nizatidine). In low doses, all of these can be purchased without a prescription.

The next step is medicines that turn off acid pro-duction. Prilosec (omeprazole) doesn’t require a prescription. The prescription drugs are Prevacid, Nexium, Aciphex, Protonix and Dexilant.

If necessary, surgical procedures can correct hiatal hernias.

Getting a Handle on Heartburn

DEAR DR. DONOHUE: My barber, Vinnie, said my eyebrows were receding and that is an indica-tion of thyroid problems. I write to see what your take is on this. I would like to establish fact from fiction. -- P.D.

ANSWER: Vinnie is somewhat near the truth by saying hair loss is a possible consequence of both an underactive and overactive thyroid gland. However, the loss is usually on the scalp, when and if it does occur. Loss only of eyebrow hair isn’t a symptom of thyroid disease. And hair loss from the scalp is not the only sign that things are wrong with the thyroid. Other signs must be present, too.

• On July 16, 1863, the draft riots enter their fourth day in New York City in response to the Enrollment Act. Although avoiding military service became much more difficult, wealthier citizens could still pay a commutation fee of $300 to remain at home.

• On July 11, 1899, E.B. White, the author of the popular children's novels "Charlotte's Web," "Stuart Little" and "The Trumpet of the Swan," is born in Mount Vernon, N.Y. White also updated and expanded "The Elements of Style," an English usage guide that remains a standard text for many students.

• On July 15, 1903, the newly formed Ford Motor Company takes its first order, from Chicago dentist Ernst Pfenning. The $850 two-cylinder Model A automobile with a tonneau (or backseat) was produced at Ford's plant on Mack Street (now Mack Avenue) in Detroit, and delivered to Dr. Pfenning just over a week later.

It's almost impossible to have a daydream about a nightmare. It happened to me, though, during the press conferences at Michigan International Speedway on June 17.

Stock-car racing changed a lot when Dale Earnhardt died be-cause, in his absence, Imperial NASCAR had no one who would stand in its way. Racing changed when Winston left, not because

RJR sold tobacco but because its leaders understood racing and put their money where their mouth was. Winston had clout. Sprint just writes a check and, as a result, NASCAR has been unchecked ever since.

David Poole's death removed an obstacle from NASCAR's path. Poole stood up to a ruling body that was interested in the media only to the extent that it could manipulate it. A bone of contention between Poole and me was his belief that I was not civic-minded enough. David would trot off a heated email in a heartbeat. He got frustrated at me because I didn't think it did any good.

If I told David once, I told him 50 times, "Sorry. I'd rather have the bad will." If I feel I'm being treated badly, it fuels my writing. "I'd love to help you, David, but that stuff just costs me too many columns."

NASCAR doesn't have drivers anymore. The cars are being piloted by drones. I thought drug testing consisted of urine samples, but some NASCAR drivers act as if they've had samples taken from their brains.

Stock-car racers are supposed to be rugged individualists. The sport grew out of hell-raising, plain-speaking men who said what they thought and didn't lose any sleep over what anyone else thought. The legends of Curtis Turner, Joe Weatherly, Tim Richmond and Junior Johnson are as alien to modern NASCAR as King Arthur and Guinevere to Prince William and Kate. It took England centuries to change. It took NASCAR a decade.

As I read the wholesome pearls of Tony Stewart's wisdom, I think of neither Earnhardt nor Poole.

Stewart: "... at the end of the day, I like to have a sanctioning body like NASCAR that is going to make sure things go the way that they are supposed to be and that they aren't pushing it that far."

I could've gotten that from an episode of "The Mickey Mouse Club": "N-A-S ... because we love you ... C-A-R ... M-O-U-S-E." Except in NASCAR, no one even goes through the motions of echoing back "Donald Duck!"

Last week I couldn't believe my ears reading what Ryan Newman said. On Friday, another driver who used to halfway answer a question straight, Denny Hamlin, sounded as if he had been guzzling Dr. Feelgood's Magical Elixir, too.

Ryan Newman, who apparently has been secretly fined at least twice, said last week, "When we are talking about fines, whether it is private or public, there is nothing really we should elaborate on because it is not something that our sport should be proud of or should elaborate. To me, it is something for you guys to write, but it is not something that is good for our sport, so it's not something we want to keep talking about, first of all. ...

"It is a negative aspect of our sport, and we should all be talking about the positive things."

I don't know whether the drivers have gone Stepford or Manchurian. NASCAR offi-cials have some-how gotten them to believe that the sport is hurt every time they express the view that NASCAR is less than godlike.

