PG COURSE - C2. LIPS: Surgical Skills and Suturing Techniques
Imaging is a crucial part of gynaecological practice. Gynaecologists performing endoscopic surgery should be able to perform gynecological ultrasound to diagnose benign and malignant diseases. The course aims to provide the necessary knowledge and practical skills to become a gynaecological ultrasound practitioner. The programme consists of brief lectures and hands-on-training sessions using transvaginal simulators, PC stations and ultrasound machines.
Laparoscopic suturing is not only a way to acquire a fundamental tool in laparoscopic surgery required when approaching more complex procedure but is also the only way to enhance our psychomotor skills outside the operative room. The objectives of the course are to acquire competences on instrument and techniques related to laparoscopic suturing in gynecological surgery.
The course is open to beginners and advanced surgeon, the hands-on will be tailored on the participant experience.

This course covers the basics of laparoscopic and vaginal procedures for the treatment of pelvic floor dysfunctions. It aims to exchange knowledge, skills and experience between surgeons that prefer one or the other approach. Both laparoscopy and vaginal surgery are characterized by minimally invasiveness and, ideally, a pelvic surgeon should have a portfolio of surgical procedures that would include both of them. The course tries to illustrate the tips and tricks of the most common procedures of vaginal and laparoscopic procedure with a final practical part with pelvic trainers.

Laparoscopic surgery for benign and deeply infiltrative diseases represents complex challenges for pelvic surgeons. Instructors from the International School of Surgical Anatomy (ISSA) of Verona, Italy will provide participants a comprehensive overview of the pelvic anatomy and guidance on how to navigate the retroperitoneal space and structure. These approaches will help surgeons to formulate a well strategized treatment plan leading to optimal surgical outcomes.
Learning Objectives: At the conclusion of this course, the participant will be able to:1) Recognize the anatomical landmarks and major pelvic structures pertinent to gynecologic laparoscopy and retroperitoneal dissection for basic, intermediate and advanced procedures;2) discuss laparoscopic surgical techniques to enter and expose avascular spaces of the pelvis, parametrial ligaments, nerves and pelvic vessels and their relations to the ureter and retroperitoneal structures;3) review the principles of a nerve-sparing;4) provide step-by- step dissection of the uterine artery and the pelvic ureter related to gynecologic retroperitoneal procedures (i.e. big uteri, intraligamentary myomas, deep endometriosis and gynecologic cancers).

Following the principles taught in the International School of Surgical Anatomy (ISSA), this hands-on cadaveric course will provide a step-by-step surgical approach to the pelvic viscera, retroperitoneal avascular spaces and pelvic ureters. Emphasis will be put upon identi-fying anatomical landmarks, including surgical principles and techniques to enter the retroperitoneal avascular spaces.Instruction on techniques for gentle tissue handling to avoid bleeding,proper traction, counter-traction, sharp and blunt dissections while preventing vascular, urinary, bowel and nervous complications. Special care will be given to nerve-sparing techniques during laparoscopicdissection, with demonstration of main pathways of visceral and somatic pelvic innervation. This course includes cadaveric specimen with intact uteri and cervix using laparoscopic instrumentation.

Suturing is a turning point towards Advanced Laparoscopic Surgery. Combination of the correct stitch and knot-tying can be responsible for the success of a complex surgery. Appropriate rules make difficult things easy. The Romeo’s Gladiator Rule put suturing at the hands of all laparoscopists. The knowledge acquired by Romeo’s Gladiator School explain which knots are safe and unsafe or dangerous and how to perform them.This course provides a broad comprehension about the art of laparoscopic suturing, mixing theory and practice.
