THANKING OUR DONORSSUPPORTING A FUTURE OF INCLUSION AND OPPORTUNITY

On behalf of the ServiceSource Foundation Trustees and the individuals with disabilities who are

served by ServiceSource, we are both humbled and grateful to present the 2016 ServiceSource

Foundation Annual Report.

Since the Foundation’s inception in 2004, our donors have raised more than $9 million in support

of programs and people. In 2016, you helped us raise more than $650,000 in program funding and

more than $250,000 on payments to Capital Campaign pledge commitments.

Your contributions to the Foundation have provided gap funding for existing programs and

start-up funding for many programs that are needed but are not funded by any traditional funding

sources. Programs such as our Autism Spectrum Services, Warrior Bridge Veterans programs,

Family Support Services and our mentor and volunteer programs were all initially implemented with

support from the Foundation. Expressive therapy programs such as music, art, dance and yoga, in addition to therapeutic

horseback riding, have all been funded by the Foundation.

Your contributions to the Foundation are well spent with .97 of every dollar donated being used in direct support of

people and programs. As traditional funding for people with disabilities has continuously decreased, your support has

increased and become more and more essential. With your support, these programs were all made possible and you have

directly helped thousands of individuals receive services not otherwise available.

Thank you for your continued support. We look forward to achieving $10 million in donations as we launch new

initiatives that will impact people with disabilities for years to come.

FAMILY SUPPORT SERVICES

The Family Support Services program was initiated through a grant

from the Foundation in response to an identified need among

individuals and family members who were concerned for the long-

term future and support of their loved ones. Since May of 2015, the

program has served more than 250 individuals, providing critical

linkage to resources and direct support in gaining access to housing,

in-home supports, customized transportation training and assistive

technology assessments.

2016 FUND ALLOCATIONIn 2016, the Foundation raised $660,000

in program donations and $286,000 in

Capital Campaign pledge payments.

Nearly $500,000 was allocated to the following programs in 2016:

AssistiveTechnologyAssessmentsattheDisabilityResourceCenter.

HABILITATION & EXPRESSIVE THERAPIES

Since 2011, the Foundation has provided

more than $500,000 to fund expressive

therapies such as music, art, dance, yoga

and therapeutic horseback riding. Each

year, hundreds of individuals participate in

these critical therapeutic programs.

DOUBLE YOUR IMPACT THROUGH THE COMMUNITY ASSISTANCE ENDOWMENT $1 FOR $1 MATCH CAMPAIGN

The ServiceSource Foundation has embarked on a $1 for $1 match

campaign to create the Community Assistance Endowment. This

five-year campaign is goaled to raise $2.5 million in un-matched

dollars, which when combined with the dollar for dollar match, will

create a $5 million Endowment used in support of individuals and

their families for unique and specific needs.

Endowment grants may include the provision of assistive

technology, in-home supports, respite care, caregiver services,

counseling supports, educational services and certifications to

support independence and help individuals obtain employment.

FRIENDSHIP HOUSE FAYETTEVILLE

The ServiceSource Foundation is also excited to announce a $1.5

million Capital Campaign to create Friendship House Fayetteville.

Friendship House Fayetteville is an intentional, faith-based living

model that promotes community inclusion and independence for

young adults with disabilities.

This transitional housing program pairs people with intellectual

and/or developmental disabilities with medical/healthcare

professional students who live as roommates. People with disabilities

gain independence and learn independent living skills, while future

healthcare professionals gain empathy and understanding to better

treat their future patients.

The Foundation agreed to match up to $200,000 in donor

contributions to support the construction and operation of this

intentional living community.

NEWINITIATIVESIN2017

INUAGURAL $75,000 CONTRIBUTION TO CREATE A LEGACY

The NVTC Alumni, formerly Parents and

Associates of the Northern Virginia Training

Center, contributed a $75,000 gift to the

Endowment Campaign to create a legacy gift

that will provide assurance that their loved

ones and others with extraordinary needs will

continue to have access to some of the services

that were provided by the Center. Additional

funds were donated to support ServiceSource

programs that provide more immediate support

and shared resources for other service providers

and to write independent, multi-year annual

reports to identify unmet needs of individuals

with intellectual and developmental disabilities

in Northern Virginia.

Presidents of NVTC Alumni, stated, “When the Training Centers were closed and our loved ones were scattered across Virginia, the Parents and Associates group wanted to know that the money we had raised would be used to provide them with access to needed supports. The Endowment provides a way to double our impact and establish a legacy gift that will last long into the future and be available to help those with critical needs.”

VOLUNTEER SERVICES PROGRAM

The Foundation funded the start-up of the ServiceSource

Volunteer program 2016 to promote and leverage community

engagement opportunities and support mentor opportunities for

individuals with disabilities.

AUTISM SPECTRUM AND SENIOR SERVICES

The Foundation provided gap funding for ServiceSource’s

Autism Spectrum Services program and the adult Senior

Services program. ServiceSource operates fourteen senior

centers in Fairfax County, serving hundreds of senior citizens

daily and integrating seniors who have disabilities.
