When a beneficiary approaches the health desk, KPO will enter the CNIC / Health Card Number to of

beneficiary.

Having entered the CNIC number KPO will choose the ‘Medical’ as visit type.

If the beneficiary family exists in our database, all the family members will be shown in a screen.

We have to click any of the member to get the available balance.

Available balance is shown.

Upon clicking any of the category in balance portion we will get the history of consumption by the

family.

The history tab shows the detail of previous treatments for the family / members.

The hospital tab shows nearby hospitals.

When we click the ‘ref’ letter’ button in ‘family’ tab. We will get this referral letter. This letter be handed

over to the patient/attendant and he will show it to the hospital doctor. Hospital doctor will fill in the

letter and will decide what type of treatment the patient needs (i.e. if it is OPD, admission, referral to

another hospital etc.)

After the printing of ‘referral letter’ visitor will appear under the ‘checkup’ tab

When we double click the ‘visit number’ in ‘checkup’ tab we will this screen to enter the doctor’s

handwritten entries of referral letter. There are various types of visits to choose from. If we choose

‘wrong entry’ or OPD then we have to fill in all the fields and ‘check’ mark the return box to save and

close the case.

In case we choose ‘referral’ then we have to mention the reason of sending this patient to another

hospital.

We have a list of all empanelled hospitals to select the hospital where we want to send the patient.

Also we have to mention the condition of the patient, along with other necessary information.

If we choose ‘ante natal’ the we have to provide certain necessary information as shown in the screen

above.

In case we choose ‘pre-admission’ as visit type and fill in all the information from ‘referral letter’ then

we will be carried to this next screen. This screen will enable us to choose the appropriate type of

treatment/procedure the patient has been advised by the hospital doctor. We have to select the

appropriate ctagory (i.e. ‘secondary’ or ‘tertiary’ ).

After selection of category next is to choose the line of treatment.

If we want to admit the patient for ‘non-surgical’ / day care the we will choose the ‘Medical (general

ward)’ as our line oftreatment.

After having chosen ‘medical (general ward)’ we will have to choose the desired treatment from the list,

as ‘non-surgical’ in this screen.

Now our choice of non-surgical is complete and the per-day cost will appear along with the balance.

In case we want to change the treatment then we will click the ‘Del’ button place at the right most of

treatment portion in screen. After having deleted the wrong treatment we will repeat the previously

mentioned process to enter/choose correct treatment.

After correct selection of treatment, we have to choose the ‘admission type’. If we choose ‘normal’ then

we have to wait to DMOs approval or for a time lapse of half an hour if the DMO could not respond to

our request. If we choose ‘emergency’ then we are able to proceed further without any delay.

After having chosen the admission type we will have to provide the information about attendant. The

correct recording of this information is very important as the concerned authorities will contact this cell

number for feedback.

The expected discharge date will be entered in format at shown above (i.e. ddmmyyyy without any

dash).

Then we will choose the status, which is ‘admission’ in case we are going to admit the patient.

Afterwards we will save the information and click the ‘Case Sheet’ button.

Upon clicking the case sheet we will get this print out, showing all relevant information relating to this

admission including treatment and balance etc.

Now you will notice that the patient has moved from ‘Checkup’ tab to ‘InPatient’ tab. If we double click

the ‘visit No.’ we will be prompted with the screen where we chose the treatment, as shown below.

This screen enables us to ad/change the treatment. Also, in case of ‘delivery’ we have the option to add

‘baby’. We click the ‘baby’ button and we get the input box to enter information about the new born

baby.

We have to choose the gender of baby.

The relation of baby.

Now save the information about baby. This will add the baby as a member of the family and the baby

will be able to get treatment in case of need. Please note that the baby information has to be saved

before the discharge of mother in case of ‘delivery’.

After the treatment is complete we will click the ‘Discharge’ button to enter required information before

discharge of the patient.

This screen enables the input information relating to discharge. Also, this screen shows the ‘Discharge

Plan’ entered by the DMO.

We will choose the appropriate ‘type’ of discharge. (LAMA means Left Against Medical Advice).

Also mention if the patient has been paid transportation charges at the time of discharge.

Enter the remarks, if any, and check if the ‘Post Discharge (PD) Medicine’ has been given to the patient

for 5 days.

We have to enter the detail of Post Discharge Medicine.

Dose of Post Discharge Medicine.

The feedback of the patient/attendant will be captured before discharge. Having done all we will save

and click the ‘Discharge’ Button.

This is the specimen of discharge slip. It has the detailed particular of the patient, feedback from them

and a receipt for the transportation charges. This will be signed by patient/attendant and the authorized

personal of the hospital.

Now the discharged patients appear under the tab ‘Discharge’

from the ‘Claims_Lodging’ tab KPO will check the box under ‘lodge’ column. Afterward he will click the

‘Save and Print’ button to lodge the claim.

A claim number is assigned to this treatment and a sheet will be printed having the details of one or

more admissions selected for lodging. Now, the KPO will send following documents to DMO, along with

this sheet:

3. Copy of patient CNIC, if other than Head, or Form ‘B’ in case of minor

4. Referral letter

5. Case sheet

6. Discharge slip

7. Copy of hospital’s treatment chart.

KPO can view the settlement status of his claims under the tab ‘Claims_status’