Q: I was a huge fan of CBS’ “Without a Trace,” so I was thrilled to see that Poppy Montgomery is coming back to TV this fall, starring in “Unfor-gettable.” What made

her decide to return to television? -- Darcy R., via e-mailA: I spoke with Poppy recently, and I asked her just that,

and it basically boiled down to many elements coming to-gether at the perfect time. Poppy told me: “The script was amazing. I’m obsessed with Niels (Arden Oplav), the director, and had been long before I ever met him. I’ve seen ‘The Girl with the Dragon Tattoo’ maybe 30 times.

“I’ve worked with CBS since I was a baby,” she said. “So it’s like coming home again. It’s such a great place for me to work, and I feel like they support their shows so wholly and completely. Everything about it lined up for me. And I want to see a strong female character leading an exciting show.”

Speaking of strong female characters, Poppy portrays “Harry Potter” author J.K. Rowling in Lifetime’s original movie, “Magic Beyond Words: The J.K. Rowling Story,” which airs Monday, July 18. Of playing the famously reclusive author, Poppy told me: “When they sent me the script I definitely had reser-vations, because I knew that J.K. Rowling wasn’t col-laborating on it. But when I read the script, I found that it was a beautiful, inspiring story and a love letter to an extraordinary woman.”

Safety and GrowthIn our 29 years helping clients plan with their finances, we are always faced with this priority

decision: where do we place the next deposit to achieve the best relative return while enduring the least possible risk? As any Financial Advisor out there can attest, this is a common question. Un-fortunately, most advisors fail to

see the potential pitfalls in placing too much faith in the financial markets. Don’t forget, the stock market was an uncommon place to invest until the last 30 years! With the advent of mutual funds, variable annuities, and retail brokerage accounts, consumers have an almost unrestricted access to the stock market. This can be a good thing for some, and, of course, has been a sheer disaster for others.

For those who wish to invest in the stock market, but are afraid of losing money, there exists a relatively new retail product. In 1995, an insurance-based investment hit the retail market. In that year, sales were a paltry $200 million nationwide. By 2010, sales topped a staggering $31 bil-lion. This represents more than a 1500% increase in 15 years. Why such demand? The answer is simple: The Opportunity for Safety and Growth - on the same dollar at the same time. The product is called a “Fixed Indexed Annuity”, or “FIA”. As with all annuities, the growth is not taxed until withdrawn and the assets bypass probate at death. But that is not why nationwide sales are sky-rocketing. In an FIA, the investor is offered the opportunity to index upwards with the market; the Dow Jones, S&P 500, the NASDAQ or all three. Interest is added to the principal annually and the resulting balance creates the new guaranteed minimum going into the next year. In the event of a down market, the principal is protected. Yes, it’s true; the account can grow in a positive market and does not lose money in a negative market.

Heads; You Win! Tails; You Don’t Lose! The most ingenious feature of the FIA is a unique one. Unlike all other investments, the profit-taking is automatically done for you. Every year there is a gain; it is paid as interest and locked in – permanently. In other investments, your profits are at risk of being lost in the following year. We have all seen this in the markets; a good year can be elimi-nated by a subsequent bad year, and so on. With an FIA, this risk has been removed altogether. This feature is called the “annual reset”. Essentially, the worst case scenario of an FIA is a zero gain for the year. With only the upside and none of the downside risk, this may be an interesting product for nervous investors. These appealing features are afforded the investor while he or she is still living. As with other market-based annuities, the owner must die to get such protection. For 2011, projected sales of FIAs are nearing $50 billion. If you like the idea of a safety net underneath your nest egg and still want better returns than the bank or other low-rate products, give us a call.

where the unconventional Pippi and her best friends Tommy and Annika fastened scrub brushes to the bottom of their feet and breezed through mounting soap suds on the kitchen floor, singing “Scrubbing day, is a holiday!” Cy and his friends have discovered that cleaning floors can be playful in their homes, too.

Inexpensive hand towels are wrapped under and over their feet and fastened together at the ankles with a thick rubber band. When the music blasts through the speakers in the family room, on cue they grab decorated plastic water bottles filled with a vinegar/water cleaning solution (one part white vinegar to three parts water) and begin their “squirt ... and slide” routine to clean and shine the hardwood floors.

He coined a term -- “Team Gleason” -- to en-courage his young children to work together as a family team when getting everyday jobs done. “’Done’ might not be the same standard I might set for myself, but who cares if there are a few streaks here or leftover spots there,” he concedes. “When doing summer chores, it’s really about being to-gether, learning something new and connecting on common ground.”
