Upper GI Bleeds
• Understand the causes• Goals of resuscitation• Pharmacological resuscitation• Procedural resuscitation
![Page 3: The Upper GI Bleeder](https://reader036.vdocuments.mx/reader036/viewer/2022062702/554aeda4b4c905852a8b548a/html5/thumbnails/3.jpg)
Upper GI Bleeds in ED
• Its challenging• Effective Mx = good PT outcomes• Underlying comorbidities ∧ complexities• Team approach• Pharmacology & procedural approach
![Page 4: The Upper GI Bleeder](https://reader036.vdocuments.mx/reader036/viewer/2022062702/554aeda4b4c905852a8b548a/html5/thumbnails/4.jpg)
Upper Vs Lower
• Consider upper first– More life threatening
• Haematemisis = Upper GI source• Bright red blood not always = LOWER GI
![Page 5: The Upper GI Bleeder](https://reader036.vdocuments.mx/reader036/viewer/2022062702/554aeda4b4c905852a8b548a/html5/thumbnails/5.jpg)
Case Study
• 52 Male• Chronic ETOH abuse• Known varices
C/O – Vomiting blood post binge
![Page 6: The Upper GI Bleeder](https://reader036.vdocuments.mx/reader036/viewer/2022062702/554aeda4b4c905852a8b548a/html5/thumbnails/6.jpg)
What to Look for!
![Page 7: The Upper GI Bleeder](https://reader036.vdocuments.mx/reader036/viewer/2022062702/554aeda4b4c905852a8b548a/html5/thumbnails/7.jpg)

The Upper GI Bleeder
Multi Team Approach:• ED, ICU – resuscitate• Gastro – scope• Interventional Radiologist – therapeutic Ix• Surgeons – surgical intervention
![Page 9: The Upper GI Bleeder](https://reader036.vdocuments.mx/reader036/viewer/2022062702/554aeda4b4c905852a8b548a/html5/thumbnails/9.jpg)
The Source
• Duodenal ulcers- 28%• Gastric ulcers- 26%• Gastritis- 13%• Varices - 12%• Esophagitis - 8%
• “Massive GI Bleed Mortality rate 20-39%”
![Page 10: The Upper GI Bleeder](https://reader036.vdocuments.mx/reader036/viewer/2022062702/554aeda4b4c905852a8b548a/html5/thumbnails/10.jpg)
The DDx!
• Intranasal• Intrapulmonary
![Page 11: The Upper GI Bleeder](https://reader036.vdocuments.mx/reader036/viewer/2022062702/554aeda4b4c905852a8b548a/html5/thumbnails/11.jpg)
Remember!
Early Interventioncan mean
difference between
Life & Death
![Page 12: The Upper GI Bleeder](https://reader036.vdocuments.mx/reader036/viewer/2022062702/554aeda4b4c905852a8b548a/html5/thumbnails/12.jpg)
The 3 Goals of Resuscitation
1 Provide Intravascular Volume Resuscitation
2 Optimise Oxygen-Carrying Capacity
3 Reverse Coagulopathy
![Page 13: The Upper GI Bleeder](https://reader036.vdocuments.mx/reader036/viewer/2022062702/554aeda4b4c905852a8b548a/html5/thumbnails/13.jpg)
Initial Resuscitating
• Start with ABCDE• x2 Large bore IVC – Bloods• Full monitoring• Get specialties involved• Arrange blood products• Consider limitations of care!
![Page 14: The Upper GI Bleeder](https://reader036.vdocuments.mx/reader036/viewer/2022062702/554aeda4b4c905852a8b548a/html5/thumbnails/14.jpg)
Intravascular Volume Resuscitation
• Limit crystalloid fluid –to early phase
Prepare for transfusion:• HB <80• Coagulopathy• Persistent hypotension
![Page 15: The Upper GI Bleeder](https://reader036.vdocuments.mx/reader036/viewer/2022062702/554aeda4b4c905852a8b548a/html5/thumbnails/15.jpg)
Intravascular Volume Resuscitation
• PRBC’s not enough• Replace clotting factors• Consider massive transfusion protocol• “PRBC:FFP:platelet ratio 1:1:1”
• Tranexamic Acid?
