Health Insurance – 30 day enrollment
Add spouse/domestic partnership (DP)/stepchildren/children to existing plan with coverage effective the date of marriage/DP
Enroll if currently not insured with coverage effective the date of event
Change from one health plan to another – requires two applications
One application to add spouse and dependents to current plan
Coverage effective the date of event
One application form to enroll in new health plan
Coverage effective the first of the month following receipt of the application form
*** Caution with Deductible and Out of Pocket Limits (OOPL)
Limits will restart if changing from one health plan to another
Prescription OOPL will transfer to new plan
ALL limits will restart if you staying with same health plan but are changing to/from HDHP OR changing from one HDHP to another HDHP
Cancel coverage if you are eligible to enroll under your spouse’s health plan

FMLA & WFMLA Provisions
FMLA and WFMLA allow you to take a job protected leave of absence from work. Time off for yourself or a family member, newborn, adopted child or to aid a family member called to active military service
FMLA: worked for the State at least 12 months (not required to be consecutive); and 1250 hours during the preceding 12-months. Only actual worked hours will count
WFMLA: worked for the State at least 52 consecutive weeks and 1000 hours worked during the preceding 52 week period (paid leave counts towards the 1000 hours)
FMLA and WFMLA leave runs concurrently