![Page 16: The Upper GI Bleeder](https://reader036.vdocuments.mx/reader036/viewer/2022062702/554aeda4b4c905852a8b548a/html5/thumbnails/16.jpg)
Optimise Oxygenation
Signs of decreased O2 delivery:– Decreased LOC– Evidence of cardiac ischaemia– Increased lactate– Cold peripheries
![Page 17: The Upper GI Bleeder](https://reader036.vdocuments.mx/reader036/viewer/2022062702/554aeda4b4c905852a8b548a/html5/thumbnails/17.jpg)
Optimise Oxygenation
• Give blood so oxygen can get to the tissue
Initially:– Provide High Flow 02
Crashing:• Intubate early
Stablised:• Titrate oxygen to need• Considered humidified
![Page 18: The Upper GI Bleeder](https://reader036.vdocuments.mx/reader036/viewer/2022062702/554aeda4b4c905852a8b548a/html5/thumbnails/18.jpg)
Intubating Isn’t Easy!
1. Intubate early2. Empty stomach (NGT)3. Intubate with HOB at 45°4. Preoxygenation5. Limit BVM6. Use experience7. Prepare for vomit
![Page 19: The Upper GI Bleeder](https://reader036.vdocuments.mx/reader036/viewer/2022062702/554aeda4b4c905852a8b548a/html5/thumbnails/19.jpg)
![Page 20: The Upper GI Bleeder](https://reader036.vdocuments.mx/reader036/viewer/2022062702/554aeda4b4c905852a8b548a/html5/thumbnails/20.jpg)
Reverse Coagulopathy
• Tailor to etiology.
Causes:• Anticoagulation• Shock– Metabolic acidosis –tissue hypoperfusion
• Chronic disorders– ETOH abusers
![Page 21: The Upper GI Bleeder](https://reader036.vdocuments.mx/reader036/viewer/2022062702/554aeda4b4c905852a8b548a/html5/thumbnails/21.jpg)
Pharmacological Interventions
• Proton Pump Inhibitors• Somatostatin• Vasopressin
![Page 22: The Upper GI Bleeder](https://reader036.vdocuments.mx/reader036/viewer/2022062702/554aeda4b4c905852a8b548a/html5/thumbnails/22.jpg)
Proton Pump Inhibitors
• Suppress gastric acid production• Potential reduction haemorrhage during scope
The evidence though:
![Page 23: The Upper GI Bleeder](https://reader036.vdocuments.mx/reader036/viewer/2022062702/554aeda4b4c905852a8b548a/html5/thumbnails/23.jpg)
Somatostatin
• (Octreotide)Reduces:• Portal venous blood flow• Splachinic vasoconstriction
=decrease GI Bleeding
Use• Variceal GI bleeds• Limited evidence – low side effect profile
![Page 24: The Upper GI Bleeder](https://reader036.vdocuments.mx/reader036/viewer/2022062702/554aeda4b4c905852a8b548a/html5/thumbnails/24.jpg)
Vasopressin
(Telipressin)• Reduces portal hypertension• Splachnic vasoconstrictor• Can cause ischaemia
• Last ditch effort in bleeding varices
![Page 25: The Upper GI Bleeder](https://reader036.vdocuments.mx/reader036/viewer/2022062702/554aeda4b4c905852a8b548a/html5/thumbnails/25.jpg)
Antibiotics
• Infection on varices– Causes bleeding
• Give antibiotics (broad spectrum)
![Page 26: The Upper GI Bleeder](https://reader036.vdocuments.mx/reader036/viewer/2022062702/554aeda4b4c905852a8b548a/html5/thumbnails/26.jpg)
Procedural Interventions
• Endoscopy• Balloon Tamponade
![Page 27: The Upper GI Bleeder](https://reader036.vdocuments.mx/reader036/viewer/2022062702/554aeda4b4c905852a8b548a/html5/thumbnails/27.jpg)
Endoscopy
• Diagnostic & therapeutic tool
Interventions:• Clips• Banding• Thermocoagulation• Sclerosant injection % adrenaline
![Page 28: The Upper GI Bleeder](https://reader036.vdocuments.mx/reader036/viewer/2022062702/554aeda4b4c905852a8b548a/html5/thumbnails/28.jpg)
Balloon Tamponade
![Page 29: The Upper GI Bleeder](https://reader036.vdocuments.mx/reader036/viewer/2022062702/554aeda4b4c905852a8b548a/html5/thumbnails/29.jpg)
![Page 30: The Upper GI Bleeder](https://reader036.vdocuments.mx/reader036/viewer/2022062702/554aeda4b4c905852a8b548a/html5/thumbnails/30.jpg)
Balloon Tamponade
• Temporising rescue device
Indicated:– Endoscopy not available– Endoscopy not successful
• Need to secure airway prior
![Page 31: The Upper GI Bleeder](https://reader036.vdocuments.mx/reader036/viewer/2022062702/554aeda4b4c905852a8b548a/html5/thumbnails/31.jpg)
